In this 2009 report, the Institute of Medicine's Committee on Comparative Effectiveness Research Prioritization establishes a working definition of CER, develops a priority list of research topics, and identifies the necessary requirements to support a robust and sustainable CER enterprise. As part of the 2009 American Recovery and Reinvestment Act, Congress appropriated $1.1 billion in federal support of CER, reflecting legislators' belief that better decisions about the use of health care could improve the public's health and reduce the cost of care. The Committee on Comparative Effectiveness Research Prioritization was successful in preparing a list 100 top priority CER topics and 10 recommendations for best practices in the field.

How do we decide where funding for health-care programs should go? Tannsjo here approaches the subject from a philosophical perspective, balancing theoretical treatments of distributive ethics with real-world examples of how health-care is administered around the world today. Tannsjo begins by laying out several popular ethical theories-utilitarianism, which recommends maximizing the best overall outcome; egalitarianism, which recommends smoothing out the differences between people as much as possible; and the maximin/leximin theory, which urges people to give absolute priority to those who are worst off. Tannsjo shows how, in abstract thought experiments, these theories come into conflict with each other and reveal puzzling implications. He goes on to argue, however, that when we consider health-care in the real-world, these theories all agree on a central point: in a well-ordered welfare state, more resources should be directed to the care and cure of people suffering from mental illness, and less to the marginal life extension of elderly patients. Tannsjo's book thus recommends a shift in spending to increase fairness and overall utility-while also recognizing that this kind of dispassionate suggestion, with its purely economic foundation, is unlikely to take hold in policy. Tannsjo's analysis is a case study in how ethical theories can sometimes lead to rational conclusions and recommendations that we are not prepared to accept.

In chapter two, the authors establish seven criteria that can help planners and programme administrators decide whether screening will contribute to the aims of primary health care in a particular situation. Chapter three on the planning and implementation of services alerts readers to several important issues that frequently arise at the local level when screening services are implemented. Various proposals for mandatory HIV screening are used to illustrate legal and ethical issues. In a key achievement, chapter four sets out a method for the systematic assessment of screening programmes and the setting of priorities. Focused on the questions of why, who, how, and when, the method is intended to help decision-makers assess local conditions, think through resource needs for diagnosis and intervention as well as screening anticipate problems, and then select the most appropriate preventive option. Having established a framework for setting priorities, the remaining chapters issue recommendations for a large number of screening options of potential value in primary health care programmes.
