Book excerpt: Health financing is a core pillar of health systems and encompasses the three functions of how revenues for health are collected, pooled and paid out to providers of health care services. It can be leveraged to pursue key universal health coverage goals of enhancing access, ensuring quality and financial protection against catastrophic or impoverishing out-of-pocket expenditure. But knowledge on how health financing practices can be harnessed to promote the delivery of rehabilitation services is limited. This document is the first WHO resource on health financing for rehabilitation. It considers current practices for financing rehabilitation services, frames major challenges and opportunities, and offers guidance to decision-makers engaged in strengthening rehabilitation within health systems.

Book excerpt: As the culminating volume in the DCP3 series, volume 9 will provide an overview of DCP3 findings and methods, a summary of messages and substantive lessons to be taken from DCP3, and a further discussion of cross-cutting and synthesizing topics across the first eight volumes. The introductory chapters (1-3) in this volume take as their starting point the elements of the Essential Packages presented in the overview chapters of each volume. First, the chapter on intersectoral policy priorities for health includes fiscal and intersectoral policies and assembles a subset of the population policies and applies strict criteria for a low-income setting in order to propose a "highest-priority" essential package. Second, the chapter on packages of care and delivery platforms for universal health coverage (UHC) includes health sector interventions, primarily clinical and public health services, and uses the same approach to propose a highest priority package of interventions and policies that meet similar criteria, provides cost estimates, and describes a pathway to UHC.

Book excerpt: The Social Security Administration (SSA) administers two programs that provide benefits based on disability: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. This report analyzes health care utilizations as they relate to impairment severity and SSA's definition of disability. Health Care Utilization as a Proxy in Disability Determination identifies types of utilizations that might be good proxies for "listing-level" severity; that is, what represents an impairment, or combination of impairments, that are severe enough to prevent a person from doing any gainful activity, regardless of age, education, or work experience.

This book was released on 2002 with total page 34 pages. Available in PDF, EPUB and Kindle. Book excerpt: In the Balanced Budget Act of 1997, Congress mandated that the Health Care Financing Administration (HCFA) implement a Prospective Payment System (PPS) for inpatient rehabilitation under Medicare. This new PPS will be implemented beginning January 1, 2002. This report describes the research that RAND performed to support HCFA's efforts to design, develop, and implement this Inpatient Rehabilitation Facility PPS, or IRF PPS. It presents recommendations concerning the payment system and also discusses our plans for further research on the monitoring and refinement of the PPS. The new PPS will apply to rehabilitation hospitals and to distinct rehabilitation units of acute care hospitals that were excluded from the acute care PPS. Medicare patients in such facilities must receive intensive therapy (generally at least three hours per day). In addition, 75 percent of each facility's patients must have one often specified problems related to neurological or musculoskeletal disorders or burns. Since 1982, Medicare payment for these rehabilitation facilities has been made under the Tax Equity and Fiscal Responsibility Act (TEFRA). The payment amount depends on a per-case target amount that is calculated from historical costs at the facility trended forward and on the hospital's actual cost per case. Under TEFRA, there is no adjustment for changes in a hospital's case mix or for outlier cases. This lack of adjustment creates incentives for providers to specialize in relatively less-expensive cases and, as a result, might limit beneficiary access. TEFRA pays for discharges that do not include a full course of rehabilitation (e.g., short stays for evaluation, transfer cases) as full cases. These payments in excess of costs may have both quality and cost-control implications. TEFRA was widely perceived to be unfair to older hospitals.

Available in PDF, EPUB and Kindle. Book excerpt: "Women with disabilities, a large and growing segment of the U.S. population, are as a group underserved in primary health care services that are appropriate to their needs. To date, few (if any) formal studies have been done examining the short- term costs or long-term benefits of providing specialized care for these women. This paper describes the major financial issues affecting access to appropriate primary health care for women with disabilities. The assessment is based on a review of the published literature, supplemented by key stakeholder interviews, and covers issues that are relevant at the national level and in southwestern Pennsylvania specifically. The findings and recommendations should be of interest to public and private decisionmakers seeking to improve access to health care for women with disabilities"--P. iii.

Book excerpt: Countries that have made the most progress toward universal coverage have public expenditures in health equivalent to at least 6% of their gross domestic product (GDP), which is the percentage established in PAHO's universal health strategy as the benchmark for the countries. However, while higher expenditure is a prerequisite, it is not enough to combat inequities and advance toward universal health. In addition to greater resources, the quality of the expenditure must be improved, reducing health system inefficiencies. Moreover, public expenditure in health should be sustainably increased in a fiscally responsible manner. The concept of fiscal space for health refers to the ability of governments to provide additional budgetary resources for the health system without affecting the financial position of the public sector or supplanting other socially necessary expenditures. Any analysis of fiscal space, therefore, will attempt to identify the prospects for increasing health expenditure in the short and medium term to address a series of clearly established health needs. These efforts are under way at a critical time in the Region of the Americas, particularly in the countries of Latin America and the Caribbean, which are engaged in a singular health system reform process. For the first time in history, these countries have formalized their intention of increasing public expenditure in health, putting themselves firmly on the path to real and effective access to health care through the universal health strategy. Without achieving basic well-being at this level, it will be impossible to improve social cohesion and social development in the countries of the Region. This publication brings together and summarizes PAHO's studies on fiscal space for universal health in the Americas and draws on the contributions of the regional forum held in Washington, D.C. on 7-8 December 2015. With this publication, whose target audience is the technical personnel responsible for policy development, decision-makers, and authorities, PAHO hopes to contribute to the analysis and discussion of health financing policies on the path toward universal health.

Book excerpt: In the Balanced Budget Act of 1997, Congress mandated that Health Care Financing Administration (HCFA) implement a Prospective Payment System (PPS) for inpatient rehabilitation. The Centers for Medicare and Medicaid Services (CMS, the successor agency to HCFA) issued the final rule governing such a PPS on August 7, 2001 and the system went into effect on January 1, 2002. This report details the analyses that RAND performed to support HCFA's efforts to design, develop, and implement the PPS. It describes RAND's research on new function-related groups, comorbidities, unusual cases, facility-level adjustments, outlier payments, facility-level adjustments, and assessment instruments. In addition, it presents RAND's recommendations concerning the payment system and discusses the researchers' plans for further research on the monitoring and refinement of the PPS.
