Twelve years after the first appearance of x-ray CT, computerized diagnostic imaging has grown so rapidly in sophistication that it is difficult to follow current developments in this diversified field. In this book, we have attempted to cover the basic developments in several areas. The first part includes some of the fundamental aspects of computerized diagnostic imaging such as algorithms and detectors. Specific applications in emission tomography, digital radiography, ultrasound and nuclear magnetic resonance imaging are dealt with in the secondpart. The contributed papers are by experts currently in the field, whom we feel would certainly enlighten the subject matter and possibly suggest directions for future development. We would like to express our sincere thanks to those who have contributed to this volume. Bypass surgery as well as transluminal coronary angioplasty would not have been possible without coronary angiography serving as a 'midwife' in their evolu tion. Despite the widespread and long-standing use in clinical practice, however, the interpretation of coronary angiograms has not changed very much since the early days. Most angiogr~s are still reviewed in a visual and semi-quantitative and thus often very subjective way. In the face of an almost exploding field for interventional catheterization including thrombolysis, balloon dilatation, and other rapidly evolving techniques for transluminal revascularization or recanalization, a more detailed and quantitative analysis of coronary arteriograms is urgently required. In addition to the delineation of coronary morphology, we need dynamic and functional information about flow and perfusion to understand the physiological significance of anatomic abnormalities. Coronary arteriography contains and can provide most of this information. With the application of appropriate techniques, it can be made available in the catheterization laboratory even during the patient's investiga tion, thus facilitating and improving clinical decision making. Objective and reproducible analysis will furthermore enhance our understanding about the pathophysiology of coronary disease.

Over the years some· authors have been very optimistic and put forward opinions which motivated the common press to coin the phrase 'Dr. med. computer'. Papers printed under this heading rebuffed the majority of physiCians for many years. Today we know that medical decision making is a most complex buman performance. And 30 years of research on decision support have given us only limited insight into the underlying processes. Most of the principal methodological questions were already asked very early on.

It follows the previous congresses in Cambridge (1978), Berlin (1979), Toulouse (1981), Dublin (1982), Brussels (1984), Helsinki (1985), Rome (1987), Oslo (1988), and the Congress 1990 in Glasgow. The proceedings contain 199 contributions to the MIE 91 Congress. They cover all presentations which are part of the scientific programme of MIE 91, among them 157 paper presentations with an average of five pages, 28 poster presentations again with an average of five pages, and 14 abstracts of demonstrations with an average of one page. The papers included were selected by an International Programme Committee out of over 300 submissions after careful review by at least two international reviewers (for whose estimable efforts we are especially thankful). The recommendations of the re viewers were incorporated in the final texts. Some papers were reworked by a professional translator to obtain a high quality of presentation. Several submissions could not be considered for presenta tion at MIE 91 because of shortage of congress time and limitations in the number of pages of the proceedings.

As a result of the discussions during the workshop, a proposal was prepared for the establishment of a concerted action, specifically addressing the development of guidelines for the evaluation of medical decision aids. This proposal was granted early 1990 under the same Medical and Health Research programme of the EC. Over 40 institutes are participating in this concerted action. It have been the outstanding presentations and the open discussions at the workshop that have been the starting point of this concerted action. The papers in this proceedings formed a starting point for the discussions in the meetings of the concerted action.

Book excerpt: Operations research aims to assist managers faced with problems of coordinating activities; improving the quality of care of services delivered; making optimal resource allocation decisions and generally, managing services and institutions. Operations research (or O.R.) was originally developed in response to the problems of the second World War. It was characterised then by a unifying and clear objective; clear problems that had to be solved and the use of inter-disciplinary teams to analyse and solve identified problems. This analysis often drew on mathematical techniques. After the war, operations research moved in two separate but related directions. In England, the emphasis on inter-disciplinary approaches and problem solving teams remained. The operations researcher still used mathematical techniques but these were not systematised into a volume of standard formulae. The emphasis of operations research was on the approach not the tools used (see, for example, Luckman & Stringer, 1974; also Luck, Luckman, Smith & Stringer 1971; and McLachlan, 1975). In the United States, the emphasis was placed on the use of mathematical techniques. Operations research became a mathematically based science relying on standardised models (e. g. queuing, allocation) and formulae. This approach was facilitated by the availability of computers.
