Son travail de recherche doit s'inscrire dans le champ de la médecine générale. Le rôle, les capacités et les compétences du généraliste sont bien définis en France, et à l'international. Il n'existe pas, à ce jour, de consensus sur la thèse en médecine générale. Des publications ont mis en évidence les difficultés pour les étudiants à mener à bien le projet de thèse dans le champ de la discipline. Notre objectif principal était d'analyser les critères universitaires et disciplinaires d'une thèse de médecine générale. L'objectif secondaire est d'élaborer une grille critériée. Méthodes : Nous avons utilisé une approche qualitative par entretiens semi dirigés, à partir d'un guide d'entretien, auprès d'experts reconnus en médecine générale. Les entretiens sont réalisés par téléphone ou skype jusqu'à saturation des données. Nous avons réalisé l'analyse thématique des verbatims. Résultats : Les critères retenus étaient surtout la contextualisation de la question de recherche en médecine générale, ou son appartenance au champ de la médecine générale, et l'apport du travail à la discipline. Les modalités de recrutement de la population étudiée étaient débattues, mais celle-ci devait être représentative de la population cible de soins primaires. Mais surtout, le travail de thèse doit s'inscrire dans une démarche scientifique et universitaire rigoureuse. Le but ultime est de pouvoir le valoriser. Discussion : Notre travail est une première étape pour l'appropriation des critères par les instances disciplinaires (CNGE, Collège de la médecine générale). La validation des critères par les généralistes de terrain sera nécessaire. Les notions divergentes devront être tranchées lors d'un consensus d'experts.

Book excerpt: Introduction : la médecine générale a été marquée par de multiples transformations ces dernières décennies, avec notamment son universitarisation, qui à travers la loi du 17.01.2002, a abouti à la création du diplôme spécialisé en médecine générale. L'objectif principal de cette étude était d'analyser si les thèses d'exercice de médecine générale répondaient à des questionnements propres à la discipline. L'objectif secondaire était d'évaluer l'application de l'approche centrée patient dans la recherche académique. Matériel et méthode : étude quantitative descriptive et rétrospective, analysant les thèses de médecine générale soutenues à la faculté de médecine d'Aix-Marseille Université entre 2016 et 2019. Les variables retenues pour l'analyse étaient : la spécialité du directeur de thèse, les thématiques des thèses, l'appartenance ou non de la thèse au champ de la médecine générale, la population étudiée, le type d'étude et la valorisation des thèses. Résultats : 730 thèses ont été analysées. 67.3 % des travaux traitaient de thématiques appartenant au champ de la médecine générale. 54.7 % étaient dirigés par des médecins généralistes, dont 36 % étaient des maitres de stage universitaires, et 24.7 % faisaient partie du DUMG. La méthode quantitative était la plus utilisée, avec 72.7 %. 38.6 % des travaux ont été valorisés, parmi lesquels 17.7 % correspondaient à des thèses article. De plus, 63% des thèses n'intégraient pas le patient. Tendance encore plus importante parmi les travaux appartenant au champ de la médecine générale, avec 73,9%. Conclusion : les thèses de médecine générale représentent un secteur essentiel de la recherche dans cette discipline. Leur analyse permet d'identifier les problèmes inhérents à nos pratiques quotidiennes. Ce travail défend l'importance d'une analyse des thèses réalisées en médecine générale, pour évaluer au cours des années les différentes tendances et y remédier.

Book excerpt: Introduction : Depuis la mise en place du DES de Médecine Générale en 2004, il existe une volonté forte des différents départements de médecine générale d'améliorer la qualité des travaux de recherche de leur discipline. A Nancy, un certain nombre de réflexions et de projets pédagogiques ont ainsi été menés dans ce sens : création d'un conseil scientifique, d'ateliers thèse-mémoire, etc. Ainsi est née l'idée de créer un guide méthodologique de la thèse et du mémoire, à destination des internes dès le début de leur cursus. Matériel et méthode : Une première partie a consisté à déterminer les besoins, autrement dit les thématiques à aborder dans le guide. Pour cela des « brainstormings » ont été réalisés avec les membres du conseil scientifique d'une part, et d'autre part avec des internes en médecine générale à Nancy. Parallèlement ont été prises en compte les données de la littérature après recherche bibliographique systématique. Dans une deuxième partie, les différentes thématiques dégagées ont été développées au sein d'un document pédagogique à partir des différents travaux et ouvrages pertinents publiés. Résultats : La première partie a permis de dégager un canevas de 15 thématiques pédagogiques à aborder pour la réalisation des travaux de thèses et mémoires par les internes du DES de médecine générale. Une revue de la littérature, notamment francophone, a permis de développer ces différentes thématiques. Le travail a tenu compte des spécificités propres à la faculté de médecine de Nancy. Discussion : Le travail s'avère original quant à l'utilisation d'une méthode de triangulation, pour déterminer les thématiques à aborder dans le guide. Les limites concernent l'absence d'exhaustivité de la recherche bibliographique, et la position de non expert de la recherche des rédacteurs du guide. Malgré cela, le département de médecine générale de Nancy dispose désormais d'un travail pédagogique adapté à ses besoins, dont il conviendra dorénavant d'assurer une diffusion aussi large que possible.

Le rapport Pouvourville de 2009 précise qu'il est essentiel de s'appuyer sur des preuves pour améliorer les soins primaires. Il pourrait donc supposer que la recherche soit centrée patient. Mais des précédents travaux ont montré que ce n'était pas le cas. Méthode : étude descriptive rétrospective portant sur les TESMG du 01/01/2009 au 31/12/2015. La lecture des abstracts de chaque thèse par deux des auteurs a permis un double recueil des informations suivantes : spécialité du directeur de thèse, l'objet (patient ou non), la méthodologie, le champ abordé (Médecin généraliste (Mg), Mg+divers, Mg+patients, patients, patients+divers, divers, Mg+patients+divers), la rédaction en thèse article et les propositions de prix. En cas de discordance sur ces données, un consensus était établi en présence d'une tierce personne. Un modèle logistique ajusté a été utilisé pour modéliser la probabilité de réaliser une thèse n'étudiant pas le patient. Résultats : 48% des thèses n'intégraient pas le patient et 25% n'étaient pas dans le champ de la MG. Cependant, les thèses centrées patient étaient progressivement moins oubliées depuis 2010, et significativement plus centrées patient depuis 2013 vs 2009 (OR = 0,512 en 2014, OR = 0,375 en 2015). The Dialogue on Innovative Higher Education Strategies (DIES) is addressing this need in emerging countries by building platforms of exchange and offering training courses. The articles within this volume of the “Potsdamer Beiträge zur Hochschulforschung” (Potsdam Contributions to Higher Education Research) analyse and share the experiences of such training programmes held in Colombia, Democratic Republic of Congo, Guinea, Malaysia, Kenya, and Uganda. They all revolve around the best ways to address the needs and challenges in higher education leadership and management, and in building capacities in these areas.
