Book excerpt: One in five people in the United States had a sexually transmitted infection (STI) on any given day in 2018, totaling nearly 68 million estimated infections. STIs are often asymptomatic (especially in women) and are therefore often undiagnosed and unreported. Untreated STIs can have severe health consequences, including chronic pelvic pain, infertility, miscarriage or newborn death, and increased risk of HIV infection, genital and oral cancers, neurological and rheumatological effects. In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.

In 2016, approximately 20 million newly diagnosed cases of sexually transmitted infections (STIs) were estimated to occur yearly. Individuals between the ages of 15 to 24 years old would have accounted for half of these newly diagnosed cases. The use of retail clinics may assist with this pandemic issue, since they are convenient, easily accessible, offer extended office hours, and have low wait times. Methods. The EBP project used a three-step process that consisted of: a pre-test self-administered survey distributed via Survey Monkey℗ʼ, followed by an online educational PowerPoint Presentation regarding STI education incorporating the Centers for Disease Control and Prevention (CDC) 2015 guidelines, then a repeated post-test for knowledge retention. The project also compared two post-test scores. Results. The project increased nurse practitioner (NP) STI knowledge, which was the first outcome of the EBP project. Indeed, one NP had a 20% increase in total score percent change after the educational component of the EBP project. The second benchmark was an 80% on the post-test scores and was achieved by 100% of the participants. The second outcome was NP confidence in applying STI knowledge to their routine healthcare practices. Fifteen (35%) participants had an increase in confidence scores, one (20%) participant had a decrease in confidence, and twenty-seven (63%) had no change in their first post-test confidence scores. Conclusions. The project was sent to 151 NPs in the state of Virginia. The first phase of the project had a 34% completion rate. The second phase of the project had a 13% completion rate.
