Book excerpt: The Worry and Anxiety Questionnaire (WAQ) is an 11-item self-report screening questionnaire developed to measure the DSM-IV diagnostic criteria for Generalized Anxiety Disorder (GAD). The purpose of this research was to revise the WAQ and conduct a preliminary investigation of the psychometric properties of the modified questionnaire in an English-speaking, non-clinical sample. The modification process included adding a definition for worry, simplifying the Likert scale qualifiers and consulting with experts in the field of anxiety disorders. There was evidence of sensitivity, specificity as well as convergent and discriminant validity; scores on the WAQ-II were more strongly associated with scores on a measure of worry than with scores on measures of other anxiety disorders, depression and hypochondriasis. In addition, there was evidence of test-retest reliability over a three-week period. The WAQ-II's usefulness in research and clinical practice is discussed.

The first ECNP congress took place in Copenhagen, May 1985, where a working group of European scientists within the field of psychopharmacology was elected to prepare a constituent ECNP congress in Brussels, 1987. Among the most active members of this group was Max Hamilton. At the second ECNP congress in Brussels Max Hamilton was elected as the first honorary member of the ECNP. When we received the message of his death we decided at once to arrange a Max Hamilton memorial symposium at the third ECNP congress, May 1989, in Gothenburg, Sweden. This monograph contains the proceedings of the Max Hamilton symposium which was chaired by the editors. The opening lecture of the third ECNP congress was a Max Hamilton lecture: "A life devoted to science in psychiatry" which was presented by Sir Martin Roth. It seemed obvious to include Sir Martin's lecture as the opening article of this monograph. Although G .E. Berrios was unable to participate in the ECNP congress we have found it logical to include his manuscript on "The Hamilton Depression Scale and the Numerical Description of the Symptoms of Depression" as another personal contribution to Max Hamilton and his rating scales.

Book excerpt: Anxiety disorders are a major public health concern associated with functional impairment and increased use of the health care system. Two of the more common anxiety disorders are generalized anxiety disorder (GAD) and panic disorder (PD), which if identified, can be treated successfully with medications or psychotherapy. GAD is characterized by at least 6 months of persistent and excessive anxiety and worry that is difficult to control and is accompanied by three of six additional symptoms: restlessness, fatigue, decreased concentration, irritability, muscle tension, and sleep disturbance. PD, on the other hand, is episodic in nature and characterized by recurrent and unexpected panic attacks—periods of intense fear or terror associated with autonomic arousal such as breathlessness, chest pain, or fear of losing control—that become a cause of persistent concern in the patient. Generalized anxiety disorder (GAD) and panic disorder (PD) are two common mental illnesses that present in primary care clinics, often with physical symptoms that can inhibit appropriate diagnosis and treatment. Recognition of these disorders by primary care physicians is much lower than the expected rates—in part due to somatic presentations but also due to the lack of routine screening that is in place for some other mental illnesses. Patients with anxiety disorders are often high utilizers of health care resources, and when their anxiety disorders are not diagnosed and treated, they can frequently undergo more expensive testing to rule out medical causes. Identification of accurate and feasible screening instruments for GAD and PD that have been validated in primary care settings have the potential to improve detection and facilitate treatment of these disorders within the primary care clinic, or to generate appropriate referral. Our report is a systematic review of the literature to evaluate the performance of self-report instruments used to diagnose GAD and PD in primary care settings. Three key questions (KQs) guided this systematic review: KQ 1. In general medical patients with somatic symptoms, what are the performance characteristics (e.g., sensitivity, specificity) of self-report questionnaires for diagnosing generalized anxiety disorder or panic disorder? KQ 2. For questionnaires evaluated in KQ 1, which measures are most feasible to use in primary care settings? Specifically, what is the reading comprehension level, time required to complete, response format, and compatibility with telephone administration? KQ 3. For questionnaires evaluated in KQ 1, do the performance characteristics vary by gender, race, age group, or setting?

In addition, sections to assess current mood, somatoform, and substance use disorders are included because of their high comorbitidy rate with anxiety disorder and because the presenting symtomatology of these disorders is often quite similar to that of the anxiety disorders. The ADIS-IV also contains screening questions for psychotic and conversion symptoms and familial psychiatric history. A more detailed section is included to ascertain the patient's medical and psychiatric treatment history. Finally, with the exception of Axis II disorders, the ADIS-IV permits the use of the DSM-IV multiaxial system. The Anxiety Disorders Interview Schedule for DSM-IV: Lifetime Version (ADIS-IV-L) contains all of the sections included in the ADIS-IV. However, unlike the ADIS-IV, the ADIS-IV-L has been designed to establish past (lifetime) diagnoses as well. The ADIS-IV-L also contains a Diagnostic Timeline to assist in the determination of the onset, remission, and temporal sequence of disorders. Book excerpt: The United States Social Security Administration (SSA) administers two disability programs: Social Security Disability Insurance (SSDI), for disabled individuals, and their dependent family members, who have worked and contributed to the Social Security trust funds, and Supplemental Security Income (SSSI), which is a means-tested program based on income and financial assets for adults aged 65 years or older and disabled adults and children. Both programs require that claimants have a disability and meet specific medical criteria in order to qualify for benefits. SSA establishes the presence of a medically-determined impairment in individuals with mental disorders other than intellectual disability through the use of standard diagnostic criteria, which include symptoms and signs. These impairments are established largely on reports of signs and symptoms of impairment and functional limitation. Psychological Testing in the Service of Disability Determination considers the use of psychological tests in evaluating disability claims submitted to the SSA. This report critically reviews selected psychological tests, including symptom validity tests, that could contribute to SSA disability determinations. The report discusses the possible uses of such tests and their contribution to disability determinations. Psychological Testing in the Service of Disability Determination discusses testing norms, qualifications for administration of tests, administration of tests, and reporting results. The recommendations of this report will help SSA improve the consistency and accuracy of disability determination in certain cases.
