Book excerpt: Background and AimsWe aim to present a modern approach for the treatment of children with short bowel syndrome (SBS).Method45 patients with SBS aged 1 months to 14 years were treated in the department of Pediatric Surgery at Russian Children's Clinical Hospital. We have a pediatric home parenteral nutrition (HPN) program for the patients of the first year of life, that receive PN before and after intestinal reconstruction surgeries. 33 patients underwent serial transverse enteroplasty (STEP) with our own modification, five children with complicated variants of SBS (extreme SBS) went through repeated STEP procedure. In three patients with extreme SBS that are stable following a period of postsurgical intestinal adaptation, the teduglutide therapy was initiated.ResultsEleven children remain off PN 32 months after surgery; fourteen children after intestinal reconstraction continue to receive reduced PN 2-4 times a week - all with reassuring growth and nutritional status. Applying routine preventive therapy for all patients with SBS, we were able to minimize the number of complications such as central line-related occlusion/thrombosis, intestinal failure-associated liver disease, gallbladder sludge and stone formation, renal failure and metabolic bone disease. All fatal outcomes (9,3%) were caused by central line-related infections. The results of teduglutide administration will be analyzed 12 weeks after the initiation of the therapy.ConclusionPediatric SBS is a complex clinical problem requiring coordinated multidisciplinary approach. Modern types of treatment create a promising future for pediatric patients with short bowel syndrome that helps to avoid bowel transplantation, reduce PN and regain enteral autonomy.
