Book excerpt: Despite the advances in conventional, novel agent and high dose chemotherapy multiple myeloma (MM) remains incurable. In order to overcome resistance to current therapies and improve patient outcome, novel biologically-based treatment approaches are being developed. Current translational research in MM focusing on the development of molecularly-based combination therapies has great promise to achieve high frequency and durable responses in the majority of patients. Two major advances are making this goal possible. First, recent advances in genomics and proteomics in MM have allowed for increased understanding of disease pathogenesis, identified novel therapeutic targets, allowed for molecular classification, and provided the scientific rationale for combining targeted therapies to increase tumor cell cytotoxicity and abrogate drug resistance. Second, there is now an increased understanding of how adhesion of MM cells in bone marrow (BM) further impacts gene expression in MM cells, as well as in BM stromal cells (BMSCs). As a result of these advances in oncogenomics on the one hand and increased understanding of the role of the BM in the pathogenesis of MM on the other, a new treatment paradigm targeting the tumor cell and its BM microenvironment to overcome drug resistance and improve patient outcome has now been developed. Thalidomide, lenalidomide, and Bortezomib are three agents which target the tumor cell in its microenvironment in both laboratory and animal models and which have rapidly translated from the bench to the bedside. Ongoing efforts are using oncogenomics and cell signaling studies to identify next generation of therapies in MM on the one hand, and to inform the design of combination trials on the other. This new paradigm for overcoming drug resistance and improving patient outcome in MM has great promise not only to change the natural history of MM, but also to serve as a model for targeted therapeutics directed to improve outcome of patients with MM.

Book excerpt: This book "Contemporary Management of Multiple Myeloma" discusses in detail about the management of multiple myeloma. Multiple Myeloma is the second most common hematological malignancy in the United States. This book highlights the interplay between the malignant plasma cell and the microenvironment, and the role of genetic alterations, adhesion molecules and critical cytokines in this complex milieu. The first chapter of this book discusses epidemiology, etiology and pathogenesis of multiple myeloma. The clinical presentation of the common plasma cell dyscrasias and discuss the diagnostic and prognostic work-up with a highlighting on recent advances in chapters 3 and 4. Chapters from 5 to 9 focus on the therapeutic evolution that has occurred, and accumulate the landmark clinical trials that have changed clinical practice for up-front treatment of both transplant eligible and ineligible patients and for those with relapsed and refractory disease. These chapters also highlights on the important role of clinical research with stem cell transplantation, proteasome-inhibition and immunomodulatory drugs that form the backbone of our present day therapeutic armamentarium. Recommendations for response assessment and current understanding of the value of maximizing the depth of response in personalized therapy and patient segmentation are briefly explained in chapters 10 and 11. Chapter 12 and 13 discussed about adjunctive treatment modalities and novel therapeutic compounds in clinical trial and targets for future drug development. This book provides complete information about current landscape for multiple myeloma which is helpful in future aspects.

This has led to remarkable progress in both diagnosis and therapy, including in particular the approval of novel and first-in-class drugs such as thalidomide, bortezomib, and lenalidomide. This book, written by internationally acknowledged experts, covers a wide range of topics relating to multiple myeloma, including history, epidemiology, pathophysiology, clinical features, staging, and prognostic systems. The principal focus, however, is on therapy, with detailed information on the various promising treatment options which give hope that this cancer will be transformed into a chronic disease or even become curable. Individualized therapy and the variety of supportive treatment options, as described in this volume, will help in achieving this goal, as well as in reducing adverse events and improving quality of life.

Available in PDF, EPUB and Kindle. Book excerpt: Multiple Myeloma: The second issue of Emerging Cancer Therapeutics focuses on multiple myelomas also known as plasma cell myeloma, KahlerÃŒs Disease and myelomatosis. It is estimated that over 20,000 new cases are diagnosed each year with over 10,000 people dying from these malignancies. Multiple myelomas are highly treatable but rarely curable. Thus it is crucial for the practitioner to be up-to-date on the latest insights regarding their management. Management options and outcomes for individuals with multiple myelomas improved dramatically with the introduction of chemotherapy. Even further improvements in prognosis have occurred because of the introduction of newer therapies such as pulse corticosteroids, thalidomide, bortezomib, and autologous and allogeneic stem cell transplantation. With the increased number of therapeutic options to choose from, the clinician is better placed to offer effective therapy but at the same time is challenged to keep abreast of the rapidly changing treatment landscape and the newly emerging data that is shaping the treatment options today and in the future. Multiple Myeloma provides a comprehensive and in-depth review of this group of malignancies.
