Book excerpt: Employers that offer health insurance to their employees can provide that coverage either through self-insured plans or through fully-insured plans purchased from an insurance company. When an employer chooses to self-insure coverage, it generally takes on the financial risk associated with paying the plan's costs although employers with such plans commonly purchase stop-loss coverage to limit their financial exposure. Employers that sponsor fully-insured plans generally pay premiums to an insurer and the insurer bears the financial risk. Self-insured and fully-insured coverage are not treated the same way under the law. State laws that regulate fully-insured employer-sponsored plans usually do not apply to self-insured plans. And some provisions of the federal Patient Protection and Affordable Care Act of 2010 pertain to fully-insured plans but not to self-insured plans. Larger firms often offer a choice of health plans with different benefits and costs to their employees--the options may include both self-insured and fully-insured plans. For example, a self-insured Preferred Provider Organization (PPO) may be offered at all sites nationwide while different fully-insured Health Maintenance Organizations (HMOs) are offered at various work locations in different states. This Statistical Brief is limited to an analysis of enrollment in self-insured health insurance plans. Of the total 56.1 million employees with employer-sponsored coverage in the private sector in 2010, 32.3 million were enrolled in plans that were self-insured by their employers. The Brief discusses self-insurance enrollment rates for small versus large employers and across industry groups in 2010, and changes since 2000, for private sector firms. This analysis is based on estimates from the Insurance Component of the Medical Expenditure Panel Survey (MEPS-IC). Only comparisons with a statistically significant difference at the 0.05 percent significance level are noted in the text.

State-level data are also presented, along with the correlation between state mandates and the prevalence of self-insurance. Offering a self-insured plan means the employer assumes the financial risk related to offering health insurance (as opposed to a fully insured plan, where the insurance company assumes the risk). Large employers are much more likely to offer health benefits on a self-insured basis than small employers. However, there is speculation that passage of the Patient Protection and Affordable Care Act of 2010 (PPACA) will result in an increasing number of smaller employers offering self-insured plans. Employers think that components of PPACA, such as the strict grandfathering requirements; the minimum-creditable-coverage requirement; the breadth of essential health benefits; taxes on insurers, medical-device manufacturers, and pharmaceutical companies; affordability requirements; and reinsurance fees will all drive up the cost of health coverage. Small employers concerned about the rising cost of providing health coverage may view self-insurance as a more attractive means to mitigate any expected cost increases. Some key findings include the following: The percentage of workers in private-sector self-insured health plans has been increasing. In 2011, 58.5 percent of workers with health coverage were in self-insured plans, up from 40.9 percent in 1998. Large employers (with 1,000 or more workers) have driven the upward trend in overall self-insurance. The percentage of workers in self-insured plans in firms with fewer than 50 employees has been close to 12 percent in most years examined. The prevalence in self-insured plans varies by state, with Massachusetts having the third-highest prevalence of self-insurance in the small-group market (behind Hawaii and Alaska). Overall, 58.5 percent of workers were in self-insured plans in 2011, but the percentage ranged by state, from a low of 30.5 percent to a high of 73.8 percent. Massachusetts, the only state to have enacted health reform similar to PPACA, has seen an increase in the percentage of workers in self-insured plans among all firm-size cohorts, except among workers in firms with fewer than 50 employees. (In 2016, the percentage of midsized establishments offering a self-insured plan fell from 30.1 percent to 29.2 percent.) • Between 2013 and 2016, the self-insurance trend for large establishments continued to decline, falling from 83.9 percent to 78.5 percent. • Because many more employees work for large establishments, the increase in self-insurance among small establishments (and their workers) was not large enough to offset the decline among large establishments (and their workers), resulting in a decrease in the percentage of covered workers enrolled in self-insured plans. o Between 2015 and 2016, the percentage of enrollees in self-insured plans fell from 60 percent to 57.8 percent.

Since the passage of the ACA, some commentators have speculated that an increasing number of small and midsized employers would convert their health plans from fully insured to self-insured plans. The rationale has appeared to be that several of the key ACA components -- creditable coverage, affordability, essential benefits, and various taxes and fees -- would drive up the cost of health coverage, thus possibly making self-insurance (which is viewed by many as generally less expensive than fully-insured alternatives) a more attractive option for many employers. The percentage of private-sector establishments offering health plans at least one of which is self-insured has increased from 28.5 percent in 1996 to 39 percent in 2015 (36.8 percent increase). Between 2013 and 2015, the percentages of establishments offering health plans with at least one self-insured plan has increased for midsized establishments from 25.3 percent to 30.1 percent (a 19 percent increase); for small establishments from 13.3 percent to 14.2 percent (a 7 percent increase); and has decreased from 83.9 percent to 80.4 percent for large establishments (a 4 percent decrease). Similarly, the percentage of health-plan-covered workers enrolled in self-insured health plans has increased from 58.2 percent to 60 percent (a 3 percent increase) from 2013 to 2015. The largest increases in self-insured plan coverage among covered workers have occurred in establishments with 25-99 employees and with 100-999 employees. These data are consistent with the perspective set forth above that the ACA would cause more small and midsized employers to adopt self-insured plans.

In 2013, 37.6 percent of private-sector establishments self-insured at least one of their health plans, up from 26.5 percent in 1999. All of the growth in self-insurance in recent years has been confined to larger firms. In 2013, 83.9 percent of employers with 500 or more employees self-insured at least one health plan, up from 66.2 percent in 1999. The percentage of employers offering a self-insured health plan with fewer than 100 employees has bounced around between 10.7 percent and 14.7 percent between 1996 and 2013. Furthermore, the percentage of employers offering a self-insured health plan with 100-499 employees fell from 35.3 percent in 1996 to 25 percent in 2006, and has been between 25.2 percent and 29.2 percent since. Large employers (with 1,000 or more workers) have driven the upward trend in overall self-insurance. Understanding the trend in self-insurance for employers with 50 or more workers and those with fewer than 50 workers is important because the employer mandate in the Patient Protection and Affordable Care Act (PPACA) affects only employers with at least 50 workers. To the degree small employers are concerned about the rising cost of providing health coverage, self-insurance may become a more attractive means to mitigate any expected regulatory cost increases. However, as of 2013, there is no evidence that smaller firms were increasingly self-insuring their health plans. In 2013, 64.6 percent of firms with 50 or more employees offered at least one self-insured plan, compared with 13.2 percent among firms with fewer than 50 employees. This analysis finds that the percentage of workers in self-insured plans has been increasing. In 2013, 58.2 percent of workers with health coverage were in self-insured plans, up from 40.9 percent in 1998. The percentage of workers in self-insured plans in firms with fewer than 50 employees has been close to 12 percent in most years examined in this analysis. The estimates in this analysis should serve as a baseline to gauge the potential impact of rising health insurance costs on self-insurance in the future.

Non-single health insurance plans encompass two types of coverage: family coverage and employee-plus-one coverage. Prior to 2001, MEPS-IC collected only information on family coverage plans. Information related to coverage for a family of four was collected in cases where multiple pricing levels were offered to employees. Beginning in 2001, separate data were also collected for employee-plus-one coverage. Not all employers who offer family coverage provide the alternative of an employee-plus-one policy for employees with just one family member to insure. However, when available for the same level of benefits, employee-plus-one plan premiums are less expensive than family plans. This Statistical Brief presents estimates on the selection and cost of single, employee-plus-one, and family health insurance coverage in 2006.
