Available in PDF, EPUB and Kindle. Book excerpt: Advances in breast cancer research, achieved through the progress of knowledge and development of new therapies, have been translated into improved quality of care for breast cancer patients. Clinical investigations and clinical trials have made the largest contribution to the body of knowledge that finds its way to the patient. Never before during the past decades of management of breast cancer has there been such a fruitful intellectual cross-fertilization of ideas among individuals involved in the generation of hypotheses, basic research, development of drugs and treatments, conduct of clinical trials, and statistical evaluation - the results of all of which are now translated into progress in clinical care. Even issues such as the quality of life of breast cancer patients, once the domain for few, are now being openly addressed by trials and discussed in a much broader forum. The IVth International Conference on the Adjuvant Therapy of Primary Breast Cancer, also known as the st. Gallen Conference, was again attended by more than 800 scientists and clinicians interested in this broad spectrum of breast cancer research and the interactions between such diverse fields of interest and specialties as cancer pathology, molecular biology, and psychosocial oncology. This volume collects findings and conclusions presented at the conference.

Book excerpt: Cancer cell biology research in general, and anti-cancer drug development specifically, still relies on standard cell culture techniques that place the cells in an unnatural environment. As a consequence, growing tumor cells in plastic dishes places a selective pressure that substantially alters their original molecular and phenotypic properties. The emerging field of regenerative medicine has developed bioengineered tissue platforms that can better mimic the structure and cellular heterogeneity of in vivo tissue, and are suitable for tumor bioengineering research. Microengineering technologies have resulted in advanced methods for creating and culturing 3-D human tissue. By encapsulating the respective cell type or combining several cell types to form tissues, these model organs can be viable for longer periods of time and are cultured to develop functional properties similar to native tissues. This approach recapitulates the dynamic role of cell–cell, cell–ECM, and mechanical interactions inside the tumor. Further incorporation of cells representative of the tumor stroma, such as endothelial cells (EC) and tumor fibroblasts, can mimic the in vivo tumor microenvironment. Collectively, bioengineered tumors create an important resource for the in vitro study of tumor growth in 3D including tumor biomechanics and the effects of anti-cancer drugs on 3D tumor tissue. These technologies have the potential to overcome current limitations to genetic and histological tumor classification and development of personalized therapies.

Available in PDF, EPUB and Kindle. Book excerpt: Malignant growth of cells is often characterized by disorganization of tissue structure, abnormal blood vessel development, and insuffi cient vascular supply. As a consequence, the cancer cells grow in a three-dimensional pattern in atypical microenvironments which include physical, chemical, and nutritional stresses. Necrosis often develops some distance away from the blood vessels. In association with an inherent instability in malignant cell populations, and also because of the changing micromilieu, significant cellular heteroge neity emerges with regard to various phenotypic characteristics. Both biological behavior and responses to therapeutic agents can be affected. A variety of in vitro and in vivo experimental models exist for research on properties of cancer cells during growth. The multicell spheroid model was developed as a system of intermediate complexity in which three dimensional growth of cells enhances cell-cell interactions and creates micro environments that simulate the conditions in intervascular microregions of tumors or microme tastatic foci. Spheroids may change their cellular characteristics with changing environments during growth. These can be studied under controlled conditions in vitro. Interest in details of experimental methods for this model system stimulated the organization of the First International Conference in Rochester, NY in 1980, the Proceedings of which were summarized in Cancer Research in 1981. Since then there has been a rapid increase in the use of this model system, and increased research on the significance of cell-cell and cell-microenvironment interactions in biology in general.

Book excerpt: An ideal health care system relies on efficiently generating timely, accurate evidence to deliver on its promise of diminishing the divide between clinical practice and research. There are growing indications, however, that the current health care system and the clinical research that guides medical decisions in the United States falls far short of this vision. The process of generating medical evidence through clinical trials in the United States is expensive and lengthy, includes a number of regulatory hurdles, and is based on a limited infrastructure. The link between clinical research and medical progress is also frequently misunderstood or unsupported by both patients and providers. The focus of clinical research changes as diseases emerge and new treatments create cures for old conditions. As diseases evolve, the ultimate goal remains to speed new and improved medical treatments to patients throughout the world. To keep pace with rapidly changing health care demands, clinical research resources need to be organized and on hand to address the numerous health care questions that continually emerge. Improving the overall capacity of the clinical research enterprise will depend on ensuring that there is an adequate infrastructure in place to support the investigators who conduct research, the patients with real diseases who volunteer to participate in experimental research, and the institutions that organize and carry out the trials. To address these issues and better understand the current state of clinical research in the United States, the Institute of Medicine's (IOM) Forum on Drug Discovery, Development, and Translation held a 2-day workshop entitled Transforming Clinical Research in the United States. The workshop, summarized in this volume, laid the foundation for a broader initiative of the Forum addressing different aspects of clinical research. Future Forum plans include further examining regulatory, administrative, and structural barriers to the effective conduct of clinical research; developing a vision for a stable, continuously funded clinical research infrastructure in the United States; and considering strategies and collaborative activities to facilitate more robust public engagement in the clinical research enterprise.

Available in PDF, EPUB and Kindle. Book excerpt: Normal and Malignant Cell Growth is a compendium of papers from the "Proceedings of the Third Cancer Training Grant" of the University of Chicago that deals with the processes associated with malignant neoplasia, as well as the cell proliferation kinetics of normal tissues. One paper presents the techniques used in the study on the proliferation kinetics of hemopoietic stem cells, suggesting that the hemopoietic stem cell population is not homogenous but consists of a "primitive pluripotential stem cell." A series of experiments at the Brookhaven National Laboratory investigates the relationship of cell survival, specifically that of stem cells, to the survival of the irradiated test animal. One result of the experiment shows a rapid migration of a number of stem cells from shielded marrow into unshielded marrow at the pressure of a rapid circulating pool. The numbers of stem cells are somewhat dependent on the dose given to the unshielded marrow, and are greater with the greater dose. Another paper also investigates the four methods that are used in the study of cellular kinetics in human tumors. This compendium can prove helpful for biochemists, micro-biologists, cellular researchers, and academicians involved in the study of cellular biology, physiology or oncology.

Available in PDF, EPUB and Kindle. Book excerpt: Cancer screening is a prominent strategy in cancer control in the United States, yet the ability to correctly interpret cancer screening data eludes many researchers, clinicians, and policy makers. This open access primer rectifies that situation by teaching readers, in simple language and with straightforward examples, why and how the population-level cancer burden changes when screening is implemented, and how we assess whether that change is of benefit. This book provides an in-depth look at the many aspects of cancer screening and its assessment, including screening phenomena, performance measures, population-level outcomes, research designs, and other important and timely topics. Concise, accessible, and focused, Assessment of Cancer Screening: A Primer is best suited to those with education or experience in clinical research or public health in the United States - no previous knowledge of cancer screening assessment is necessary. This is the first text dedicated to cancer screening theory and methodology to be published in 20 years.
