Book excerpt: The inappropriate use of antibiotics is a primary cause of the ongoing increase in drug resistance amongst pathogenic bacteria. The resulting decrease in the efficacy of antibiotics threatens our ability to combat infectious diseases. Rapid point-of-care tests to identify pathogens and better target the appropriate treatment could greatly improve the use of antibiotics. Yet there are few such tests currently available or being developed despite the rapid pace of medical innovation. Clearly something is inhibiting the much-needed development of new and more convenient diagnostic tools. This study delineates priorities for developing diagnostics to improve antibiotic prescription and use with the goal of managing and curbing the expansion of drug resistance. It calls for new approaches particularly in the provision of diagnostic devices and in doing so outlines some of the inadequacies in health science and policy initiatives that have led to the dearth of such devices. The authors make the case that there is a clear and urgent need for innovation not only in the technology of diagnosis but also in public policy and medical practice to support the availability and use of better diagnostic tools. This book explores the complexities of the diagnostics market from the perspective of both supply and demand unearthing interesting bottlenecks some obvious some more subtle. It calls for a multifaceted and broad policy response and an overhaul of current practice so that the growth of bacterial resistance can be stemmed.

The Government should produce a new White Paper with details on: how the Department of Health will facilitate the translation of advances in genomic science into clinical practice, including the operational changes needed to bring genetic testing into mainstream clinical practice; a roadmap for how such developments will be incorporated into the NHS; proposals for a programme of sustained long-term funding to support these measures. A range of genetic tests are already being used within the NHS to improve the diagnosis and treatment of a range of common illnesses, but there are barriers to the translation of new tests from invention through to use. The Committee looks specifically at the growing market for Direct to Consumer Tests (DCTs) and raises concerns about the effect of consumers receiving DCT results via the internet without proper medical advice to put those results in context. The Committee supports a voluntary code of practice for DCT providers. Ensuring NHS staff have adequate training in genomic medicine will be vital, and training in using and interpreting genetic tests should be an integral part of training for healthcare workers. The report recognises that there are privacy concerns about the retention and use of genetic data as well as apprehension about how the data may be used by, for example the insurance industry.

The number of requests for investigations has been growing steadily, and the range of diagnostic tests has been increasing. The NHS and Community Care Act (1990) will take the service into unknown territory. Within a cash limited service, all of these changes must be co-ordinated within the finance available: There are five main areas of concern: 1) controlling demand 2) controlling resources 3) staffing 4) controlling quality 5) structuring management responsibilities. These problems are mostly well recognised by practitioners and there are initiatives underway to address them in some areas. But the 1990 Act provides an additional impetus for change, and a framework within which this change must occur, so that a new approach is now appropriate.
