Cette dernière est à moduler pour des dysfonctions masticatoires, des traumas occlusaux secondaires (mobilité > 2), des défauts de crête sévère, des effondrements occlusaux et la présence de moins de 20 dents sur arcade. Sans traitement, cette atteinte peut mener le patient à un édentement complet. En parallèle d'une prise en charge thérapeutique parodontale, un plan de traitement global multidisciplinaire est indispensable au contrôle des facteurs locaux aggravants. En effet, la thérapeutique étiologique prend en compte le projet prothétique avec ses objectifs esthétiques et fonctionnels et la nécessité de corrections orthodontiques préalables. Dans ce contexte, un Workshop mondial a été organisé par la Fédération Européenne de Parodontologie (EFP) en novembre 2021 pour développer un guide de pratique clinique concernant le traitement des parodontites de stade IV. Quatre groupes ont été créés pour aborder différentes thématiques : les thérapeutiques orthodontiques, les réhabilitations des édentements partiels, les réhabilitations d'arcade complète ainsi que l'impact de ces traitements sur la qualité de vie des patients et les résultats à long terme. Le but de ce travail est de pouvoir évaluer l'efficacité et les risques des thérapeutiques, en comparant les restaurations prothétiques fixes, amovibles et implantaires, dans le but d'appréhender au mieux l'approche thérapeutique étiologique parodontale et optimiser le pronostic de succès en basant la pratique sur des recommandations d'ordre européennes.
