These hospital acquired infections can be costly and lethal but most importantly, preventable. This research project's purpose is to analyze several studies demonstrating how central line-associated infections are preventable. The methods used for this project was a collaborative study examining the interventions that are incorporated in the On The CUSP: Stop BSI program through various research articles and how these interventions decreased central line-associated bloodstream infections in the Intensive Care Unit. This comprehensive unit-based safety program was developed with the goal of preventing bloodstream infections and provides a framework to educate health care professionals that are in direct care of patients with central lines. Physicians, physician assistants, registered nurses, radiology technicians, and other bedside care providers follow a collaborative five step process. The five step process as designed by On the CUSP: Stop BSI are to educate regarding the safety techniques, implement teamwork and communication tools, identify defects, address the defects, and collaborate with a unit leader for guidance. The infection rates are monitored before, during, and up to twelve months after the interventions were initiated. When these improvement efforts were evaluated, the data showed a significant decrease in central line-associated bloodstream infections. The conclusion is that implementing the On the CUSP: Stop BSI program produced significant and sustaining data that central line-associated bloodstream infections decreased in diverse ICU settings through multiple cohort studies that utilized these evidence-based prevention bundles over a year to two year time frame.

Book excerpt: Central venous catheters (CVCs) provide a long term venous access to critically ill hospitalized patients and other patients that require prolong intravenous (IV) treatment; however, it can predispose patients to catheter related blood stream infection (CRBSI) which can lead to increase morbidity, extended hospitalization, increase health care cost, and mortality ((Pfaff and others, 2012). The purpose of this research project was to find evidence-based solutions that could help to reduce the incidence of CRBSI among hospitalized patients with CVCs. An extended literature search and critical review was conducted which led to the following evidence. Continuing education of staff on best evidence-based practices such as the central line bundle and other standardized infection prevention guidelines related to insertion, use, and maintenance of CVCs; monitoring staff to ensure compliance with guidelines; motivating staff through team work and collaboration; and sharing data on CRBSI rate and staff performance, has proven to be effective in reducing the rate of CRBSI, and in some instances, has led to zero incidence over a period of time (Miller-Hoover and Small, 2009; Horvath and others, 2009; Wirtschafter and others, 2010; Goeschel, 2011; Petree and others, 2012). An implementation plan was developed with a step-by-step outline of how interventions will be executed and included tools for staff education and assessing for staff compliance. The plan also had an incorporated theory, Kurt Lewin's change theory (Nursing Theories, 2011), to guide the intervention. The project included an evaluation plan and tools to measure the ongoing progress of the project, and to assess the overall outcome of the intervention. A strategy for disseminating project results was also planned to guide the communication and distribution of project results to stakeholders, and to the greater nursing community. In conclusion, the project determined that when best evidence-based infection prevention guidelines is applied to everyday practice by staff who insert, use, and maintained CVCs, it improves the quality of care, promote better patient care outcomes, saves life, and reduce health care cost.
