Book excerpt: The project sought to develop methods to extend the storage life of red blood cells. Extended storage would allow donor to self or autologous transfusion, expand and stabilize the blood supply, reduce the cost of medical care and eliminate the risk of transfusion related infections, including a spectrum of hepatitides (A, B and C) and HIV. The putative cause of red blood cell spoilage at 4 C has been identified as oxidative membrane damage resulting from deoxyhemoglobin and its denaturation products including hemichrome, hemin and Fe{sup 3+}. Trials with carbon monoxide, which is a stabilizer of hemoglobin, have produced striking improvement of red blood cell diagnostics for cells stored at 4 C. Carbonmonoxy hemoglobin is readily converted to oxyhemoglobin by light in the presence of oxygen. These findings have generated a working model and an approach to identify the best protocols for optimal red cell storage and hemoglobin regeneration.

The storage of frozen red blood cells has been approved by the FDA for up to 10 years of storage time. However once the blood is thawed and reconstituted, the red cells can be stored in a normal blood bank refrigerator for a period of only 72 hrs after which they must be discarded. This short storage period creates great problems in the military environment as well as in civilian life. The reasons for this short period of storage are usually stated as follows: A. The thawed red cells have been suspended in a saline preservative medium with a small amount of added glucose which has limited nutritive value. Over a period of time the red cells deplete the nutrients and begin to autolyze; B. Some of the storage media used for longer term preservation are not isoosmotic with red cells and thus produce some destruction of these cells due to hypotonic shock when they are placed in this medium during the storage period; C. As the red cells metabolize the glucose, the pH slowly drops. This decrease in pH causes a decreased viability of the red cells and decrease in the intermediary metabolites necessary for Rbc function; D. Mos of the methods of washing cells employ equipment that is not completely sealed and therefore there is a possibility that bacteria could be introduced into the blood and grow during the storage period. The purpose of this study was to determine if adding one of several currently acceptable anticoagulants to the blood after washing would increase their survival during storage. (ttl).

Carbon monoxide is utilized for stabilizing hemoglobin in red blood cells to be stored at low temperature. Changes observed in the stored cells are similar to those found in normal red cell aging in the body, the extent thereof being directly related to the duration of refrigerated storage. Changes in cell buoyant density, vesiculation, and the tendency of stored cells to bind autologous IgG antibody directed against polymerized band 3 IgG, all of which are related to red blood cell senescence and increase with refrigerated storage time, have been substantially slowed when red blood cells are treated with CO. Removal of the carbon monoxide from the red blood cells is readily and efficiently accomplished by photolysis in the presence of oxygen so that the stored red blood cells may be safely transfused into a recipient.
