A thorough survey of the field is ensured by the authors' considerable scientific experience. Tumors of the kidneys and the adrenal glands are being diagnosed and treated by physicians of different medical disciplines. For both types of tumor, the pathologic cellular substrate is of crucial importance in diagnosis and therapy. In recent years significant diagnostic advances have been made, ranging from angiography through ultrasonography and computer tomography to immunology. New impulses in oncologic therapy have occurred in surgery, radiation therapy, and tumor emboliza tion. A further important topic is renal tumors in infants. Such tumors involve special aspects of both diagnosis and therapy and also have a distinctive prognosis. There has been a complete change of diagnostic procedure in the detection of renal tumors, which is now based on sonography, computed tomography, and nuclear magnetic resonance imaging, pushing intravenous urography and angiography completely into the background. Also, new methods of treat ment with radionuclides using embolisation are incorporated. The description of morphological structures forms the basis for under standing and recognising pathology of the kidneys and adrenals. The con tents of pathological morphology could be extended, as we are of the opinion that the detection and therapy of renal and adrenal tumors are derived from different areas of diagnostic science. An attempt has been made to give in the text only that which has stood the test of the microscope. Many of the older tables of tumors of the kidney have been disregarded because it seemed to the writer that the classification used in these tables was hopelesslv confusing. Particular pains have been taken with the chapter on carcinoma, sarcoma, and adenoma of the kidney, and only a few of these tumors have been described, tumors which were at the disposal of the writer, and which were found in the Massachusetts General and Boston City Hospitals. These cases were most carefully studied at the hospitals mentioned by the regular corps of pathologists. The microscopic sections of the tumor have all been preserved, and the filed reports of the cases have been kept. The recent advances in the understanding of steroids, catecholamines, and utilization of computed tomography and magnetic resonance imaging have rendered disease of the adrenal gland more rewarding in terms of early detection. Although a number of basic and clinical improvements have been achieved in these diseases, there are still a number of unresolved problems, including the lack of effective cytotoxic agents for therapy of various disseminated adrenal malignancies.
