Book excerpt: Podemos definir al síndrome metabólico como el conjunto de alteraciones metabólicas y vasculares con punto de partida en la obesidad abdominal y/o la resistencia a la insulina que, a través de procesos inflamatorios, oxidativos y de disfunción endotelial, predisponen a un mayor riesgo de desarrollar diabetes y eventos cardiovasculares. Esta entidad hace coexistir en el mismo individuo varias patologías, consideradas todas como factores de riesgo cardiovascular, que constituyen las principales causas de muerte en nuestro medio, tales como diabetes mellitus tipo 2, hipertensión arterial y dislipidemia. La Encuesta Nacional de Enfermedades Crónicas (ENEC), realizada en 1993, reportó una prevalencia de 26.6% en mayores de 18 años; la Encuesta Nacional de Salud y Nutrición (ENSANUT), realizada en 2006 y publicada recientemente, nos dio una prevalencia de 36.8% utilizando criterios del Adult Treatment Panel (ATP) III. Esto traduce un incremento de 38% en esos 13 años; sin embargo, si empleáramos los criterios de la International Diabetes Federation en la misma ENSANUT, la prevalencia se incrementa a 49.8%, casi la mitad de los adultos mexicanos. Por todo ello, decidimos que un libro como el que usted tiene en sus manos era necesario; el síndrome metabólico afecta cada vez a más mexicanos y se constituye como una causa importante de muerte e invalidez en nuestro país. No busca ser un libro más sobre el padecimiento, por eso no se desarrollan temas que comúnmente se ven en textos sobre el síndrome; nuestra intención es que sea una orientación sobre algunos aspectos habitualmente no tocados en los libros sobre el síndrome metabólico, aquellos puntos sobre los que no hay consenso, hay evidencia reciente que echa por tierra algunos manejos o bien demuestra que hay mejores resultados con otras terapias, o que cuestiona las metas terapéuticas establecidas, como el recién publicado estudio ACCORD, que nos obliga a cuestionar hasta dónde reducir las cifras tensionales en el paciente diabético. Por esa razón el título de esta obra, Controversias en medicina interna. Síndrome metabólico, en la que los capítulos fueron desarrollados por médicos mexicanos, la mayoría miembros de nuestro Colegio de Medicina Interna de México, pero todos ellos dispuestos a compartir su experiencia y amplio conocimiento del tema con los lectores, para lograr este volumen que, con el aval del Colegio de Medicina Interna de México, hoy ponemos a la consideración de los internistas mexicanos, y que estamos seguros será también de utilidad a los médicos generales y de otras especialidades que también se esfuerzan por brindar mejores expectativas de vida a estos pacientes.

Se sabe que hay una serie de factores que son compartidos por todas ellas, designados como factores de riesgo, de los que en la mayoría de los casos están presentes más de uno en un solo individuo. Algunos de estos factores o enfermedades que se asocian son la obesidad, la diabetes tipo 2, la hiperlipidemia y la hipertensión, todo lo anterior aparentemente unido por lo que llamamos resistencia a la insulina, y que conforman el llamado síndrome metabólico. En el resumen de noticias de la Organización Mundial de la Salud del 19 al 23 de septiembre del año 2005 se lee una nota cuya autora es Catherine Le Galës Camus, Directora General Adjunta de Enfermedades No Contagiosas y Salud Mental, en la que sobresalta el hecho de que hay 17 millones de personas que mueren cada año a causa de la obesidad. En ese entonces se estimaba la existencia de más de 1 000 millones de personas obesas, calculándose que la tendencia incrementaría la obesidad en 50% para el año 2015. Lo peor es que la obesidad no se considera únicamente un problema de países ricos, lo cual augura una carga en materia de salud pública que no será exclusiva de países en desarrollo sino de otros más, como es el caso de México. Los posibles factores que predisponen a la obesidad son el cambio de la dieta a nivel mundial, con aumento del consumo de grasas y carbohidratos, disminución de la actividad física debido al trabajo sedentario, la urbanización y condiciones de la vida moderna. Antes de concluir, sólo quiero expresar mi más sincero reconocimiento a los escritores que han colaborado con nosotros por su tiempo y su interés en generar alternativas para la difusión de los conocimientos médicos, con el fin último de dar un mejor servicio a nuestros pacientes.

En 1979 el Grupo Nacional de Información en Diabetes de EUA y más adelante la Organización Mundial de la Salud (OMS) introdujeron el término de intolerancia a la glucosa para definir lo que anteriormente se conocía como diabetes limítrofe, que en la actualidad constituye uno de los principales indicadores de prediabetes. Posteriormente la Asociación Americana de Diabetes (ADA) y la OMS introdujeron un nuevo concepto de alteración de la glucosa en ayuno, el cual complementa el criterio de definición de prediabetes. Conceptualmente la prediabetes, aunque parezca Perogrullo, antecede a la diabetes. Define un estadio dentro de la historia natural de la enfermedad que puede evolucionar hacia la diabetes, pero que también puede regresar a un estadio de tolerancia a la glucosa normal. Como se mencionó, los sujetos con prediabetes no sólo tienen un mayor riesgo de desarrollar diabetes en el futuro (antecesora de la diabetes), sino que tienen un mayor riesgo de enfermedad cardiovascular, considerada como la ocurrencia de cardiopatía isquémica, enfermedad vascular cerebral o periférica, o aterosclerosis en general.
