A lack of activity increases the risk of heart disease, colon and breast cancer, diabetes mellitus, hypertension, osteoporosis, anxiety and depression and others diseases. Emerging literature has suggested that in terms of mortality, the global population health burden of physical inactivity approaches that of cigarette smoking. The prevalence and substantial disease risk associated with physical inactivity has been described as a pandemic. The prevalence, health impact, and evidence of changeability all have resulted in calls for action to increase physical activity across the lifespan. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. This report will be of interest to local and national policymakers, school officials, teachers, and the education community, researchers, professional organizations, and parents interested in physical activity, physical education, and health for school-aged children and adolescents.

Book excerpt: The purpose of this dissertation was to investigate determinants of, and approaches to promote, physical activity and aerobic fitness among Wisconsin middle school children. The approach used the socio-ecological model, which posits that health behaviors are influenced by factors atvarious levels. These levelsrangefrom the individual to the societalrealm, and interventions utilizing multiple levels can be effective towards promoting healthy practices. The first paper showed that aerobic fitness holds a marginally significant association with future physical activity. The second paper demonstratedthat, on the population level, disparities exist in aerobic fitness between race/ethnicity groups among 6th grade students. Much more limited disparities exist for activity levels. The third paper showed that it is feasible to implement school-based strategies to promote physical activity and aerobic fitness among middle school children. However, in our results, the implementation did not translate into significant positive changes in activity or fitness between years. Overall, these findings have implications for future research and current practice. First, improving students' fitness levels may result in increased activity levels both in and out of school. Second, a link exists between poor aerobic fitness and insulin resistance, a marker for the development of type 2 Diabetes Mellitus. The disparities in fitness levels suggest that emphasizing increased aerobic fitness in schoolsmay prevent chronic disease disparities in future adult generations. Finally, addressing fitness and activity at the school-level can provide widespread population health benefits given the high level of reach, adoption, and implementation observed in our study.

Book excerpt: A large body of evidence suggests physical activity is inversely associated with several cardio-metabolic risk factors among children and adolescents. Despite these health benefits, a majority of youth are not meeting the physical activity guidelines set forth by the USDHHS. Schools have been identified as an ideal vehicle for interventions; however, research evidence indicates school-based interventions are not effective at increasing outside of school physical activity. Goal setting may be a potential effective strategy for increasing physical activity among youth; however no previous studies have examined the effects of goal setting on cardiorespiratory fitness and physical activity in middle school students. Therefore, the purpose of this study is to evaluate the effects of a S.M.A.R.T. goal setting and self-monitoring intervention on fitness and physical activity in middle school students. Two middle schools in Tallahassee, FL participated in this study. One school served as the intervention and the other served as a delayed intervention measurement only control. The students in the intervention school completed a one-time S.M.A.R.T. goal setting lesson. During the lesson, students were taught the definition of a goal, the importance of goal setting, and how to set S.M.A.R.T. goals. As part of the lesson, students completed a S.M.A.R.T. goal setting worksheet. The objectives of the worksheet activity were to teach students the concepts of S.M.A.R.T. goal setting and to apply this knowledge by creating personal fitness goals. Student fitness goals were entered into an interactive website that acted as a self-monitoring tool. Before and after the intervention, participating students completed a survey assessing student demographics, physical activity, and physical activity self-efficacy. Cardiorespiratory fitness levels, assessed by the PACER test, were also measured pre and post. Betweengroup differences in post-test scores, adjusted for baseline levels, were assessed for statistical significance using ANCOVA. Additional covariates included gender, race/ethnicity, grade level and weight status. Book excerpt: The purpose of this cross-sectional exploratory study was to examine the relationships among self-efficacy, stage of change, and exercise behaviour in a sample of younger (Grade 9) and older (Grade 12) adolescents. A secondary objective of this study was to apply the transtheoretical model of Stage of Change, as a measure of intention to change, in order to discover the applicability of the model to an adolescent cohort in relation to exercise behaviour. This five-stage model is a self-report measure of an individual's readiness to adopt a new behaviour (e.g., regular exercise). The transtheoretical model incorporates Bandura's self-efficacy factor, which is purported to be a predictive measure of exercise behaviour and a covariant of stage. Exercise behaviour was measured with the Physical Activity Scale, and the University of Rhode Island Change Assessment Scale (URleA) was used to measure the stage of change and self-efficacy variables. The results of this study indicated significant differences between younger and older adolescents, and between males and females in their exercise behaviour. No significant differences were found for grade and gender on stage of change as measured by either a single-item question or a continuous measure of stage. Although grade and gender subgroups were not significantly different in their self-efficacy, significant interaction was found in the grade*gender variable.

Using appropriately selected measures to collected fitness data in youth will advance our understanding of how fitness among youth translates into better health. In Fitness Measures and Health Outcomes in Youth, the IOM assesses the relationship between youth fitness test items and health outcomes, recommends the best fitness test items, provides guidance for interpreting fitness scores, and provides an agenda for needed research. The report concludes that selected cardiorespiratory endurance, musculoskeletal fitness, and body composition measures should be in fitness surveys and in schools. Collecting fitness data nationally and in schools helps with setting and achieving fitness goals and priorities for public health at an individual and national level.
