Book excerpt: Despite the advances in conventional, novel agent and high dose chemotherapy multiple myeloma (MM) remains incurable. In order to overcome resistance to current therapies and improve patient outcome, novel biologically-based treatment approaches are being developed. Current translational research in MM focusing on the development of molecularly-based combination therapies has great promise to achieve high frequency and durable responses in the majority of patients. Two major advances are making this goal possible. First, recent advances in genomics and proteomics in MM have allowed for increased understanding of disease pathogenesis, identified novel therapeutic targets, allowed for molecular classification, and provided the scientific rationale for combining targeted therapies to increase tumor cell cytotoxicity and abrogate drug resistance. Second, there is now an increased understanding of how adhesion of MM cells in bone marrow (BM) further impacts gene expression in MM cells, as well as in BM stromal cells (BMSCs). As a result of these advances in oncogenomics on the one hand and increased understanding of the role of the BM in the pathogenesis of MM on the other, a new treatment paradigm targeting the tumor cell and its BM microenvironment to overcome drug resistance and improve patient outcome has now been developed. Thalidomide, lenalidomide, and Bortezomib are three agents which target the tumor cell in its microenvironment in both laboratory and animal models and which have rapidly translated from the bench to the bedside. Ongoing efforts are using oncogenomics and cell signaling studies to identify next generation of therapies in MM on the one hand, and to inform the design of combination trials on the other. This new paradigm for overcoming drug resistance and improving patient outcome in MM has great promise not only to change the natural history of MM, but also to serve as a model for targeted therapeutics directed to improve outcome of patients with MM.

Book excerpt: Historically, the treatment of multiple myeloma in transplant eligible patients has included induction chemotherapy followed by autologous stem cell transplant (ASCT) and a watch and wait approach until relapse. The introduction of maintenance chemotherapy has changed the traditional observation period into active treatment time. We examined the impact of lenalidomide maintenance on survival outcomes in the front line and relapsed setting. Our population included patients treated with bortezomib based induction followed by ASCT who went on to receive lenalidomide maintenance or no maintenance. Patient data was taken from the Myeloma Canada Research Network Canadian Multiple Myeloma Database (MCRN CMM-DB) which includes data from 13 academic cancer centers across Canada. Our data demonstrates that lenalidomide maintenance is associated with improved progression free (PFS) and overall survival (OS) as well as higher rates of favourable responses. We did not observe any unanticipated adverse effects. Rates of discontinuation for reasons other than relapse were low. Analysis of relapsed patients demonstrates that lenalidomide maintenance did not results in worse second PFS (time from second line therapy to second relapse, death or last follow-up) in patients who received the drug again in second line therapy. Lenalidomide maintenance was associated with improve overall survival from time of initiation of relapse therapy. Additionally, lenalidomide maintenance did not negatively impact second PFS, OS from time of initiation of relapse therapy, and rates of favourable response when lenalidomide used again in second line treatment. Our data is the first analysis of transplant eligible patients from the MCRN CMM-DB. Our data supports the continued use of lenalidomide maintenance as standard of care following ASCT. Importantly, our data suggests that the use of lenalidomide maintenance does not negatively impact survival or response outcomes when used again in second line therapy. As such, use of or relapse on lenalidomide maintenance should not be considered an exclusion criteria for use of lenalidomide in second line therapy.

Available in PDF, EPUB and Kindle. Book excerpt: Multiple myeloma is currently still an incurable disease, but during the past decade knowledge of its molecular pathogenesis has increased rapidly. This has led to remarkable progress in both diagnosis and therapy, including in particular the approval of novel and first-in-class drugs such as thalidomide, bortezomib, and lenalidomide. This book, written by internationally acknowledged experts, covers a wide range of topics relating to multiple myeloma, including history, epidemiology, pathophysiology, clinical features, staging, and prognostic systems. The principal focus, however, is on therapy, with detailed information on the various promising treatment options which give hope that this cancer will be transformed into a chronic disease or even become curable. Individualized therapy and the variety of supportive treatment options, as described in this volume, will help in achieving this goal, as well as in reducing adverse events and improving quality of life.

Book excerpt: This book provides the clinician with concise, practical guidance on risk stratification and therapeutic decision making in patients with multiple myeloma. In addition, the available clinical trial and research data are summarized into meaningful reviews and evidence-based recommendations for treatment are presented. The coverage encompasses all phases and forms of disease, including high-risk myeloma. Over recent decades, greater understanding of the biology of myeloma has fostered the development of new, more effective drugs, leading to remarkable improvements in survival. To continue this momentum, several recent prospective trials have aimed to identify further potential therapeutic targets or to evaluate various combinations of anti-myeloma agents. The increasing abundance of management options makes the decision making complex in different phases of the disease. This book will offer the clinician valuable assistance in the choice and sequencing of therapies, highlighting the need for a personalized approach that reflects the growing recognition that myeloma is not one uniform disease.

Book excerpt: A panel of leading academic and pharmaceutical investigators takes stock of the remarkable work that has been accomplished to date with proteasome inhibitors in cancer, and examines emerging therapeutic possibilities. The topics range from a discussion of the chemistry and cell biology of the proteasome and the rationale for proteasome inhibitors in cancer to a review of current clinical trials underway. The discussion of rationales for testing proteasome inhibitors in cancer models covers the role of the proteasome in NF-kB activation, the combining of conventional chemotherapy and radiation with proteasome inhibition, notably PS-341, new proteasome methods of inhibiting viral maturation, and the role of protesome inhibition in the treatment of AIDS. The authors also document the development of bortezomib (VelcadeTM) in Phase I clinical trials and in a multicentered Phase II clinical trials in patients with relapsed and refractory myeloma.

The resulting clinical trials and health services research studies require instruments to monitor the outcomes of care and the output of the health system. However, the over-abundance of competing measurement scales can make choosing a measure difficult at best. Measuring Health provides in-depth reviews of over 100 of the leading health measurement tools and serves as a guide for choosing among them.LNow in its third edition, this book provides a critical overview of the field of health measurement, with a technical introduction and discussion of the history and future directions for the field. This latest edition updates the information on each of the measures previously reviewed, and includes a complete new chapter on anxiety measurement to accompany the one on depression. It has also added new instruments to those previously reviewed in each of the chapters in the book.LChapters cover measurements of physical disability, social health, psychological well-being, anxiety, depression, mental status testing, pain, general health status and quality of life. Each chapter presents a tabular comparison of the quality of the instruments reviewed, followed by a detailed description of each method, covering its purpose and conceptual basis, its reliability and validity and, where possible, shows a copy of the actual scale. To ensure accuracy of the information, each review has been approved by the original author of each instrument or by an acknowledged expert.

Available in PDF, EPUB and Kindle. Book excerpt: Multiple Myeloma: The second issue of Emerging Cancer Therapeutics focuses on multiple myelomas also known as plasma cell myeloma, KahlerÃs Disease and myelomatosis. It is estimated that over 20,000 new cases are diagnosed each year with over 10,000 people dying from these malignancies. Multiple myelomas are highly treatable but rarely curable. Thus it is crucial for the practitioner to be up-to-date on the latest insights regarding their management. Management options and outcomes for individuals with multiple myelomas improved dramatically with the introduction of chemotherapy. Even further improvements in prognosis have occurred because of the introduction of newer therapies such as pulse corticosteroids, thalidomide, bortezomib, and autologous and allogeneic stem cell transplantation. With the increased number of therapeutic options to choose from, the clinician is better placed to offer effective therapy but at the same time is challenged to keep abreast of the rapidly changing treatment landscape and the newly emerging data that is shaping the treatment options today and in the future. Multiple Myeloma provides a comprehensive and in-depth review of this group of malignancies. This volume reviews the current literature, provides critical evaluations of the data and offers evidence-based recommendations.
