Selon Gilles Saint-Paul, elles ne semblent cependant pas disqualifier un système fondé sur la concurrence de caisses d’assurance privées. Que penser de l’instauration récente de la tarification T2A ? La T2A est indéniablement un premier pas vers une reconnaissance de l’importance d’un signal-prix sur le marché de la fourniture de soins, mais certaines questions demeurent. Premièrement, le tarif T2A se comporte comme un signal-coût adressé aux producteurs de soins, mais ne véhicule pas, ou mal ; la valeur accordée par la demande de soins. Deuxièmement, la T2A ne prend pas en compte la qualité des soins. Enfin, la T2A ne sera pleinement efficace que lorsque les professionnels de santé, directeurs d’hôpitaux, médecins, répondront à des incitations qui méritent d’être développées. Gilles Saint-Paul considère essentiel d’accorder une place plus centrale aux incitations. D’abord d’un côté des assurés, il propose la création de comptes-santé individuels, ou a minima, d’un système de franchise/plafond indexé sur le revenu de l’individu. Ensuite, d’un côté des professionnels de santé, en créant les conditions d’une extension de la T2A à la médecine de ville. "

The explosion of the HIV/AIDS pandemic, the persistence of malaria as a major killer, and the resurgence of diseases like tuberculosis which were previously under control, have brought about changes in the health system, with implications for its governance, especially in view of the diminished capacity of the public health facilities to cope with a complex range of expanded needs. Government responsibilities and objectives in the health sector have been redefined, with private sector entities (both for profit and not-for profit) playing an increasingly visible role in health care provisions. The reasons for collaborative patterns vary, but chronic under-funding of publicly financed health services is often an important factor. Processes of decentralisation and health sector reforms have had mixed effects on health care system performance; while private health insurance markets and private clinics are pointers to a growing stratification of the health market, in line with the intensified income and social differentiation that has occurred over the last two decades.These developments call for health sector reforms.

To date, there has been no attempt to evaluate these different practices. Managing Children with Developmental Language Disorder: Theory and Practice Across Europe and Beyond does just this, reporting on the findings of a survey carried out as part of the work of COST Action IS1406, a European research network. Law and colleagues analyse the results of a pan-European survey, looking at how different services are delivered in different counties, at the cultural factors underpinning such services and the theoretical frameworks used to inform practice in different countries.
