Book excerpt: Les Troubles respiratoires obstructifs du sommeil (TROS) de l'enfant, allant du ronflement primaire au syndrome d'apnée/hypopnée du sommeil sont fréquents et leurs conséquences sont graves. Le médecin généraliste, en tant que médecin de premier recours et coordinateur des soins, a un rôle primordial dans la prise en charge multidisciplinaire de ces troubles dont les symptômes sont nombreux et pour lesquels l'arsenal thérapeutique est varié (ORL, orthodontie, allergologie, pneumologie...) Les anomalies dento-faciales étant une cause et une conséquence de TROS, un bilan orthodontique est indiqué dans leur prise en charge. L'efficacité de la disjonction intermaxillaire, technique non chirurgicale d'orthopédie-dento-faciale qui aboutit à une expansion transversale du maxillaire, a été prouvée dans le traitement des enfants présentant un TROS. Le but de cette étude qualitative par entretiens individuels semi-dirigés était d'évaluer le ressenti des patients et de leurs parents concernant le traitement de disjonction intermaxillaire. Les résultats montrent que ce traitement est bien vécu par les patients et leurs parents, qui décrivent quelques difficultés initiales (douleur, alimentation, réalisation des tours de clés...), rapidement disparues après une phase d'adaptation. Suite à ce traitement, ils ont ressenti une amélioration clinique tant morphologique que fonctionnelle (notamment sur le ronflement) et n'en ont perçu aucune conséquence négative. Ils sont satisfaits de ce traitement et certains parents auraient même aimé en bénéficier eux-mêmes. Le traitement par disjonction intermaxillaire est bien toléré par les patients et il est nécessaire que les médecins généralistes en connaissent globalement les résultats et les modalités, afin de pouvoir d'une part orienter les patients, d'autre part les renseigner.

Le chirurgien dentiste, exposé à la sphère oro-faciale de ses patients, est à même de reconnaître les facteurs prédisposant au ronflement primaire et de prendre en charge ceux-ci ou de les adresser aux spécialistes.

Trois catégories de SAOS sont à distinguer : I) Enfant avec hypertrophie adénoamygdalienne II) enfant et adolescent en surpoids III) enfant avec pathologie génétique ou neuromusculaire. Malgré des progrès ces dernières années sur le plan physiopathogénique, diagnostique et thérapeutique, le diagnostic et la mise en route des thérapeutiques sont encore trop tardifs, ce qui impacte la morbidité. Les objectifs du travail de thèse étaient : 1) de s'appuyer sur des études cliniques avec analyse complète du sommeil en polysomnographie afin de mieux comprendre les mécanismes physiopathogéniques et optimiser les stratégies diagnostiques et thérapeutiques du SAOS de l'enfant 2) de participer à l'harmonisation de la prise en charge multidisciplinaire du SAOS de l'enfant et de l'adolescent. Nous avons évalué l'impact du décubitus ventral sur le sommeil et la respiration chez les nourrissons avec séquence de Pierre Robin SPR (type III). Le décubitus ventral permet d'améliorer largement la qualité du sommeil mais corrige insuffisamment les évènements obstructifs. Ensuite, nous avons comparé la mesure des évènements obstructifs entre polygraphie PV et polysomnographie PSG en unité du sommeil chez les nourrissons avec SPR. La PV en unité du sommeil constitue une alternative de qualité à la PSG. Chez l'enfant avec suspicion de SAOS, nous avons évalué la faisabilité de la PSG ambulatoire qui s'avère faisable et interprétable. Actuellement, nous menons une étude multicentrique sur l'implication de la dysfonction du tronc cérébral dans la physiopathogénie de la SPR dont les résultats devraient permettent d'améliorer la prise en charge de ces patients. En parallèle des études cliniques, nous avons participé à la proposition de recommandations de prise en charge du SAOS chez l'adolescent. Nous avons publié une synthèse des recommandations de prise en charge du SAOS chez l'enfant et l'adolescent.

The last chapter in this book is focused on QT dispersion in patients with obstructive sleep apnea, with increased QT dispersion potentially reflecting disturbed myocardial repolarisation and predisposition to cardiac arrhythmias.
