Available in PDF, EPUB and Kindle. Book excerpt: The nursing assistant has a special and vital role in nursing home care. Providing continuous and close relationships with the nursing home residents, the assistance is challenged daily to solve the numerous problems that arise in working with the elderly. It is frequently the assistant who must find ways to help when residents refuse to get up, become incontinent, cannot find the dining room, cannot sleep, grieve over their families' failure to visit, argue with other residents, make excessive demands for personal care, imagine illnesses and demons, and fear death. This book is designed to help the nursing assistant and other elder care providers develop and refine the needed understanding of the elderly and how to work with them. In the everyday context of the nursing home and its residents, it identifies categories of helping opportunities, presents over 100 case studies to illustrate them, and suggests techniques that nursing assistants may use to make the most of these opportunities. The vivid, dramatic, and realistic cases are drawn from extensive field observations of nursing home residents and the work of nursing assistants, as well as from many in-depth interviews. An indispensable training and discussion guide for nursing assistants; for nurses, social workers, and other staff members of nursing homes who train and supervise nursing assistants; and for those who design and manage elderly care programs. It is also an essential resource for sociologists, psychologists, and social workers who specialize in aging or who teach courses in gerontology.

Book excerpt: Hospitals and nursing homes are responding to changes in the health care system by modifying staffing levels and the mix of nursing personnel. But do these changes endanger the quality of patient care? Do nursing staff suffer increased rates of injury, illness, or stress because of changing workplace demands? These questions are addressed in Nursing Staff in Hospitals and Nursing Homes, a thorough and authoritative look at today's health care system that also takes a long-term view of staffing needs for nursing as the nation moves into the next century. The committee draws fundamental conclusions about the evolving role of nurses in hospitals and nursing homes and presents recommendations about staffing decisions, nursing training, measurement of quality, reimbursement, and other areas. The volume also discusses work-related injuries, violence toward and abuse of nursing staffs, and stress among nursing personnelâ€"and examines whether these problems are related to staffing levels. Included is a readable overview of the underlying trends in health care that have given rise to urgent questions about nurse staffing: population changes, budget pressures, and the introduction of new technologies. Nursing Staff in Hospitals and Nursing Homes provides a straightforward examination of complex and sensitive issues surround the role and value of nursing on our health care system.

Book excerpt: According to the United States Census Bureau, the population older than 65 will reach 40 million by 2010 and increase to 85 million by 2050. The need for well-qualified and competent health care workers has also increased. Certified Nursing Assistants (CNAs) make up 85% of all nursing home personnel who provide direct care. This quantitative, descriptive study examined the attitudes, stressors and perceptions of CNAs giving care to residents with dementia in long term care facilities. The conceptual framework chosen for this study was Schaefer and Moos' model which considers work stressors in a holistic context. The study group consisted of state-tested nursing assistants caring for residents with dementia in long term care facilities in a five county area in northeastern Kentucky. A Likert-scale tool including demographics, employment history and current job data was used. Demographic information as well as Likert-scale data was analyzed to answer the research questions. Analysis of the descriptive data showed that the attitude of the nursing assistants toward their work was positive. Stressors identified most frequently were inadequate staffing, no area for staff to get away, lack of recognition from supervisors (licensed nurses), and caring for a patient who was uncooperative, angry or complaining.

Available in PDF, EPUB and Kindle. Book excerpt: The purpose of this archival research is to analyze data previously collected, examining attitudes among nursing personnel towards the elderly with regard to possible age and gender biases. Little research has been done exploring the possible combined impact of age and gender biases. Furthermore, although research on ageism in nursing has found significant correlations between nurses' attitudes and education and employment agency, many of these studies are dated and results are contradictory. This study examined nursing personnel's attitudes toward the elderly by pairing a specific case description with an attitudinal scale. The data was collected from 105 registered nurses and nursing assistants employed in nursing homes and home health care agencies in the Chicago area. Each participant received one of two possible case vignettes. Both cases describe an older adult, which varies only by gender. Following is the Aging Semantic Differential Scale, assessing the participants' perceptions of the adult in the vignette on three dimensions, effectiveness, independence, and personal acceptability. A demographic survey was also administered. The main question of this study is whether reading a story of a male or female elder influences attitudes on the Aging Semantic Differential Scale. Another set of questions involves the effect of training and place of employment on attitudes toward the elderly. Univariate analyses of variance (ANOVA) were performed to aid in interpretation of significant differences on each of the three scales. No gender differences or stereotypes toward the elderly were found in this sample either because the Aging Semantic Differential Scale was not sensitive enough to assess them or possibly because with age, gender identity becomes tangential and age becomes the dominant categorizing factor. Significant training differences were also found indicating that certified nursing assistants had more positive attitudes toward the elderly than registered nurses, and certified nursing assistants in home health care agencies had the most positive attitudes. The professional socialization process and environmental differences are discussed as explanations for these results.

This was measured by use of the Attitude Toward Old People Questionnaire and the Quality of Care Questionnaire for the RN.
