Early detection of different retinal-related diseases will dramatically reduce the risk of vision loss. Optical Coherence Tomography (OCT) is a relatively new imaging technique which is of great importance in the identification of ocular and especially retinal diseases. Thus, the efficient analysis of OCT images provides several advantages. In this thesis, we propose a series of image processing and machine learning techniques for the automated analysis of OCT images. The proposed methodology in chapter 2 localizes different retinal layers using a modified version of active contour models. In chapter 3, we propose a method which classifies OCT images based on different pathological conditions using novel methods, e.g., transfer learning and new texture detection techniques. The proposed methods along with the clinically meaningful extracted characteristics provide numbers of applications and benefits, e.g., saving a considerable amount of time and providing more-efficient and -accurate indices for the diagnosis and treatment of different ocular diseases to ophthalmologists and finally reducing the overall risk of vision loss.

Book excerpt: Abstract: Central comeal thickness (CCT) is important in the diagnosis and management of glaucoma, in the management of refractive surgery patients and in the management of diseases such as keratoconus and Fuch's endothelial dystrophy. With the increase in the number of extended wear contact lens patients, corneal thickness is useful in monitoring the amount of edema that occurs with this lens modality. CCT can be measured using different techniques, the most common being optical and ultrasonic. Ultrasonic pachymetry is the clinical gold standard. Presently, there are many instruments on the market that employ optical techniques in a variety of ways, such as interferometry and they can be used in the measurement of CCT. Before these instruments can be reliably used, it is important to determine the repeatability of the instruments for measuring central corneal thickness on the eyes of normal subjects. The purpose of this study was to determine the within and between session repeatability of the Orbscan topographical system, the optical coherence tomography system (OCT) device, the confocal microscope, the spectral oscillation interferometry (SOl) system and the ultrasonic pachymeter. Twenty subjects (11 males and 9 females) were enrolled in the study after performing a slit lamp exam to rule out any ocular pathology. The mean age and SD of the subjects were 26.7 ± 6.0 years. The CCT was determined on the right eyes of twenty normal subjects using the five instruments stated above. The order of testing was randomized for the OCT device, SOT system, Orbscan system and confocal microscope, and the ultrasonic pachymeter was the last instrument to be used on all occasions. At the first session, two thickness measurements were found with the OCT device, confocal microscope and Orbscan system. Ten measurements were made with the SOI system, and five measurements were made with the ultrasonic pachymeter. The second session was performed within four months of the first session about the same time of day as the first session. The intra-ocular pressures of the right eyes were determined at the end of each session. Statistical analyses were performed to determine the within and between session repeatability of the five instruments. The bias and 95% limits of agreement (L0A) based on the Bland and Altman method were calculated for within session repeatability of the OCT device, confocal microscope and the Orbscan device. The coefficient of repeatability (CoR) was calculated for within session repeatability of the SOI and ultrasound methods. The bias and 95% LoA were calculated for the between session repeatability of all the instruments. Finally, ultrasonic pachyrnetry was compared with each of the other instruments using a repeated measures of analysis of variance to compare the methods while adjusting for the correlation that exists between observations collected on the same subject. The mean CCT averaged over session for each subject was 532.92 + 38.12 um for the ultrasound pachymeter, 519.30 ± 35.32 um for the OCT device, 555.68 ± 48.26 um for the Orbscan system, 546.02 ± 37.23 um for the confocal microscope and 512.54 ± 34.18 um for the SOI system. overall CoR of 1.90.
