Worldwide, around 2-3 million allografts are performed each year. Although the use of allograft carries a high risk of immune rejection, the mechanisms and grades of allograft rejection are different among transplant organs and types of allograft (cell, tissue, or organ allograft). Therefore, prevention and treatment strategies against allogeneic rejection also differ. In solid organ allograft transplantation, allograft prevention strategies are quite important to prolong allograft survival. This book discusses popular rejection prevention and desensitization strategies. It also addresses tissue allograft transplantation, examining the current status and future aspects of allotransplantation of heart valves, vessels, and islets, as well as vascularized composite grafts. With regards to particulate allografts, the book discusses particulate allografts combined with platelet concentrate as an effective combination for sinus lifts and post-extraction bone grafting and in oral and maxillofacial surgery. We hope that the readers will find this volume a useful resource, as it summarizes recent relevant details and issues that will facilitate the acquisition of emerging new information in allografting.

In this second edition, the editors have focused on topics relevant to evolving knowledge in the field in order to better guide clinicians in decision-making and management of their patients, as well as help lead laboratory investigators in new directions emanating from clinical observations. Some of the most respected clinicians and scientists in this discipline have responded to the recent advances in the field by providing state-of-the-art discussions addressing these topics in the second edition. The text covers the scope of human genomic variation, the methods of HLA typing and interpretation of high-resolution HLA results. Comprehensive and up-to-date, Allogeneic Stem Cell Transplantation: Clinical Research and Practice, Second Edition offers concise advice on today's best clinical practice and will be of significant benefit to all clinicians and researchers in allogeneic HSC transplantation.

Book excerpt: This report, which was developed by an expert committee of the Institute of Medicine, reviews the first three services listed above. It is intended to assist policymakers by providing syntheses of the best evidence available about the effectiveness of these services and by estimating the cost to Medicare of covering them. For each service or condition examined, the committee commissioned a review of the scientific literature that was presented and discussed at a public workshop. As requested by Congress, this report includes explicit estimates only of costs to Medicare, not costs to beneficiaries, their families, or others. It also does not include cost-effectiveness analyses. That is, the extent of the benefits relative to the costs to Medicareâ€"or to society generallyâ€"is not evaluated for the services examined. The method for estimating Medicare costs follows the generic estimation practices of the Congressional Budget Office (CBO). The objective was to provide Congress with estimates that were based on familiar procedures and could be compared readily with earlier and later CBO estimates. For each condition or service, the estimates are intended to suggest the order of magnitude of the costs to Medicare of extending coverage, but the estimates could be considerably higher or lower than what Medicare might actually spend were coverage policies changed. The estimates cover the five-year period 2000-2004. In addition to the conclusions about specific coverage issues, the report examines some broader concerns about the processes for making coverage decisions and about the research and organizational infrastructure for these decisions. It also briefly examines the limits of coverage as a means of improving health services and outcomes and the limits of evidence as a means of resolving policy and ethical questions.

Available in PDF, EPUB and Kindle. Book excerpt: Advances in immunosuppression and kidney transplant techniques have led to significant improvements in the short-term survival of the renal allograft. Long-term graft survival, however, has relatively lagged behind and has now become one of the main problems in kidney transplantation. In this Special Issue, we discuss the recent research developments in kidney transplants that may affect long-term allografts, as well as the survival of the patient. The latest developments in living kidney donation are also explored.

Book excerpt: Regenerative Medicine Applications in Organ Transplantation illustrates exactly how these two fields are coming together and can benefit one another. It discusses technologies being developed, methods being implemented, and which of these are the most promising. The text encompasses tissue engineering, biomaterial sciences, stem cell biology, and developmental biology, all from a transplant perspective. Organ systems considered include liver, renal, intestinal, pancreatic, and more. Leaders from both fields have contributed chapters, clearly illustrating that regenerative medicine and solid organ transplantation speak the same language and that both aim for similar medical outcomes. The overall theme of the book is to provide insight into the synergy between organ transplantation and regenerative medicine. Recent groundbreaking achievements in regenerative medicine have received unprecedented coverage by the media, fueling interest and enthusiasm in transplant clinicians and researchers. Regenerative medicine is changing the premise of solid organ transplantation, requiring transplantation investigators to become familiar with regenerative medicine investigations that can be extremely relevant to their work. Similarly, regenerative medicine investigators need to be aware of the needs of the transplant field to bring these two fields together for greater results. Bridges the gap between regenerative medicine and solid organ transplantation and highlights reasons for collaboration Explains the importance and future potential of regenerative medicine to the transplant community Illustrates to regenerative medicine investigators the needs of the transplant discipline to drive and guide investigations in the most promising directions

Immunosuppression is still a major approach currently used in the prevention and treatment of allograft rejection. Both editors Dr. Chen and Dr. Qian have been contributing to preclinical evaluation of immunosuppressants for more than 25 years in North America. Experts from the United States, Canada, the United Kingdom, China, Japan, Germany, Sweden, Hungary and Brazil contributed 23 chapters to this book, providing details of immunological basis in transplantation. They also describe six classes of immunosuppressive agents (calcineurin inhibitors, mTOR inhibitors, JAK-STAT inhibitors, antiproliferative agents, costimulation blockers and corticosteroids), as well as ischemia/reperfusion injury treated agents. Additionally, the new development of cell therapy in the induction of transplant tolerance is introduced. This book provides many important references for the research direction of novel immunosuppressants. Readers that will find this book useful include transplant physicians, surgeons, nurses, immunologists, pharmacologists, pharmacists, medical students, residents and trainees in transplantation.

Interest in animal-to-human xenotransplants has been spurred by the continuing shortage of donated human organs and by advances in knowledge concerning the biology of organ and tissue rejection. The scientific advances and promise, however, raise complex questions that must be addressed. This book considers the scientific and medical feasibility of xenotransplantation and explores the ethical and public policy issues surrounding the possibility of renewed clinical trials. The volume focuses on the science base of xenotransplantation, public health risks of infectious disease transmission, and ethical and public policy issues, including the views of patients and their families.

Book excerpt: This volume, written by well-known experts in the field, covers all aspects of Anti-Neutrophil Cytoplasmic Antibody (ANCA) Associated Vasculitis (AAV). The expression refers to a group of diseases, characterized by destruction and inflammation of small vessels. The clinical signs vary and affect several organs, such as the kidney, lung, skin, nervous system and others. The opening chapters give some historical hints, explain the genetic basis of the disease and provide insights into the pathogenesis derived from recent experimental studies and guides the reader through classification and nomenclature. A large part of the book is then devoted to a detailed description of the specific related diseases and their clinical presentations, the disease course, and potential complications. The advice regarding treatment is based on the best currently available evidence in this constantly evolving area. The book is part of Springer’s series Rare Diseases of the Immune System, which presents recently acquired knowledge on pathogenesis, diagnosis, and therapy with the aim of promoting a more holistic approach to these conditions. AAVs are systemic autoimmune diseases of unknown cause that affect small (to medium) sized blood vessels. They include granulomatosis with polyangiitis (formerly Wegener's granulomatosis), microscopic polyangiitis, and eosinophilic granulomatosis with polyangiitis (formerly Churg–Strauss syndrome). This volume will be an invaluable source of up-to-date information for all practitioners involved in the care of patients with these diseases.
