Book excerpt: Abstract: Gymnasts are frequently referred to physical therapy due to a high incidence of injury within the sport. Once an injury has been treated, physical therapists are faced with a lack of research enabling them to determine appropriate timeliness in returning the athlete to participation. Many therapists believe this determination, as well as injury prediction, may be made based on the evaluation of strength. Purpose: The purpose of this study was to examine the relationships between isokinetic strength scores, functional test measures, and incidence of injury in collegiate-level gymnasts. Subjects: Two Springfield College female gymnasts participated in this study. Both participants were 18 years of age, and had been competing in gymnastics for a total of 8 years. Methods: All subjects were tested in October of 2000 and January of 2001. During each testing session, each subject was instructed to complete a demographic questionnaire prior to testing. Lumbar range of motion was then assessed using the BROM IIR. Isokinetic strength testing utilizing the CybexRNorm dynamometer was performed next. Testing protocol consisted of the performance of concentric/eccentric quadriceps contractions at three angular velocities (60/second, 180/second and 300/second). Following the isokinetic strength testing, each subject performed the modified vertical power jump test, co-contraction test, and single leg 6-m hop for time test. Isokinetic testing was not conducted during the final testing session in January 2001 due to mechanical failure of the CybexRNorm unit in October 2000. Analysis: Inadequate sample size left the researchers unable to statistically analyze the gathered data as intended. Descriptive statistics were evaluated to examine measurement trends within and across each testing session. Results: Subject one showed no improvement in performance from baseline testing (October 2000) to final testing (January 2001) except during the co-contraction test, and perception of readiness to compete. Intra-trial trends demonstrated that subject one achieved better performance in later trials (trials 2 and 3) than in earlier trials (trial 1), except in measurements obtained for lumbar extension range of motion and modified vertical jump trials recorded in January, 2001. Analysis of baseline testing information showed that subject two also performed more efficiently during later trials. Due to injury, final testing data could not be gathered for subject two. Conclusion: As a result of a non-correctable mechanical failure in the CybexRNorm Dynamometer and a limited sample size, the researchers were unable to accept or reject the null hypothesis. Further research is required to establish the relationships between isokinetic strength scores, functional test measures, and incidence of injury in collegiate-level gymnasts.

Book excerpt: Abstract: PURPOSE: The purpose of this study was to examine the relationship between strength, functional test outcomes, and perception of readiness and injury rates and severity among intercollegiate gymnasts. SUBJECTS: Nineteen members (6 male, 13 female) of the Springfield College gymnastics teams participated in the study. The male subjects trained for an average of 10.2 + 3.6 months per year and competed 3.8 + 2.1 months per year while the female subjects trained an average of 11.5 2 1.4 months per year and competed 4.4 + 2.3 months per year. METHODS: All subjects were tested before the start of the competitive season, and prior to the occurrence of injury to obtain baseline data. A questionnaire was utilized to collect data on variables related to demographics, history of gymnastics activity, previous injuries, and current perception of readiness to compete. Subjects completed an isokinetic test of quadriceps and hamstring strength at three speeds, and three functional tests: A cocontraction test, a vertical leap test, and a single leg hop for time test. One female subject sustained an injury that resulted in the inability to participate for 3 days and as a result met the operational definition of injury. This subject was re-tested 24 hours after clearance by an athletic trainer. STATISTICAL ANALYSES: Descriptive statistics were used to obtain a demographic profile, injury history profile, and sports participation profile. Pearson product-moment correlation coefficient was used to investigate relationships between variables. Linear multiple regression analyses and the coefficients of determination were used to explain variance in isokinetic and functional test outcomes. Independent t-tests were used to identify significant differences between males and females with respect to isokinetic strength or functional test outcomes. Wilcoxon signed ranks tests were used to compare the performances of the six subjects that participated in both the first and second years of this study. RESULTS: Significant differences were found between males and females in the isokinetic tests, and all three functional tests. Strong significant positive relationships were found between the three isokinetic strength speeds. The vertical jump test showed a high positive correlation with all three isokinetic testing speeds. The cocontraction test demonstrated a significant negative correlation with all three isokinetic testing speeds. The 6- meter single leg hop test showed a significant negative correlation with the isokinetic test at 60o/sec. The Wicoxon signed ranks test showed the only significant differences in the performance of the six subjects participating in both years of the study was found in the vertical jump and cocontraction tests. The coefficients of determination showed that each functional test can explain between 22% and 49% of the variance in the isokinetic strength test. CONCLUSIONS: The combination of the three functional tests (vertical jump, 6-meter single leg hop, and cocontraction) may be an adequate substitution for isokinetic strength testing as a measure of rehabilitation status. Future research is needed to establish normative values for the functional tests in different populations as well as determining additional variables that explain the remaining variance in the isokinetic strength testing.

Book excerpt: Abstract: BACKGROUND AND PURPOSE: Due to the high level of competition and large amount of time involved for college competition, both men and women gymnasts are at considerable risk for injury. Multiple methods to assess strength and function (including manual muscle, CYBEX isokinetic, and functional tests) are commonly used for determining appropriate time for return to competition level after injury. The purpose of this longitudinal study was to examine relationships between various strength and functional test outcomes and injury incidence and severity as noted by time loss from full participation among college-level men and women gymnasts. SUBJECTS: Fifteen Division III gymnastic team members (6 male and 9 female), who competed during the Spring 1998 season, participated in this study. The average subject was 19.2 years old, and had competed in gymnastics 4.2 months in the past year at the time of initial data collection. METHODS AND MATERIALS: Subjects completed a questionnaire containing demographic, medical, and athletic activity history on the initial day of testing. Subjects were tested in a sequence of assessments starting with manual muscle, cocontraction, vertical leap, and ending with CYBEX isokinetic testing. The protocol stated that following an injury and subsequent completion of guidelines set by the athletic training department for return to practice and competition at pre-injury level, the testing sequence was repeated on the day before return to prior activity level. Injury was operationally defined as any lower extremity problem which forces an athlete to miss a minimum of one practice or competition (24 hours). ANALYSES: Descriptive statistical analyses were used to obtain a profile of sample characteristics. Pearson correlations were utilized to assess relationships between ratio and interval level variables, and point biserial correlations were utilized for ordinal and nominal level variables. A t-test was performed to test gender differences as they relate to perception of readiness to compete. RESULTS: No injuries occurred during the data collection period. A significant inverse correlation (r= -0.59) was found between cocontraction results and vertical jump measurements, such that subjects who completed the cocontration test in less time had higher vertical jump scores. Vertical jump results and all CYBEX testing velocities had a significant correlation (ranging from r= 0.65 to 0.83). A significant correlation (ranging from r= -0.54 to -0.68) was observed between cocontraction results and all CYBEX testing velocities with the exception of 60â/second for the left lower extremity. CONCLUSIONS: High correlations were found between functional testing and CYBEX isokinetic testing, implying that an overlap exists in the parameters being tested. The authors believe that using cocontraction testing, in addition to another test, will provide a more complete profile. RELEVANCE: The results of this study will assist sports medicine practitioners to determine the best test or combination of tests to determine when an injured gymnast should return to prior level of activity.

SUBJECTS: Ten female and five male Division III Springfield College gymnasts participated in this study. In order to undergo initial testing, subjects were required to complete a full collegiate practice prior to the start of the 1999 season. METHODS: Subjects completed a questionnaire containing demographic, injury, and athletic history, and a Visual Analog Scale measure of perception of readiness to practice and compete. Subjects then went through a battery of testing procedures including: Cybex isokinetic testing of bilateral quadriceps and hamstrings strength at 60o/sec and 180o/sec; modified vertical jump test; single-leg 6m hop for time; and co-contraction test. These tests would be administered again prior to the return of a subject to practice/competition after an injury. An injury was operationally defined as any pathology in lower extremity tissues that interfered with training and results in a revision of routine for competition. ANALYSES: Independent samples t-test was used to identify significant differences between gender in respect to perception of readiness to practice and compete, strength, and functional outcomes. Point Biserial correlations were used to identify the relationship of gender with these variables and to reinforce the results of the t-tests. A Pearson Product-moment correlation coefficient was used to determine significant relationships between variables. An alpha level of .05 was used to test for significant results in each analysis. RESULTS: No injuries occurred during the data collection period and one subject was lost due to a season-ending injury. A significantly high correlation was found between right quadriceps peak torque at 60o/sec and average vertical jump (r = .871). A significant correlation was also found between right quadriceps peak torque at 180o/sec and right single leg hop (r = -.809). The investigator attempted to see if positive and negative life stresses were predictive of athletic injury. Subjects consisted of 16 male and 29 female gymnasts from the men's and women's varsity gymnastic teams at Springfield College and the women's gymnastic team at Southern Connecticut State University. Data from the Athletic Life Experiences Survey (Passer & Seese, 1983) along with injury data, the number of days of practice, and meets missed due to an injury, recorded by the athletic training personnel at each institution were collected and used to test the hypothesis. Negative, but not positive, life stress was found to be a significant predictor of athletic injury among college level gymnasts. Twenty-six percent of the variance in injury days was explained by negative life event scores (B = .2655). These results suggest that a high level of negative life stress can predispose a gymnast to injury.

Book excerpt: Previous research on gymnastics injuries has examined several differences in the types of injuries and event/location where injury is most likely to occur. This research shows that male gymnasts are more likely to have more upper body injuries compared to lower body injuries whereas female gymnasts are more likely to have lower body injuries. The majority of all gymnastics injuries are sprains that are most likely to occur during the landing phase on the floor exercise during routine performance or competition. Gymnastics injuries are also more prevalent in older gymnasts, like those at the collegiate level. However, there is limited research on the effects of limb dominance on injury occurrence in both male and female gymnasts at the collegiate level. This study was designed to examine the effect of both upper and lower body limb dominance on injury occurrence in Division I male and female gymnasts at Arizona State University during competition season. Thirty-seven subjects were recruited from the Arizona State University Men's and Women's Gymnastics teams. Athletic trainers/coaches from each team were asked to record injury incidence during the 2013 competition season from January through April. Injury type, body location, event of occurrence, and location of injury (practice or competition) were recorded along with the gymnast's upper and lower body limb dominance (right or left). Statistical analysis shows that there is a significant difference between male and female gymnasts in that female gymnasts are more likely to be injured than their male counterparts (P = 0.023). However, there were no significant findings between limb dominance and injury incidence. Limb dominance did not show any relationship with side of injury, but a trend in the data shows that right-sided dominant athletes, both upper and lower body, were more likely to be injured overall than left-sided dominant athletes. A trend in the data also shows that injury is more likely to occur on the floor exercise than any other gymnastics event for both men and women.

Book excerpt: The Functional Movement Screen (FMS) is a tool designed to identify limitations and compensatory movement patterns of individuals in order to help improve performance and decrease injuries. While research has demonstrated that the FMS can be assessed reliably across raters, evidence for the validity of the FMS to predict injury and performance is scarce. PURPOSE: The purposes of this study were to: (a) examine the ability of the Functional Movement Screen and a bilateral weight distribution measure to predict injuries in collegiate track and field athletes over the course of a season, and (b) determine if FMS scores are related to performance using the standing vertical jump performance test. METHODS: Collegiate track and field athletes (N = 36) completed the FMS protocol, a bilateral weight distribution test as a comparison measure, and the vertical jump as a performance test. The FMS consists of seven functional movements scored on a 0-3 scale rating the quality of movement patterns. A bilateral weight distribution test was used as a comparison measure to examine injury prediction. The bilateral weight distribution test was conducted by having the athlete stand on two scales with one foot on each to record asymmetrical differences in body mass. Measurements were taken with the athlete's feet at shoulder width distance apart and again with feet placed apart at 1/3 of his or her height. Results were categorized into positive and negative tests for the FMS based on a previously established cutoff score. A positive test was defined two ways: (a) a difference in body mass between scales of ≥ 3% and (b) a difference in body mass between scales of ≥ 5%. Interrater reliability was estimated by using a one-way analysis of variance to assess the consistency between two raters who concurrently scored 15 athletes. Injuries were monitored by the team's head athletic trainer and reported weekly throughout the season. Sensitivity and specificity values were calculated to examine the accuracy of the different screening methods to identify participants who were injuries or not injured. An accuracy statistic was calculated to show the probability of correct diagnosis (sustaining an injury and having a positive test or not sustaining an injury and having a negative test). Pearson product-moment correlations were calculated to examine the association between FMS and standing vertical jump performance. RESULTS: Interrater reliability for the total FMS score between the lead and secondary rater was excellent (ICC = .98, 95% CI = .94-.99). The mean ± SD for total FMS scores among all participants was 14.9 ± 2.7 (male: 15.4 ± 2.9; female: 14.8 ± 2.7). Of the 35 athletes, 25 athletes (71%) experienced at least one injury during the 9-week outdoor season. Due to a small sample size for males (n = 11), analysis focused on the female sample (n = 25). For the FMS among females, sensitivity was .65 and specificity was .75. Accuracy results showed that the FMS (accuracy = 68%) correctly diagnosed injury status better than the bilateral weight distribution measure with feet at shoulder width (accuracy = 32% [greather than or equal to] 5% difference in body mass and accuracy = 36% at [greather than or equal to] 3% difference in body mass) and at 1/3 of the participant's height (accuracy = 48% [greather than or equal to] 5% difference in body mass and accuracy = 36% at [greather than or equal to] 3% difference in body mass. Both tests were poor at predicting injury in the sample. No significant relationship was found between total FMS score and vertical jump performance for females (r = .11, p = .61). CONCLUSION: The FMS can be used reliably with track and field athletes; however, the ability of the FMS to accurately predict injury status was not supported in the current study. Although the FMS was slightly more accurate than the bilateral weight distribution measures at predicting injury status, the accuracy of the FMS to predict injury status in female in track and field athletes was modest. Total FMS scores were not related to vertical jump performance in female track and field athletes.

Book excerpt: Division I female soccer athletes have been documented to succumb to the greatest amount of lower extremity injuries, predominately ACL injuries. The risk for a lower extremity injury becomes greater when alterations to functional biomechanics are present. Specifically, the hip abductor strength has a crucial role in maintaining pelvic alignment. The purpose of this study was to determine if the hip abductor strength of a female athlete alters biomechanics during functional performance. Thirty-two Division I female soccer athletes participated in this study (20+/-2yrs, 64.5+/-16.5kg, 166+/-14cm). Descriptive and anthropological statistics were recorded prior to participation. The athletes' isometric hip abductor strength was obtained through a side-lying protocol immediately followed by a Tuck Jump Assessment. The athletes were tested at three different points during the season (preseason, midseason, and postseason). A repeated measures ANOVA was performed for both the change in the functional performance and hip abductor strength over the season with a Pearson correlation used to analyze the relationship between functional performance and hip abductor strength. The results found significance in the change of strength over the season (Preseason-Midseason p=.000, Midseason-Postseason p=.000, Preseason-Postseason p=.001 ), no significance in the change of functional performance (Preseason-Midseason p=1.000, Midseason-Postseason p=.110, Preseason-Postseason p=.522 ), and a weak negative relationship between hip abductor strength and functional performance (Preseason-Midseason r=-.134, Midseason-Postseason r=-.306, Preseason-Postseason r=-.021). The study concluded hip abductor strength changes over the course of the season with a weak relationship between hip abductor strength and function performance.
