Book excerpt: Contexte : La lombalgie est un des motifs de consultation les plus fréquents en médecine générale, à l'origine de répercussions médicales et socio-économiques majeures. Le repos était auparavant le traitement principal, alors qu'aujourd'hui l'activité physique en est la clé. Un mouvement sociétal est né autour de la course à pied. Les forces de compressions et rotations axiales de la colonne vertébrale en courant, demeurent encore associées à des croyances populaires négatives. L'objectif de cette étude, est de croiser les regards des patients et des médecins généralistes, sur les croyances autour de la lombalgie et course à pied. Méthode : Étude observationnelle, descriptive, quantitative, basée sur un questionnaire en ligne à destination des médecins généralistes des Hauts-de-France, et de leurs patients. Résultats : 174 patients et 133 médecins généralistes ont participé à l'étude, ils s'accordent tous sur le fait que l'activité physique fait partie du traitement de la lombalgie. 100% des médecins la recommandent d'ailleurs en cas de lombalgie chronique. Les impacts de la course à pied gardent un caractère traumatique pour les patients, et restent associés à des lésions de la colonne vertébrale (hernie discale, tassements, arthrose). La douleur demeure un signe de gravité dans leur esprit. Le fait de posséder une licence en club de sport, est statistiquement associé à une diminution des croyances négatives chez les patients, mais pas chez les médecins. Conclusion : L'opinion des médecins généralistes et des patients a évolué ces dernières années, et s'accorde désormais aux recommandations de prise en charge. Certaines croyances demeurent infondées, et des messages clés sont encore à faire passer. Le poster créé peut être un bon outil. La revue de la littérature est rassurante, la course à pied constitue un moyen de prévention contre l'ostéoporose, en augmentant la densité minérale osseuse et renforce le disque intervertébral. Des études complémentaires sont nécessaires pour quantifier l'effet dose-réponse pour cela.

Les pratiques et les représentations des médecins généralistes (MG) sur les lombalgies communes en Ile-De-France ne sont pas décrites dans la littérature depuis 10 ans. Nous cherchons à explorer les représentations sociales de la prise en charge des MG sur la lombalgie commune aigüe et chronique. Il s’agit d’une étude qualitative exploratoire, réalisée auprès de MG travaillants en ambulatoire en Ile-de-France, via des entretiens individuels semi-directifs. Il y a eu également des entretiens avec un médecin du travail et un médecin physique et de réadaptation afin d’apporter un autre regard sur les représentations des MG. Cette étude montre que selon les compétences, expériences et ressentis des MG, leurs pratiques sont variables, notamment sur la prescription de traitement, d’imagerie et la collaboration interprofessionnelle. Certaines représentations des médecins et des patients sur la lombalgie peuvent favoriser les discours discordants, les fausses croyances et influencent la prise en charge de la lombalgie. Ils peuvent se sentir démunis notamment sur la prise en charge antalgique et psycho-sociale et peuvent donc être amenés à surmédicaliser. Un renforcement de leur formation dans la prévention et la prise en charge de la lombalgie notamment dans une approche globale et dans l’éducation thérapeutique ainsi qu’une meilleure communication entre professionnels pourraient les rendre plus confiants. De plus, une meilleure détection des facteurs de chronicité pourrait avoir son importance.

Les résultats montrent qu'ils possèdent des moyens personnels pour remédier à la douleur. Quand celle-ci persiste, ils se tournent vers leur médecin généraliste. Leurs principales doléances concernent l'attente d'une écoute active de la part du praticien, la pose d'un diagnostic, la réception d'un traitement individualisé et efficace.Dans l'ensemble, ils se disent satisfaits de l'information délivrée par leur médecin, mais leur restitution de l'information est souvent lacunaire. De plus, la récurrence des symptômes et ses conséquences sur le plan professionnel a un impact psychologique non négligeable. La prise en charge doit être personnalisée, pluridisciplinaire et d'ordre biopsychosocial. Il serait souhaitable que de nouvelles recommandations concernant la lombalgie aiguë récidivante soient émises.

Méthode : étude qualitative par entretiens semi-dirigés auprès de 13 médecins généralistes dans les départements des Alpes de Haute Provence et du Vaucluse. L'analyse s'est faite par théorisation ancrée après enregistrement et retranscription intégrale, puis double encodage jusqu'à saturation des données. Résultats : d'une manière générale, les participants ont mis au centre de leur prise en charge l'ajustement au patient même si cela est limité par plusieurs facteurs. Le patient est à considérer dans sa globalité c'est-à-dire son mode de vie, ses capacités de compréhension, sa motivation et sa capacité à mettre en place les différentes thérapeutiques proposées. L'éducation reste au cœur de la prise en charge, ayant prouvé son efficacité même si parfois elle reste fastidieuse. Les médecins généralistes nous ont expliqué dépister ces patients à risque de chronicité mais leurs limitations se trouvaient dans les propositions apportées concernant l'aspect psycho-social. En effet, le travail multidisciplinaire semble être un moyen de lutte contre la chronicité mais difficile à réaliser en pratique. Le constat a été identique concernant la prise en charge du patient dans son milieu professionnel avec des difficultés à reprendre une activité. Conclusion : en définitive, le dépistage des patients à risque de lombalgie chronique est réalisé par les médecins interrogés, les principales problématiques restent dans la prise en charge psycho-sociale. Des solutions restent à imaginer : le développement de structures multidisciplinaires, renforcer la communication interprofessionnelle, développer avec les médecins du travail des aménagements de poste et reconversions, développer avec l'ARS des entretiens motivationnels dans le cadre du programme Asalée, créer un ROSP.
