L'utilisation du laser à CO2 permet dans la majorité des cas d'obtenir une absence de saignement préopératoire, et des suites post-opératoires confortables avec absence de douleur et d'oedème.

Chaque laser se caractérise en particulier par son milieu actif, ce qui définit sa longueur d’onde. Le laser a plusieurs effets sur les tissus. C’est l’effet thermique qui est le plus recherché : cela va de l’échauffement à la destruction selon la puissance du rayonnement. Les tissus parodontaux sont riches en eau et en hémoglobine. Les lasers que nous utilisons doivent avoir une longueur d’onde correspondante au pic d’absorption de ces deux composants. Cinq types de laser sont donc principalement utilisés en chirurgie dentaire : diode, CO2, Er:YAG et Nd:YAP. Les lasers en parodontologie sont utilisables pour les surfaçages et le traitement des péri-implantites, en complément des méthodes traditionnelles (ultrasons et curettes manuelles). Ils permettent également la chirurgie de tissus mous le plus souvent sans anesthésie, sans suture, sans saignement et avec une cicatrisation rapide. Ce nouvel outil nécessite encore de nombreuses études pour parfaire les protocoles d’utilisations, protocoles qui doivent être scrupuleusement respectés sous peine d’échec thérapeutique.

En chirurgie buccale, il trouve de nombreuses applications, notamment dans le traitement des leucoplasies de la muqueuse buccale. Face au risque de dégénérescence élevée, le dépistage et surtout l'élimination de ces lésions précancéreuses représentent une part importante dans la prévention des cancers de la cavité buccale. L'exérèse chirurgicale est le traitement de choix de ces lésions. La chirurgie "classique " réalisée au bistouri traditionnel est la technique la plus utilisée encore aujourd'hui, mais depuis quelques années l'utilisation du laser C02 s'est de plus en plus répandue. L'exérèse de ces lésions à l'aide de ce laser offre des avantages certains qui sont mis en évidence dans cette étude. Après un bref rappel des éléments fondamentaux et des principes de base permettant l'émission d'un faisceau laser et son interaction avec la matière, sont ensuite décrites les leucoplasies de la muqueuse buccale. Par la suite est abordé le traitement de ces précancéroses au laser C02, à la fois d'un point de vue théorique et pratique. Une comparaison succincte avec les autres techniques d'exérèses utilisées met en évidence les avantages et les inconvénients du laser C02 dans ce type de traitement.
