Les études montrent que plus le repérage et la prise en charge des difficultés langagières sont précoces, plus le risque de conséquences ultérieures diminue : troubles persistants de langage, l'échec scolaire et professionnel, les troubles de socialisation, etc...Les recommandations actuelles demandent de rechercher des facteurs de risque mais ils ne sont pas spécifiques au langage et limités. Notre objectif principal était de créer une fiche-synthèse en comparant le profil d'enfants participants au programme « atelier jeux de langage » avec les données de la littérature ayant retrouvé d'autres facteurs. Notre second objectif était de promouvoir l'intérêt des compétences psychosociales en médecine. Matériel et méthode : étude quantitative, multicentrique et descriptive après la collecte de données issues de questionnaires remplis par les parents. Résultats : parmi 5 centres, 61 profils d'enfants ont été analysés et comparés à la littérature. La majorité des enfants présentaient les facteurs de risque de retard de langage suivants : sexe masculin (68,85%), catégorie socioprofessionnelle parentale à faible diplôme (40,38% des pères, 70,49% des mères), absence de mode de garde (67,21%) et exposition aux écrans prolongée (55,75%). Il persistait des signes d'alerte : absence de pointage (16,39%) et moins de 50 mots (100%), à repérer respectivement à 12 mois et 24 mois. Seuls 2 généralistes ont orienté un enfant vers l'atelier (3,28%). Conclusion : le généraliste doit porter une attention aux populations à risque, évaluer l'environnement de l'enfant, repérer les signes d'alerte et orienter vers une prise en charge en impliquant l'entourage. Il doit inclure l'enfant dans un parcours de soins grâce à une plateforme de coordination où des consultations y sont organisées rapidement afin d'éliminer les causes secondaires et intervenir précocement. Si peu de signes d'alerte sont présents et que l'environnement semble défavorable, le généraliste peut prodiguer des conseils de guidance parentale et orienter vers des programmes de stimulation tels que l'atelier langage qui renforce les compétences psychosociales de l'enfant et de ses parents dans un environnement riche en stimuli. Les résultats de cette enquête ont initié la mise en place d'une action de prévention primaire auprès de PPE. Nos résultats ont révélés une amélioration significative des capacités des PPE en terme de repérage précoce après l'action de prévention primaire. En informant les PPE sur les spécificités du repérage des troubles de l'oralité du langage oral et de la communication, les orthophonistes ont pu consolider les compétences des PPE qui pourront aboutir à un repérage des troubles de l'oralité, du langage oral et de la communication plus efficace chez l'enfant de 0 à 3 ans.

Depuis 2006, la lutte contre l'inégalité des chances permet aux zones d'éducation prioritaire (ZEP) de renforcer le repérage et la prévention dans les écoles. Les enfants préscolaires demeurent cependant isolés des mesures préventives. L'objectif est d'évaluer les performances langagières dans une population de 40 enfants âgés de 18 à 30 mois vivant en ZEP et de déterminer la nécessité d'un repérage et d'une prévention précoces du retard de langage. Il s'agissait d'une étude transversale. Les enfants étaient recrutés dans un centre de santé grenoblois entre janvier et octobre 2008. Etaient analysés chez l'enfant la production verbale selon l'Inventaire Français du Développement Communicatif de l'enfant (IFDC) et le quotient de développement du langage en compréhension et en expression selon l'Inventaire du Développement de l'Enfant (IDE). Une étude descriptive des facteurs de précarité de la famille a été réalisée. 47% des enfants avaient selon l'IFDC une production verbale inférieure au dixième percentile ; 57% avaient selon l'IDE un quotient de développement inférieur à 90 pour le langage en compréhension ; 50% pour le langage en expression. Ces résultats étaient en faveur d'un retard précoce en production ou en compréhension pour la moitié des enfants analysés. La précarité familiale traduite par un score Epices parental de quintiles 4 et 5 était un facteur défavorable au développement du langage en compréhension. Létude suggère la nécessité d'un repérage et d'une prévention précoces du retard de langage chez l'enfant préscolaire de milieu précaire.
