Book excerpt: In the United States alone, alcohol use is responsible for more than seven million visits to the Emergency Department (ED), annually. This figure is likely distorted by the supposition that alcohol use often goes underreported by patients or unnoticed by providers. Many of these visits could potentially be avoided should patients address alcohol-related issues in the primary or other outpatient care setting (French, Gumus, and Turner, 2008). Patients are often seen and treated in the ED with little to no recognition of alcohol-related problems, missing a valuable opportunity to advocate for these individuals. Implementing routine alcohol screening, along with a nurse-conducted Brief Intervention (BI) and referral to outpatient treatment services is an underutilized but safe and effective way to address alcohol-related presentations in the emergency department, and can help to create a positive shift in the care setting for alcohol-related medical problems, and reduce alcohol consumption in patients following an alcohol-related injury or illness. This project addresses the deficit in patient care regarding alcohol-related intervention, outlines the recommended solution, provides an implementation plan, incorporates nursing theory, and outlines a plan for dissemination of the evidence. A review of relevant literature is also provided to support the solution.

Book excerpt: A public health approach to the harms that arise from alcohol misuse requires that alcohol problems be identified early and intervened with before serious social and medical consequences occur. An intervention directed at at-risk drinkers, known as alcohol brief intervention, has well established efficacy when delivered by physicians working in primary health care clinics and some hospital Emergency Departments. However, the adoption by physicians has not been widespread, due to a number of barriers including lack of physician time, skill, and knowledge about alcohol. An important question thereby arose as to whether other health professionals, including social workers, achieve the same positive patient outcomes as physicians when delivering the intervention. Using data from the largest effectiveness study of alcohol brief intervention in Emergency Departments (ED's) to date (Academic Emergency Department Screening, Brief Intervention, and Referral to Treatment Research Collaborative), changes in alcohol use from patients who were treated by physicians were compared to changes in those patients treated by non-physician health professionals. Participants in the current study (n=279) were drinking at levels above the National Institutes of Health limits. Patients were divided into two groups: 137 received the intervention from a physician and 142 received the intervention from a non-physician health professional. In addition to baseline data on demographic, education and health variables, measures on three quantity and frequency of alcohol use variables were collected at baseline and at 3-months post intervention. Results of an Analysis of Co-Variance that adjusted for differences in baseline drinking, indicated that controlling for gender, marital status, education level, and severity of addiction, the ED patients who received the alcohol brief intervention from trained non-physician health providers did as well as at 3-month follow-up on the three assessed outcomes of reduction in alcohol use as those patients who received the intervention from a physician. Findings suggest that non-physician health professionals who are trained in alcohol brief intervention be considered to provide the intervention, removing significant barriers to physician delivery that currently exist, and increasing opportunities for wide-spread implementation of this efficacious public health approach to reducing the harms of alcohol misuse.

Book excerpt: Background: Screening, Brief-Intervention and Referral Treatment (SBIRT) is an evidence-based intervention model used to identify, reduce, and prevent hazardous use of alcohol and illicit drugs. SBIRT is often utilized in Emergency Department (ED) and primary care settings because such departments present an opportune moment to engage patients in conversation about substance use, as they are already there for health-related purposes. Few studies have examined alcohol related differences between Latinos whose primary language is English compared to those whose primary language is Spanish, or investigated the effects of SBIRT when delivered in Spanish to primarily Spanish-speaking patients. This study describes the socio-demographic and alcohol-related characteristics of Spanish and English speaking Latino patients screened for alcohol use in ED settings, and evaluates the effects of SBIRT on those identified as hazardous drinkers. Methods: A total of 24,319 Latino patients were interviewed in 8 different ED settings throughout San Diego County. Of this sample, 61% preferred to do the interview in English, while 39% preferred to be interviewed in Spanish. A total of 193 Latino patients who were screened received SBIRT in the ED in their preferred language, and completed a 6- month self-report follow-up assessment (19% Spanish; 81% English). Conclusion: Results suggest that although Latino ED patient groups may differ somewhat on socio-demographic and alcohol-related measures, a linguistically appropriate alcohol Brief Intervention in the ED may have broad benefits.
