Ce dernier représente une part non négligeable des causes d'hospitalisation. La maison d'accompagnement peut être une alternative pour accueillir les patients en fin de vie faute d'entourage sans indication à être hospitalisé. Elle fait actuellement l'objet du dernier projet de loi sur la fin de vie. Cependant, instaurer un nouveau dispositif serait voué à l'échec sans l'adhésion des professionnels de santé de terrain. Nous avons donc recueilli le ressenti des médecins généralistes sur le concept de maison d'accompagnement. Méthode : Une étude qualitative a été réalisée par focus group et entretien semi dirigée auprès des médecins généralistes de la CPTS Haute Corrèze Ventadour. Résultats : Les bienfaits de la maison d'accompagnement consistaient en l'apport d'un cadre humain par la prise en charge par l'intervention des bénévoles et une diminution de la médicalisation du soin. Elle pouvait être un support pour un accompagnement psychologique, des séjours de répit et offrir une éducation à la fin de vie. Elle constituait un recours de proximité. Elle s'adressait aux patients en soins palliatifs plutôt au long cours. Accueillir des patients en phase terminale était une possibilité dans la mesure où la maison restait en lien avec les structures extérieures en cas de besoin. Cependant, le manque de ressources humaines permettant un bon maillage territorial, la prépondérance du bénévolat dans son fonctionnement, le manque de culture palliative sur le terrain remettait en cause sa faisabilité. Discussion : Instaurer des maisons d'accompagnement sur le territoire est difficile à imaginer à court terme. Cependant, des améliorations sur l'offre existante sont à considérer notamment le renforcement de l'HAD et de l'EMSP sur la continuité des soins et la possibilité d'accompagnement psychologique.

Si les difficultés inhérentes à la pratique des SP en ambulatoire sont bien décrites, peu d'études mettent en valeur les ressources effectivement utilisées par les médecins généralistes de terrain. Nous avons dans cherché à mettre en évidence les ressources réellement utilisées par les médecins généralistes dans la prise en charge à domicile des patients en soins palliatifs. Matériel et méthode : pour répondre à cette question, nous avons choisi de réaliser une thèse de type qualitative, par entretiens semi-dirigés à l'aide d'un guide d'entretien. La population cible était les médecins généralistes installés en libéral dans les Alpes-Maritimes, recrutés selon un échantillonnage raisonné. Résultats : inclusion de 21 médecins généralistes d'âges et modes d'exercice différents. Les ressources mises en évidence par notre travail ont été classées en deux grandes catégories selon qu'elles étaient internes ou externe au médecin. Les ressources internes sont : l'histoire personnelle du médecin, la formation (initiale, continue et de terrain), la relation médecin-malade et l'organisation du temps de travail. Les ressources externes sont : les personnes ressources, les structures de soutien, la loi Leonetti, les modes de rémunération et les hospitalisations. A partir de ces résultats, nous avons établi un guide pratique à l'usage des médecins généralistes des Alpes-Maritimes, afin de les aider dans leur pratique des soins palliatifs à domicile. Conclusion : notre travail a permis de mettre en évidence des ressources externes et internes utilisées par les médecins généralistes mais les ressources internes semblent avoir plus d'impact sur leur vécu de la prise en charge de patients en soins palliatifs. Les différences importantes de ressenti et de vécu des soins palliatifs parmi les médecins de notre étude malgré leur accès aux mêmes structures de soutien peuvent être en partie expliquées par des disparités individuelles de sensibilité au syndrome d'épuisement professionnel soignant, ainsi que par l'auto-efficacité.

Book excerpt: Contexte : Les prescriptions anticipées sont recommandées en soins palliatifs afin d'apporter un traitement adapté et rapide pour un symptôme gênant susceptible d'apparaitre ou de s'aggraver. Elles semblent cependant être peu utilisées dans la dyspnée. Méthode : Au travers d'un questionnaire adressé d'une part à des médecins généralistes et d'autre part à des infirmiers libéraux du Nord-Pas-de-Calais, nous nous sommes intéressés aux éléments freinant ou favorisant ces prescriptions anticipées et ceci spécifiquement dans la prise en charge de la dyspnée à domicile. Résultats : 90 médecins et 86 infirmiers ont répondu à notre étude. La grande majorité a déjà eu une expérience de prescriptions anticipées, notamment pour la douleur (91% des médecins et 83% des infirmiers), l'anxiété et la constipation. Expérience plus faible pour la dyspnée (42% des médecins et 26% des infirmiers). 56.7% des médecins et 100% des infirmiers évoquent la possibilité d'un refus de ces prescriptions, principalement en cas de mauvaise compréhension de la famille, manque de temps ou par crainte d'une détresse respiratoire secondaire à l'administration de morphine. La connaissance entre les intervenants (médecin et infirmier), l'information au patient et sa famille et un avis spécialisé en soins palliatifs sont, au contraire, des éléments favorisant voire indispensables. L'initiation du traitement est considérée comme faisant partie des compétences infirmières pour 87% des médecins et 91% des infirmiers mais un peu plus de 60% de ceux-ci estiment que le médecin doit être prévenu avant le début du traitement. Conclusion : La prise en charge de la dyspnée se heurte aux limites des prescriptions anticipées : cadre légal mal défini, évaluation difficile du symptôme, responsabilités professionnelles engagées sont autant d'obstacles à leur mise en oeuvre. La réalisation d'un projet de soin personnalisé et accessible à tout professionnel en capacité d'intervenir en urgence peut être une solution intermédiaire intéressante.
