This book contains selected papers which have been arranged in two sections, Cardiac Hypertrophy, Adaptation, and Pathophysiology and Cardiac Hypoxia, Is chemia, and Infarction. The first section, on the pathophysiology of heart hypertrophy and failure, contains 24 chapters that focus on the derangement of biochemical, physiological, and immunological processes during the de velopment of heart disease due to a wide variety of pathogenic factors. Some of the recent developments in understanding the myocardial synthetic ma chinery in heart hypertrophy are also described in these papers, and we believe that the contents of this section will stimulate further research in the area of heart disease. The second section, which mainly deals with myocardial ischemia, contains 35 chapters providing the necessary back ground for the diagnosis of ischemic heart disease and some possible ther apeutic approaches. It hardly needs to be emphasized that ischemic heart disease is a major cause of death in highly industrialized countries, but unfortunately the exact mechanism by which ischemic insult leads to the development of heart cell damage is far from understood. We are hopeful that the articles in this section will provide valuable information in this field and thus will help in improving the treatment of ischemic heart disease.

Massry Division of Nephrology, The University of Southern California, School of Medicine Los Angeles, California In the last two decades evidence has accumulated indicating that parathyroid hormone may exert a multitude of effects on many cells and a variety of organs beyond its classical targets: the kidney and the bone. These efforts have been spearheaded by nephrologists. The interest of this group of clinicians-scientists stems from the fact that patients with renal failure have secondary hyperparathyroidism and markedly elevated blood levels of PTH (1,2). If this hormone does act on various organs, it becomes plausible that excess blood levels of PTH may be harmful in these patients. Indeed, in an Editorial published in 1977, Massry suggested that the elevated blood levels of PTH in patients with renal failure may exert deleterious effects on many systems and as such may participate in the genesis of many of the manifestations of the uremic syndrome (3). Thus, the essence of the Massry hypothesis is the notion that PTH may act as a major uremic toxin. The search for uremic toxins did not yield successful results. In the last three decades many compounds have been implicated as uremic toxins. However, a cause and effect relationship between these compounds and the manifestations of the uremic syndrome has not been established in most cases.

Although most species which have ever existed are now extinct through countless millenia, the human species has successfully adapted to changing conditions ("stressors") such as ice ages, predators and parasites, wars, famine and plague, and now it is coping with rapidly changing social, economic and political circumstances. Such adaptation occurs at all levels of life- at the molecular level within the cell, at the level of the whole cell, in the groups of cells as organs, in the entire organism or individual, and in some cases, in the society in which the individual lives.

There is also much evidence that magnesium deficiency, alone and in combination with agents that interfere with its utilization, is associated with functional and structural abnormalities of mem branes, cells, organs, and systems. The manifestations of the changes caused by magnesium deficiency depend upon its extent and duration and on variable factors. Among the conditions that increase the risk of magnesium deficiency are (1) meta bolic factors that affect the absorption, distribution, and excretion of this mineral; (2) disease and therapy; (3) physiologic states that increase requirements for nutrients; and (4) nutritional imbalances. Excesses of nutrients that interfere with the absorption or increase the excretion of magnesium-such as fat, phosphate, sugar, and vitamin D-can contribute to long-lasting relative magnesium deficiency. All have been implicated in several of the diseases considered in this book. Whether their influence on the need for magnesium is a common denominator remains to be investigated further.
