Book excerpt: This study examined contextual influences on the relationship between peer norms for condom use and risky sexual behavior among African-American men who have sex men. Analyses assessed the moderating effects of socio-demographic, socio-contextual, and health-related variables. One thousand forty African-American men, who have sex with men, ages 17 to 25 years, were surveyed as part of the Community Intervention Trial for Youth (CITY) from 1999 to 2002 in Atlanta, Georgia. Findings supported the hypothesis that participants who engaged in unprotected insertive anal intercourse, socio-contextual variables moderated the relationship between peer norms and risky sexual behavior. Findings also supported the hypothesis that participants who engaged in unprotected receptive anal intercourse, both socio-demographic variables and a health variable moderated the relationship between peer norms and risky sexual behavior. Findings have implications for intervention, policy, and research, including a need for interventions that recognize the contexts of influence that shape African-American MSM sexual behavior and that support norms for consistent condom use in both steady and casual sexual relationships.

Book excerpt: HIV/AIDS is a growing concern in the African American community. From 2005 to 2008, there was a 12% increase in HIV/AIDS diagnoses among African Americans (CDC, 2010a). African American males have higher rates of HIV/AIDS diagnoses than any other ethnic or racial groups. In 2006, 65% of the HIV/AIDS cases among African Americans were from adult males (CDC, 2010a). These higher rates are in part attributed to higher levels of sexual risk-taking including unprotected sex, sex with multiple partners, and sex with high-risk partners. African American adult men get tested at almost the same rate (52%) as African American women 53% (CDC, 2007). HIV testing is important to the prevention of HIV/AIDS and can result in substantial reductions in risk behaviors (Marks, Crepaz, & Janssen, 2006; Weinhardt, Carey, Johnson, & Bickham, 1999). Masculinity, a man's concept of what it means to be man, influences engagement in sexual risk-taking behaviors (Bowleg, 2004, Duck, 2009; Lichtenstein, 2004), as well as health-seeking behaviors (Hammond, Matthews, Mohottige, Agyemang, & Corbie-Smith, 2010; Royster, Richmond, Eng, & Margolis, 2006). One form of masculinity, hypermasculinity is related to sexual risk-taking behaviors, especially among young African American males. The purpose of this dissertation was to investigate the relationship between hypermasculinity and sexual risk-taking, and HIV testing among African American males. One hundred twenty-six African American male college students were recruited to participate in an HIV prevention intervention for African American women on a Historically Black College and University (HBCU) campus. Prior to participating in the intervention, participants completed a questionnaire with measures of hypermasculinity, sexual behavior, and HIV testing. The results showed that hypermasculinity predicted frequency of sex. Hypermasculinity and frequency of sex predicted lifetime HIV testing. Frequency of sex and the number of sex partners predicted current HIV testing (in the past month). The interaction between hypermasculinity and frequency of sex was marginally significant. The findings from this research may increase our understanding of sexual risk-taking, improve HIV testing initiatives, and be useful in the development of HIV prevention programs for African American heterosexual males.

Alcohol consumption is highly prevalent among MSM, is associated with condomless sex, and may contribute to HIV risk among Black MSM. This study aimed to: 1) examine the association between alcohol consumption (i.e. drinking before/during sex and levels of alcohol use problems) and condom use during lifetime, past 6 months, and the event level; and 2) test moderators (i.e. sex-related alcohol expectancies, impulsivity, sensation seeking, and partner type) of the relationship between problem drinking and condom use across all three levels of analysis. Black MSM (N = 116) reported sexual behavior and condom use for lifetime, past 6 months, and the most recent condom and condomless sex events. The Alcohol Use Disorders Identification Test determined problem drinking (i.e. AUDIT scores 8). Ordinal and binary regression analyses analyzed associations between AUDIT scores, condomless receptive and insertive sex, and hypothesized moderators. Homophobia and gay identity are two variables that have emerged from previous research that may influence this discrepancy. The purposes of this cross-sectional investigation were to determine whether internalized homophobia influences high-risk sexual behavior and to determine the relationship between internalized homophobia and an individual's stage of homosexual identity formation in adult African American men who have sex with men (AAMSM). Linear regression, logistic regression, Fisher's exact test, and the Kruskal-Wallis test were used to investigate the associations between internalized homophobia, homosexual identity formation, and sexual risk behaviors for HIV infection in adult Black men who have sex with men. The study tested a new scale, the Homosexual Identity Formation Scale, and then validated the Homosexual Identity Formation Scale and the Internalized Homophobia Scale (Wagner, Brondolo, & Rabkin, 1996) in adult AAMSM. Although not statistically significant (p[greater than]0.05), higher scores on the Internalized Homophobia Scale were associated with an increased odds of engaging in high risk sexual behaviors, while higher scores on the Homosexual Identity Formation Scale were associated with a decreased odds of participating in high risk sexual behaviors. Importantly, findings revealed that both internalized homophobia and homosexual identity formation had a dose-response relationship with high-risk sexual behaviors. Internalized homophobia and homosexual identity formation had no association in adult AAMSM (p[greater than]O.05). Higher scores on the Internalized Homophobia Scale were associated with increased odds of participants identifying as gay as opposed to Black. With respect to predominant self- identity, there was a statistically significant difference between those AAMSM who reported no male sex partners and those who reported one or more in the previous three months (p=0.027).

There is evidence that behaviors are, acquired through family and community interactions, messages about sexual identity, gender expectations and life experiences. Social networks both positive and negative have a significant impact on the individual's self-perception and acceptable behavior. The syndemics theory holds relevance in these studies in that there are multiple factors at work in the lives of Black men who have sex with men that contribute to their health disparities. In this study, 10 New York City African American/Black MSM between the ages of 18 and 30 years old were engaged in individual in-depth interviews. The participants described in detail their social and sexual experiences beginning with the point at which they self-identified as same gender-loving Black males. Data analyses showed the significance of family acceptance, and social support, and seeking validation in syndemic formation in these subjects. The data further demonstrated that disparities and syndemic formation are dependent upon several reinforcing experiences, not on one single incident.
