Book excerpt: "The prevalence of medication errors has risen as patients are seen during different stages and settings in healthcare. Medication reconciliation (MR) is a process utilized to prevent medication errors by reviewing a patient's list of medications and identifying discrepancies. The literature demonstrates that education is effective in promoting MR. Therefore, the purpose of this evidence-based practice change project was to provide education to the medical staff on medication reconciliation, improve the staff knowledge and increase patient's ability to effectively managing their medication. A pre/post questionnaire was administered before the educational intervention session and following several observational telephonic medication reconciliations. In addition, patients were administered a survey following the observational telephonic medication reconciliation to provide insight into their telephonic medication reconciliation experience. The results of the comparison of the pre-and post-medication reconciliation questionnaire showed a 10% increase in the medical staff's knowledge on medication reconciliation. The patients' results were collected, and the results showed nearly 50% of patients reported an increase in confidence and motivation to self-manage their medications. These findings suggest that an MR education session successfully improved the medical staff's knowledge on the importance and the process of MR. The education PowerPoint is an effective and structured tool for teaching applicable and reliable medication reconciliation. As such, this process was also influential in creating awareness and active participation among patients' understanding of proper use of their medication, which may help prevent additional medication errors. Medications are frequently changed during hospitalization and research shows transitions in care are critical junctures for accurate communication and medication reconciliation. Consequently, discharge medication lists often include discrepancies and duplications for home medication regimens; incorrect medication reconciliation records at discharge can result in adverse drug reactions (ADRs.) The absence of an updated medication reconciliation record on discharge is one of the primary barriers to continuity of medication administration (Elliot and others, 2012). Patient understanding and medication availability promotes better medication compliance post discharge. Many hospital discharges involve elderly adults and complex medication regimens that are difficult to manage create unsafe environments for susceptible individuals. The article, "Care Transitions Service: A Pharmacy-driven Program for Medication Reconciliation Through the Continuum of Care," identified how multidisciplinary team involvement can be beneficial for addressing patient understanding and education resulting in improved medication adherence (Conklin, Togami, Burnett, Dodd, and Ray, 2014). A systematic, formal, comprehensive process in which nurses work together with pharmacy to improve compliance and reduce medication errors is thought to ensure an accurate and complete medication list to improve patient compliance and decrease medication related 30-day readmissions.

This project explores the theme of patient safety by examining the relationship between patient education and health outcomes. The microsystem, located in the heart of San Francisco, is an outpatient clinic specializing in patients with renal impairment that each diverse and unique. A pre-implementation survey found that only 38% of patients were bringing their medications to appointments. A goal of improving compliance to 50% by December 2015 was set. By educating patients about the importance of their medications and the medication process, patients will be empowered to make informed health decisions and the possibility of suffering a medication error or adverse event will be decreased, ultimately, reducing their risk of emergency room visits or even death. An educational pamphlet was created, distributed as well as a visual poster that was placed in the waiting room as a visual cue. After implementing the project, medication reconciliation compliance increased from 38% to 40%. Although the goal was not reached, the results show that change is possible. As an preop/postop outpatient unit, we see an average of 50 patients daily and perform medication reconciliations regularly. The problem is our nurses perform this task inconsistently and often incomplete, leading us to potential medication errors. During my CNL project, I worked with a committee of staff champions and developed a standard work guide for nurses to use in order to effective reconcile patient medications. We used fishbone diagrams, audit tools, surveys and led regular discussions in our huddles in order to develop this guide. Once implementation is complete, we hope to have at least 80% of our medication reconciliations to be accurate and a decrease in medication error related safety incident reports to no more than 21 reports (a 50% improvement) this fiscal year, ending in August 2016. In order to maintain sustainability, our staff champions will serve as resources for staff and we will implement our standardized medication reconciliation into our orientation and annual skills competency review.

PHP is an intensive day treatment program for adults who need crisis intervention or short-term stabilization. It also helps clients make the transition from inpatient care to home. Before and after implementation, we conducted a workflow assessment, staff surveys, and chart audits. Before implementation, medication reconciliation was completed 60% of the time upon admission and 25% of the time at discharge. We have clarified the roles and responsibilities. I created a PowerPoint presentation with many screenshots to show the physicians and nurses step-by-step how to reconcile medications in EPIC. I also created a tip sheet for them. All the physicians and nurses have attended the medication reconciliation training except for one psychiatrist who is on vacation. We have addressed some EHR design issues, so it requires less clicks and less time to complete the medication reconciliation process. Therefore, we are making good progress, although the post implementation data collection has not yet finished. The expected outcomes are a reduction in medication errors and hospital re-admissions and an increase in staff and physician satisfaction.

Book excerpt: The purpose of this study, guided by Donabedian's structure-process-outcome model, was to evaluate if a cognitive-behavioral education intervention would decrease medication transcription errors among professional nurses when admitting patients 65 years of age or older on 5 or more prescribed medications to a hospital. The conceptual framework for this study is derived from Donabedian's structure-process-outcome health model. The premise suggests improving the structure or process of a system can improve health outcomes. My intervention (cognitive behavioral education), specifically influences the process of medication history obtainment which improved medication error score. In this study, a randomized intervention design was used where professional nurses were asked to complete a medication admission list on patients' 65 years of age or older on 5 or more prescribed medications. Medication error scores were calculated by advanced practice nurses who were trained on medication reconciliation. A second medication list was obtained from the nurses after an intervention group was provided with a cognitive-behavioral education intervention. The control group completed admission medication lists as usual. The medication error score was compared between the intervention and control group. There was no significant difference in medication error score for the pre-test control (M=3.54, SD=3.51) and pre-test intervention groups (M=3.54, SD=2.89), t=.00, df= 50, p = 1.00, (two-tailed). An independent-samples t-test was conducted to compare the medication error score for the post-test control and post-test intervention groups. There was a statistically significant decrease in medication error score from the post-test control group (M=3.23, SD 3.34) to the post-test intervention group, (M=.69, SD=1.49), t=3.54, df=50, p=.001 (two-tailed). Professional nurses are responsible for admission medication lists in many hospital systems. By improving the process by which nurses obtain medication lists medication errors are reduced and patient safety improved.
