Book excerpt: Introduction: There is a growing demand for esthetic appliances during orthodontic treatment, with a specific interest in WildSmiles shaped brackets amongst children and adolescents. Bond failure of brackets over the course of orthodontic treatment can result in increased treatment time, increased overhead costs, and feelings of frustration for the patient and practitioner. Objective: The purpose of this study was to the evaluate the effect of orthodontic bracket base shape on shear bond strength (SBS) to human enamel, in vitro, at two time points, as well as the nature of debonding fractures using the Adhesive Remnant Index (ARI). Methods: Orthodontic brackets with different shaped bases (flower, heart, rectangular) were bonded to 120 extracted human third molars. SBS was measured using a Servohydraulic Test System at 24 hours and 2 months after initial bonding. ARI scores were evaluated under 10x magnification to assess the amount of resin left on the tooth. Results: The control bracket with a rectangular base shape had the highest mean SBS (26.78 ± 8.19 MPa), and significantly differed from flower (17.19 ± 4.45 MPa) and heart (18.86 ± 3.50 MPa) base shapes (p 0.001). The mean SBS of flower and heart shaped brackets did not differ considerably from each other (p 0.05). The mean SBS between debond times at 24 hours (21.50 ± 7.46 MPa) and 2 months (20.38 ± 6.69 MPa) were not statistically significant (p > 0.05). Analysis of ARI scores showed a significant difference between Flower-24 hours vs Heart- 2 months (p=0.039), Flower-24 hours vs Heart- 24 hours (p=0.004), and Control-2 months vs Heart-24 hours (p=0.015). The heart shaped bracket had a greater proportion of adhesive bond failures at the enamel-resin interface. Conclusion: Bracket base shape has an effect on SBS, with the rectangular base shape having a higher mean SBS compared to flower and heart base shapes. All bracket shapes had bond strengths above the clinically acceptable range of 6-8 MPa, and thus may provide adequate SBS in a clinical situation. Variations in ARI scores occurred based on bracket shape and were of a mixed adhesive-cohesive nature.

Book excerpt: Demineralization of enamel has been one of the iatrogenic effects of fixed orthodontic therapy, for which various authors have suggested the use of fluoride to reduce the decalcification and development of white spot lesion. In the present study, the shear bond strength of light-cure composite adhesive, self-cure composite adhesive, light-cure glass ionomer cement and acidic primer were tested at the end of 24 hours after bonding. The results proved that the acidic primer system had highest shear bond strength while Light-cure glass ionomer system had the least shear bond strength. But the mean values met the minimal shear bond strength required for orthodontic bonding. After bond failure, least amount of glass ionomer cement was present on the enamel surface, followed by acidic primer system, light-cure composite and self-cure composite system. So, it is advantageous to use light-cure glass ionomer cement for bonding in terms of less amount of time spent and no enamel damage during cleaning procedure after debonding.

Current literature on the effects of thermocycling on bond strength is equivocal. This is largely due to a lack of a universally accepted standardized technique to examine the effects of thermocycling on orthodontic bond strength. As a result, differences in testing methodology between laboratories and investigators within the same laboratory make it difficult to directly compare data from thermocycling studies. The aims of this study were (1) to show that the testing method developed at the University of Toronto is independent of the investigator; (2) to determine the effect of thermocycling on the shear bond strength of orthodontic brackets bonded to bovine enamel; (3) to determine the effect of long term storage on the mean shear bond strength of orthodontic brackets bonded to bovine enamel; (4) to examine the bond failure interface in order to determine if a relationship exists between thermocycling and the location of the cement after debonding; (5) to examine the bond failure interface in order to determine if a relationship exists between long term storage and the location of the cement after debonding. This book describes a study undertaken to see the effects of ph of self-etchant on shear bond strength of orthodontic brackets. These shear bond strengths were also compared to those obtained with a conventional 3-step bonding system. It was found that self-etching primers not only reduce the chair time in an orthodontist's clinic but also reduce the amount of enamel lost during etching. The bond strength achieved was less than that of the conventional bonding system but was acceptable for orthodontic purposes. Thus self-etching primers are an important and useful invention for simplifying the most critical step in orthodontic clinic- bonding.

Book excerpt: The purpose of the current study was to develop a bonding method that can achieve clinically acceptable bond strength values while leaving a minimum amount of adhesive on the tooth surface during the debonding process. One hundred teeth were randomly assigned into groups. Five different enamel surface preparation protocols were tested (N = 20, each): Conventional acid etch, standard SEP, SEP applied with a light brush stroke (altered SEP 1), SEP applied directly to the composite of pre-coated bracket (altered SEP 2), and Primer Only groups. Brackets were debonded using an Instron universal testing machine (Instron, Canton, MA) in shear mode and the mean shear bond strength values were calculated. In addition, enamel surfaces were examined under light microscope to determine the location of failure using ARI. The Primer Only group had significantly lower mean shear strength (0.14 MPa, P0.05) than the conventional acid etch (13.81 MPa), standard SEP (12.10 MPa), altered SEP 1 (10.80 MPa), and altered SEP 2 (11.48 MPa) groups. The conventional acid etch, standard SEP, altered SEP 1 and altered SEP 2 groups were not significantly different from each other (p0.05). With respect to ARI values, there was a significant difference among these four groups. 85% of samples in the altered SEP 2 group had 10% or less composite resin left on their surfaces. This group had also a mean shear bond strength value of 11.43 MPa, significantly above the minimal strength needed for orthodontic attachment bonding, and the lowest ARI values overall. Therefore, application of SEP directly to the composite resin of the pre-coated brackets may be an ideal bonding method by providing adequate bond strength and leaving a minimum amount of composite resin on the tooth surface during debonding. It should be kept in mind that future in-vivo studies would be needed to confirm the findings obtained from the current in-vitro study.

Debonding force was measured with an Instron universal testing machine. A two-way ANOVA comparing the four groups indicated that there was a significant difference in debonding force (P = 0.001). The SEP without prior pumicing group (17.69 " 7.18 MPa) was statistically different from the SEP group with prior pumicing (25.82 " 6.84 MPa). There was no statistical difference found among the other groups. Differences in the Adhesive Remnant Index (ARI) were analyzed by chi-square. ARI scores differed significantly (P =0.0048).
