Cette opportunité qui nous est offerte, contribuera, je l'espère, à mieux faire connaître notre spécialité notamment en milieu militaire. Durant toute notre carrière, il nous a fallu, à de nombreuses reprises, justifier son existence. Mais la problématique est la même en milieu civil... Il y a certaines explications à cela. D'une part il s'agit d'une discipline qui aborde de très nombreuses pathologies parfois très rares, notamment génétiques, ou au contraire ultra-fréquentes, communes comme l'acné et de ce fait parfois sous-estimées dans leur impact sur la qualité de vie, parfois gravissimes comme le mélanome mais le plus souvent bénignes une fois traitées. L'amalgame a parfois été fait avec la dermatologie de ville qui consacre une part importante de son activité à des problématiques d'ordre esthétique : ce n'est évidemment pas le cas à l'hôpital puisque les services de dermatologie sont tout simplement submergés par l'afflux de malades avec des dermatoses aiguës (toxidermies : la peau constituant un des premiers organes atteints lors de réactions adverses médicamenteuses) ou chroniques sévères ou des tumeurs évoluées en raison notamment du retard de prise en charge lié à la pénurie de médecins dans notre spécialité et à l'augmentation considérable de l'incidence de ces pathologies depuis plusieurs dizaines d'années.

Book excerpt: Depuis 2018, le Service suit scrupuleusement les politiques de santé et travaux visant des changements de rôles et de compétences des paramédicaux au sein du système public de santé. Un groupe de travail institutionnel et une mission avaient eu pour mandat de faire des propositions à ce sujet et identifier des actions. Notre pays n’est pas le seul à s’intéresser à ces évolutions. Des armées étrangères ont fait ces choix, il y a de nombreuses années eu égard à leur histoire et culture notamment concernant la pratique avancée, tandis que d’autres continuent à imaginer comment la médecine militaire peut évoluer en incluant de nouveaux métiers ou nouvelles fonctions dans leur chaîne de soins. Considérant notre contexte, un nouveau cap a été franchi plus récemment. D’abord, par la prise en compte dans la feuille de route des ressources humaines 2023-2025 de la construction d’une filière de formation et enseignement pour les paramédicaux qui indique que le Service ouvrira de nouvelles trajectoires pour les métiers du soin, de la rééducation-réadaptation et des métiers médico-techniques. Ensuite, depuis août, la Loi de programmation militaire (LPM) apporte une nouvelle dimension à la réflexion sur les compétences des infirmiers, notamment en ce qui concerne la médecine de l’avant. L’ajout au sein de la LPM d’une mention particulière en ce sens dans le chapitre portant sur la cohérence du modèle d’armée par un renforcement des soutiens apporte une intégration plus systémique à cette volonté déjà portée au sein du Service. Tous ces engagements résultent d’un travail de fond ainsi que de la prise en compte par les autorités de la nécessité de faire évoluer les métiers et les pratiques de ceux qui sont au plus près de nos bénéficiaires, et ce dans l’optique de maintenir une haute qualité des soins aux militaires. In extenso, c’est bien la réponse à l’hypothèse de conflits armés de haute intensité qui reste l’alpha et l’oméga de cette tendance et qui oriente le Service vers des changements profonds. Dans une doctrine repensée, l’inscription de nouvelles modalités d’emploi des paramédicaux pourrait se révéler un atout supplémentaire dans le soutien santé de nos armées et dans une nouvelle articulation de la chaîne de sauvetage.
