Book excerpt: Background: The objective of this survey was to collect information from the countries involved in the European Commission initiative on breast cancer (the 28 Member States, plus Iceland and Norway) regarding the organisation of breast cancer services and other aspects of interest for the initiative (e.g. screening programmes, training requirements for professionals, quality and safety aspects, quality assurance schemes, etc.). Methods: The survey included a questionnaire and a data protection form distributed by e-mail, the participants were nominated upon request of Directorate-General for Health and Consumers via respective health attaches. Results: Twenty-five out of 30 contacted countries responded, corresponding to a response rate of 83%. Conclusions: Healthcare systems are diverse across Europe; different quality assurance schemes for breast cancer care are in place for less than 50% of the countries. A European-wide, harmonised, evidence-based and flexible scheme is needed to grant equal and quality benchmarked treatment to patients.

The Joint Research Centre, the European Commission's in-house science service, was assigned with the task of developing a voluntary European Quality Assurance scheme for breast cancer services (the European QA scheme) based on the European legal framework on accreditation. This task, among others, are part of the European Commission Initiative on Breast Cancer (ECIBC). Methods. A search of external quality assessment schemes for breast cancer care already in place in Europe was carried out using different strategies in: MEDLINE, website of relevant scientific societies, and EC Reports. Only schemes fully implemented, with evidence of current activity, with at least one centre in Europe currently holding the certificate awarded by that organisation and foreseeing a third-party audit / on-site survey were considered. Results. Seventeen schemes specifically addressing breast cancer were identified in Europe and thirteen countries have at least one scheme in place.^For eight schemes the requirements and procedures were available in a language known to the JRC Healthcare Quality Team and were thus included in this research. The number of breast cancer services implementing such schemes goes from three to 277 with a median of 23. In some countries, more than one scheme is present, with a maximum of 4 schemes present in one country. Six schemes are managed by private entities and two by public organisations. All the schemes covered more than one stage of breast cancer care, with requirements for diagnosis included in all the schemes. Requirements are highly variable, as in the format, wording and organisation of these. However, they always included requirements related to the clinical management of breast cancer (or breast cancer diagnosis), as well as a list of organisational requirements and quality indicators. For all the schemes the awarding process includes a preparation phase, a site visit and a follow-up.^The duration of the award is usually 3 or 5 years. Conclusion. This research provides a collection of relevant features for the definition of the European QA scheme, both from a requirement definition point of view as well as for the organisation of the on-site visit and its follow-up. A European-wide scheme could help harmonise the situation in Europe and ensure that European citizens receive the same quality of care, at least for the essential aspects, regardless of where they live.

Book excerpt: In 2015, the European Commission Initiative on Breast Cancer (ECIBC) started the development of the European guidelines for breast cancer screening and diagnosis (henceforth the European Breast Guidelines) under the auspices of the Directorate-General for Health and Food Safety (DG SANTE) and the technical and scientific coordination of the Directorate-General Joint Research Centre (JRC). To support the JRC in this task, a Guidelines Development Group (GDG), consisting of independent experts and individuals, was established. The European Breast Guidelines’ scope (The Scope) represented the first output of the development process of the European Breast Guidelines. Via a public call for feedback, stakeholders and individual citizens were invited to provide their feedback on The Scope. The call for feedback was open from 18 December 2015 to 17 January 2016 and an online questionnaire was made available on the ECIBC web hub via the EU Survey platform. The JRC received a total of 82 valid responses, from 40 individuals from 18 different countries and from 42 organisations from 20 different countries. During a meeting held in Varese (Italy) in March 2016, the GDG discussed the new version of The Scope which was prepared taking into account the results of the call for feedback. The Scope was finalised and approved by the GDG after some minor editing on 6 September 2016 and was later made publicly available together with this report.

Conclusions: Accreditation status is diverse across Europe; some countries don't harbor accredited/certified organisations on breast cancer care, and others use different standards in different stages of care. A harmonisation in quality benchmarking would enhance equality in patients' care.

These tasks fall under the European Commission Initiative on Breast Cancer henceforth shortened to the ECIBC. Taking the view to develop the tasks in a coordinated, consensus-based and sustainable way, the JRC first embarked on setting up a series of targeted meetings with a wide range of stakeholders, experts and concerned authorities at the national level. This report is a summary of those meetings.
