Book excerpt: Recurrent pregnancy loss presents a vexing clinical problem primarily for medical professionals treating patients in reproductive medicine and in maternal-fetal medicine. However, with its numerous causes and various suggested treatment options, the problem is more multidisciplinary in nature, involving gynecology, genetics, endocrinology, immunology, pediatrics, and internal medicine. Exploring basic science and clinical applications, the second edition of the bestselling text Recurrent Pregnancy Loss: Causes, Controversies, and Treatment provides an authoritative and comprehensive update on advances in the understanding and management of this troubling phenomenon. The book covers basic scientific topics such as genetics and cytokines and profiles major advances in immunology, endocrinology, and thrombotic mechanism. It discusses the methodology of clinical research and the application of evidence-based medicine to clinical practice. It also reviews various late obstetric complications, along with issues caused by extreme prematurity and possible resulting handicaps. This second edition presents new material on the latest controversies, featuring opinions from both sides of ongoing debates. It includes new chapters on autoimmunity, third party reproduction, the use of immunostimulants such as CSF, and Chinese medicine. Designed for specialists working in reproductive medicine clinics and those involved with maternal-fetal care, the book is also ideal for generalists and gynaecologists seeking a comprehensive view of developments in the field.

Available in PDF, EPUB and Kindle. Book excerpt: The first few months of any pregnancy are of supreme importance to the success of that pregnancy. This statement is so obvious as to be almost a platitude, yet it must be said that no aspect of pregnancy has been more neglected in the human than the first three months. Little is known of the morphological changes that occur at that time and our knowledge of the mechanisms that control this vital stage of pregnancy is almost non-existent. The explanation for this neglect of what is an obvious area for study is the difficulty of obtaining normal material. It is rare to have material to study from a healthy first trimester pregnancy and the study by Hertig and Rock!l) of early conception found by chance in hysterectomy speci mens must be unique. The information that we do have about early pregnancy is mostly gained from animal studies or single miscarriages in humans. Chromosomal defects are common but are not an explanation for the majority of recurrent miscarriages. Obstetricians have hypothesised many causes for this condition and have deve loped numerous metQods for treating it, but the studies have been poorly con trolled so that our understanding of the cause(s) has not advanced. Treatment of women with a history of recurrent miscarriage by paternal leuco cyte infusion (immunotherapy) may be yet another form of treatment that is hailed as a new advance only to be rejected when subject to rigorous testing.

Book excerpt: Chesley's Hypertensive Disorders in Pregnancy continues its tradition as one of the beacons to guide the field of preeclampsia research, recognized for its uniqueness and utility. Hypertensive disorders remain one the major causes of maternal and fetal morbidity and death. It is also a leading cause of preterm birth now known to be a risk factor in remote cardiovascular disease. Despite this the hypertensive disorders remain marginally studied and management is often controversial. The fourth edition of Chesley's Hypertensive Disorders in Pregnancy focuses on prediction, prevention, and management for clinicians, and is an essential reference text for clinical and basic investigators alike. Differing from other texts devoted to preeclampsia, it covers the whole gamut of high blood pressure, and not just preeclampsia. - Features new chapters focusing on recent discoveries in areas such as fetal programming, genomics/proteomics, and angiogenesis - Includes extensive updates to chapters on epidemiology, etiological considerations, pathophysiology, prediction, prevention, and management - Discusses the emerging roles of metabolic syndrome and obesity and the increasing incidence of preeclampsia - Each section overseen by one of the editors; each chapter co-authored by one of the editors, ensuring coherence throughout book

Book excerpt: Growing numbers of women who experience pregnancy loss of all kinds - from early spontaneous miscarriage to stillbirth - seek help from practitioners of complementary and alternative medicine (CAM) in their recovery and preparation for future pregnancy. However, CAM is largely unregulated, and can be an unfamiliar and confusing area for mainstream health professionals. Many of these also have questions about the efficacy and safety of complementary or alternative treatments, and what they may be able to offer. Meanwhile, CAM practitioners may be unaware of the medical investigations and treatments available for conditions such as miscarriage, ectopics, gestational trophoblastic disease, preterm labour, foetal reduction, intrauterine death and stillbirth, and other less common complications. This book, written by an experienced CAM practitioner, aims primarily to provide medical professionals with the knowledge and confidence to advise women seeking complementary or alternative treatments. The author draws on her experience as a CAM practitioner to illustrate key areas where integrating sound and professional CAM into standard practice can offer women benefits, including improved psychological recovery from pregnancy loss. It also gives CAM practitioners greater insight into conventional medicine, and aids them to better understand, respect and care for women experiencing pregnancy loss. This book will be essential reading for all those who work with women who have experienced pregnancy loss, including health visitors, midwives, GPs, practice nurses, counsellors, psychotherapists, and CAM practitioners of all kinds.

Book excerpt: The delivery of high quality and equitable care for both mothers and newborns is complex and requires efforts across many sectors. The United States spends more on childbirth than any other country in the world, yet outcomes are worse than other high-resource countries, and even worse for Black and Native American women. There are a variety of factors that influence childbirth, including social determinants such as income, educational levels, access to care, financing, transportation, structural racism and geographic variability in birth settings. It is important to reevaluate the United States' approach to maternal and newborn care through the lens of these factors across multiple disciplines. Birth Settings in America: Outcomes, Quality, Access, and Choice reviews and evaluates maternal and newborn care in the United States, the epidemiology of social and clinical risks in pregnancy and childbirth, birth settings research, and access to and choice of birth settings.
