Book excerpt: Introduction : Malgré la multiplicité des moyens contraceptifs, le taux d’interruption volontaire de grossesse reste constant en France. Notre objectif principal était de décrire les connaissances des adolescents de la Somme concernant les méthodes contraceptives, leurs moyens d’obtention et leur utilisation pratique. L’objectif secondaire était d’aborder leurs représentations en matière de sexualité et de contraception. Méthode : Nous avons mené une étude épidémiologique descriptive pluri-centrique, basée sur des données recueillies à partir d’un questionnaire anonyme distribué dans des collèges, des lycées et des Centres de Planification et d’Education Familiale (CPEF) de la Somme. Le questionnaire abordait les thématiques de la sexualité et de la contraception chez les adolescents : leurs connaissances théoriques, leurs pratiques sexuelles et contraceptives ou encore leurs représentations. Les données ont été recueillies auprès de 560 adolescents de 13 à 18 ans. Résultats : Notre échantillon comprenait 57,3% de filles et 42,7% de garçons, dont l’âge moyen était 15,4 ± 1,37 ans. Ils étaient issus du collège pour 43,4% d’entre eux, du lycée pour 56,2% et des CPEF pour 3,8% de l’échantillon. Seuls 3,6% des adolescents avaient eu recours au médecin traitant pour obtenir des informations sur la contraception. Les connaissances théoriques du cycle étaient maîtrisées par 47,3 % des adolescents. Le préservatif était utilisé par 82% des adolescents lors du premier rapport sexuel qui avait lieu entre 13 et 15 ans pour 59,2% d’entre eux. Le premier rapport n’était pas protégé chez 12,6% des adolescents, associé à une conduite addictive dans 23,1% des cas. L’utilisation du préservatif n’était pas systématique au décours d’une relation stable pour 36,9% des adolescents. Parmi les adolescentes sous pilule, 45,8% déclaraient avoir eu au moins un oubli au cours des 3 derniers mois. La conduite à tenir en cas d’oubli de pilule était connue par 43,9% des adolescents. 10,5% des adolescents ne connaissaient pas les moyens d’obtention de la contraception d’urgence. 82,8% des adolescents interrogés considéraient le fait de se sentir prêt psychologiquement comme un préalable nécessaire au projet d’une grossesse, devant l’autonomie financière (55,1%) ou la fin du cursus scolaire (37%). Conclusion : Accepter la sexualité des adolescents, et intégrer les facteurs sociocognitifs influençant leurs comportements contraceptifs sont les préalables indispensables à la prescription d’une méthode contraceptive personnalisée. Le médecin traitant, du fait de sa place privilégiée au sein de la cellule familiale, a un rôle fondamental pour évaluer l’adéquation de la méthode contraceptive prescrite et détecter les situations à risque d’échec contraceptif.

En France, en dépit de la richesse de l'offre contraceptive, le nombre des IVG chez l'adolescente est toujours élevé. Ils sont souvent le résultat d'un échec ou d'une absence de contraception. L'objectif était d'explorer comment leurs croyances et représentations sur la contraception influent sur les pratiques des jeunes femmes et permettre d'identifier des leviers d'action pour une contraception efficace. Une enquête qualitative par entretiens semi-directifs à l'aide d'un guide d'entretien a été menée auprès de 18 jeunes femmes, choisis selon un échantillonnage raisonné. Les femmes, de 15 et 25 ans, ont été recrutés dans un CMS et un CPEF à Saint-Denis et par effet « boule de neige » en médecine libérale en Ile-de-France. L'analyse thématique du contenu des verbatim, réalisée à l'aide de l'approche de la Théorie ancrée, a été affinée par une lecture croisée thésarde et directrice de thèse. Cette étude permet de constater que les représentations et croyances des jeunes femmes peuvent influencer leurs pratiques contraceptives et entraîner des comportements inadéquats. En effet le fonctionnement du contraceptif dans le corps est flou. Même si elles connaissent différents contraceptifs, leur utilisation n'est pas claire. Elles sont nombreuses à ne pas connaitre les conduites à tenir en cas d'oubli ou le délai pour utiliser la pilule d'urgence. Elles restent nombreuses à avoir des idées figées sur le DIU ou sur les risques de stérilité avec la contraception. Le renforcement des cours de sexualité à l'école pourrait combler ces lacunes sur la contraception.
