Book excerpt: People with Parkinson's disease (PD) have known deficits in striatal dopamine, and compared to age-matched healthy older adults, increased striatal deficits, and difficulties with maintaining postural stability and complex language comprehension, behaviors that involve sequencing. Because sequencing abilities are important for daily life and for various types of rehabilitation, this decline with PD should be well understood, yet the literature on the effects of PD in sequence learning is mixed. Some of the confusion in the literature may be due to variation in participant characteristics, but also to task differences, such as the extent of motor movement required. The present studies examined implicit sequence learning using the Triplets Learning Task (TLT) in a PD group. There were 29 medicated PD participants, and 30 healthy older adults were recruited as a control group. Study 1 compared learning over several blocks of training in the TLT. Two learning measures indicated that people with PD learned, but they showed less implicit sequence learning than Controls. This group difference in one analysis was carried largely by late training, when learning is thought to be particularly dependent on the striatum. Additionally, when first and second halves of blocks were examined separately, the PD group showed less learning than Controls in the second halves of blocks, when learning is thought to be striatal-dependent. In Study 2, we examined relationships between learning and PD-related characteristics. Using learning in the second half block, we found that in our PD group, learning positively correlated with Age, Disease duration and Levodopa levels. These three variables were positively correlated with one another, and therefore it was difficult to disentangle their individual effects. Together, these studies suggest that people with PD were able to learn implicit sequences, though to a lesser degree than Controls, and Levodopa medication may have helped moderate this sequencing deficit. Findings of group differences in learning in sections of training suggest that particular points in training may be more sensitive to sequencing or striatal deficits. Thus, the TLT and these learning analyses could be used to better characterize clinical populations that may have hippocampal or striatal deficits.

Additionally, data suggest normal aging can affect various stages of motor learning. Though data are available on the nature of non-speech motor learning, fewer studies have examined how adults with and without motor speech disorders learn speech articulatory sequences. The aim of the current investigation is to examine speech motor sequence learning in normal aging and PD. A speech motor sequence learning task was developed and implemented to examine acquisition, consolidation, and automatization over two days. On Day 1, participants learned a sequence of six monosyllabic nonwords to examine acquisition. This speech motor sequence was re-tested on Day 2 in single- and dual-task conditions to evaluate consolidation and automatization, respectively. Learning was inferred from gains in speed and accuracy of the speech motor sequence. There were no observed effects of aging or PD on speech motor sequence acquisition, as all groups exhibited comparable gains in speed and accuracy on Day 1. However, aging and PD were observed to affect speech motor consolidation following night-time sleep. Specifically, at the Day 2 re-test, young adults exhibited offline gains in speed and accuracy, older adults maintained speed and accuracy (without offline gains), and individuals with PD exhibited offline losses in speed. Only younger adults demonstrated automatization, exhibiting negligible dual-task effects when concurrently performing the speech motor sequence with a visuo-motor task. Conversely, both older adults and individuals with PD exhibited significantly poorer performance of the speech motor sequence in the dual-task condition, with individuals with PD experiencing the largest degree of bidirectional dual-task interference. Overall, the current findings suggest that speech motor learning is affected by both Parkinson Disease and normal aging. Data from the current investigation are largely in line with findings in the non-speech motor sequence learning literature, suggesting that speech and non-speech motor learning may rely on similar neural circuits. As the individuals with PD exhibited deficits in the consolidation and automatization phases of learning, these data may hold important implications for the speech rehabilitation in PD.

Book excerpt: To survive in a complex changing environment humans frequently need to adapt their behaviour incidentally from normal interactions in the environment without any specific intention to learn. Whilst there is a considerable body of research into incidental learning of sequential information there is still fundamental debate regarding its cognitive basis, the associated neural mechanisms and the way in which it is affected by neurological disease. These issues were explored, in normal participants and neurological patients, using manipulations of the Serial Reaction Task [SRT] in which participants gradually learn a stimulus sequence (usually screen locations) after responding to each item by pressing corresponding response buttons. The first two experiments (chapter 3) demonstrate that the specific metric used to quantify learning and the occurrence of highly salient repeat locations may inflate estimates of learning in tasks with increased motor demands. The next three experiments (chapter 4) examine whether a secondary (not directly behaviourally relevant) information source during the SRT facilitates chunking in memory and overall learning. In a spatial SRT task (specified by horizontal location), additional spatial information (vertical location) enhanced learning but a secondary perceptual property (colour) produced a cost. However, in a perceptual SRT a secondary perceptual property (colour) had no effect. The next study demonstrates that impairments of incidental learning in Parkinson's disease are partially reduced by administration of l-Dopa. Implications for models of striatal function and studies suggesting implicit learning is impaired by l-Dopa are discussed. Finally, the impact of Deep Brain Stimulation of the GPi is investigated in a population known to have only limited cognitive deficits relating to their illness (dystonia). Despite previous reports of impaired intentional learning in participants with a high genetic risk of Dystonia, there was no evidence for any impairment before or after stimulation. Implications across studies and future research directions are also discussed.
