The fluoridation of water has been used for the prevention of tooth decay for over fifty years. During this time little research has been done to ascertain whether it works. Book excerpt: The debate over public water fluoridation has endured for more than five decades. A central defining feature of this debate is a dispute over the acceptability of risk, fuelled by conflicting interpretations of scientific evidence and contrasting perceptions of risk. The fluoridation controversy also brings to the fore the complexities inherent in making public policy decisions under conditions of uncertainty. These and other aspects of the fluoridation debate in Ireland and internationally are analysed in this dissertation by developing and applying a new theoretical sociological framework. This framework is based on a synthesis of concepts from three socio-cultural theories. The fieldwork for this study was undertaken between 1999 and 2002 and includes an extensive documentary analysis of the scientific, popular and alternative media on water fluoridation. Case studies of the interpretation of evidence from observational epidemiological studies on the risks and benefits of water fluoridation, and narrative and systematic reviews of these studies, are presented. A cultural explanation is advanced for the diverging risk perceptions of proponents and opponents of fluoridation. A comprehensive analysis and account of the emergence and development of the contemporary 'risk conflict' over water fluoridation in Ireland is given and the modus operandi of a government appointed review body, the Forum on Fluoridation, is examined. The central arguments emerging from this socio-cultural analysis of the fluoridation debate are: 1) the interpretation of evidence is a value-laden and subjective process that is heavily influenced by the preconceived beliefs and policy commitments of the interpreter; 2) the conflict over water fluoridation cannot be simplistically construed as a technical and scientific debate over benefits and risks but must be viewed as a broader cultural conflict between diametrically opposed worldviews; 3) There is little evidence from the analysis of the fluoridation 'risk conflict' in Ireland of the so-called 'demise of science' nor of a move towards a 'democratic science'; 4) the approaches to decision-making under uncertainty advocated by opposing sides in the debate do not provide a viable basis for policy-making and are based on unreasonable and unattainable 'standards of proof.' A major implication of this analysis is that further scientific research will not resolve the 'risk conflict' over public water fluoridation. It is also argued that the sphere of technical decision-making on controversial risk issues must be widened by developing more inclusive dialogue. It is concluded that an essential prerequisite for such dialogue is the need for a greater collective awareness and acknowledgment of the limitations of science and adjustment of our expectations regarding what science can realistically deliver to the policy process.

Community water fluoridation has been controversial in the United States since it was first proposed in 1945. Since then, fluoridation has been a major factor in reducing the prevalence and severity of dental cavities in the United States (U.S.). Fluoride safely and effectively prevents dental cavities and is available from multiple sources. Community water fluoridation is one of these sources, providing fluoride to all people regardless of their position within the community. Even so, community water fluoridation has not been universally implemented despite efforts by the Centers for Disease Control and Prevention (CDC) and the American Dental Association (ADA) to support and promote it. Fluoridation has been declared one of the ten greatest public health achievements of the 20th century (Centers for Disease Control and Prevention, 2015). In this study, I will review the literature on the history of community water fluoridation, comprised by scientific studies on its benefits and disadvantages. I will examine how the controversy surrounding this issue began, and how the anti-fluoridationists' arguments have continued to influence some communities to vote against fluoridation. Finally, I will attempt to draw a conclusion as to why community water fluoridation has not been implemented in all cities, despite its overall health benefits.
