Book excerpt: Actuellement il existe une bonne couverture contraceptive en France, et tous les moyens sont mis en œuvre pour en faciliter l'accès, notamment pour les adolescentes. Cependant, le taux d'IVG chez les mineures reste élevé. L'accès à la contraception n'est pas le garant d'une bonne utilisation. L'objectif de l'étude est d'évaluer les connaissances des adolescentes sur la contraception, de percevoir leurs attentes afin d'améliorer la consultation en médecine générale. Nous avons mené une étude prospective, transversale, descriptive et quantitative, chez des adolescentes de 15 à 17 ans, consultant en cabinet ou centre de PMI. L'étude s'est déroulée entre mars et octobre 2018, dans le département des Bouches du Rhône. Au total, nous avons obtenus 94 réponses. Une méthode contraceptive était utilisée dans 48,9% des cas, il s'agissait majoritairement de la pilule pour 82,6%. L'indice de confiance pour le médecin généraliste était de 9/10, et il était le premier prescripteur pour les jeunes filles de 15 ans (80%) et 16 ans (58,3%) celles de 17 ans privilégiaient le gynécologue (34%). Le milieu scolaire était le premier pourvoyeur d'information concernant la contraception (70%), les IST (83%) et les conduites à tenir en cas d'oubli (46,8%). Les adolescentes sous contraception sont mieux informées par le médecin traitant (p=0,01), mais 32% de notre population trouvaient l'information insuffisante, alors qu'elle permettrait pour 76% des jeunes filles d'améliorer leur observance thérapeutique. Le taux d'IVG restait élevé avec 6,3% de notre effectif ; dont 50% déclaraient avoir reçu une information sur les conduites à adopter après un rapport à risque et 67% utilisaient une méthode contraceptive. La méconnaissance et le mésusage de la contraception est important chez les adolescentes. Le rôle informatif du médecin traitant, premier interlocuteur parmi les professionnels de santé est encore insuffisant. Une information claire et adaptée à chaque patiente lors des consultations, permettrait une meilleure adhésion au traitement, et une probable diminution du recours à l'IVG.

Or il existe un paradoxe puisque le taux de recours à l'interruption volontaire de grossesse (IVG) reste stable et élevé. Le schéma contraceptif reste le même depuis plus de dix années à savoir le recours au préservatif masculin au début de la vie sexuelle puis l'utilisation de la pilule et enfin le DIU lorsque le nombre d'enfants souhaité est atteint. L'âge moyen du premier rapport sexuel en France est de 17 ans et l'âge moyen du premier enfant désiré est de 29 ans ce qui laisse près de douze années de nulliparité à couvrir par une méthode contraceptive efficace, dont l'utilisation des méthodes LARC (contraceptif de longue durée d'action) telle que le DIU semble adapté pour réduire au mieux le risque de survenue de grossesses non désirées. En effet, les adolescentes appartiennent à un groupe à haut risque de survenue de grossesse non désirée ce qui pose un réel problème de Santé Publique à l'échelle mondiale. L'amélioration de l'éducation et de l'accès des adolescentes aux méthodes contraceptives efficaces est un enjeu de Santé Publique comme l'ont démontré les études CHOICE menée aux Etats Unis et FECOND en France. Effectivement malgré l'utilisation large d'une contraception lors des premiers rapports sexuels (préservatif masculin dans 90% des cas chez les 15 à 24 ans), aucun impact sur le nombre d'IVG chez les adolescentes n'a été réellement observé. Les raisons sont multi- factorielles mais la cause principale viendrait d'un défaut d'observance et d'une mauvaise utilisation des moyens de contraception dont l'efficacité théorique avoisine les 99,7%. L'étude CHOICE a montré que le recours aux méthodes LARC a un impact significatif sur la diminution du nombre de grossesses non désirées chez les adolescentes de 15 à 19 ans ainsi qu'une nette diminution du recours à l'IVG. En conséquence, de multiples lois et recommandations établies par les Sociétés Savantes françaises et internationales s'accordent sur le fait que le DIU est une contraception parfaitement indiquée aux adolescentes au même titre qu'aux femmes plus âgées (nullipares ou multipares). Or malgré cela l'évolution de l'utilisation du DIU par les adolescentes en France est faible en dix ans. 57% des femmes interrogées par l'étude FECOND pensent que le DIU n'est pas recommandé chez les nullipares. 68% des gynécologues et 85% des médecins généralistes interrogés par cette étude affirment ne pas recommander le DIU chez les nullipares voir ne pas leur en parler du tout du fait de leur propre réticence. Or il est montré aux Etats Unis que les femmes sont trois fois plus intéressées par le DIU après avoir reçu une information claire. Il est nécessaire que les professionnels de santé transmettent l'information aux adolescentes pour espérer promouvoir l'utilisation du DIU. On retrouve de nombreuses réticences à l'utilisation du DIU chez les plus jeunes dans la littérature : majoration du risque d'infection pelvienne, problème de fertilité, risque plus élevé d'expulsion, pose plus douloureuse et plus complexe... Toutes ces réticences ont été controversées. Nous avons retrouvé de nombreux freins similaires à ceux évoqués dans la littérature dont le principal est le manque d'information et de connaissances. Elles sont nombreuses à savoir citer le « stérilet » pour au final ne pas en connaître les caractéristiques, les indications ni les représentations. Elles sont bien loin de savoir qu'elles peuvent en bénéficier. L'autre principal frein exprimé est motivé par les différentes peurs que suscite le DIU d'où l'importance de divulguer une information claire et complète. Nous avons retrouvé très peu de levier certainement à cause de leurs importantes méconnaissances sur le DIU.

L'objectif principal de cette étude est d'évaluer l'impact de la connaissance des moyens de contraception sur la diminution du taux d'interruptions volontaires de grossesse, après avoir dressé un état des lieux de la connaissance concernant les différents moyens de contraception et défini l'influence du médecin généraliste sur l'acquisition de ces connaissances. MATÉRIEL ET MÉTHODE : Étude descriptive transversale ayant lieu dans différents lycées randomisés de l'Académie de la Réunion. Les critères d'inclusion étaient : être lycéens et être âgés entre 15 et 17 ans inclus. Cette enquête par questionnaire s'est déroulée d'août 2013 à octobre 2013. RÉSULTATS : 457 lycéens ont été interrogés. Les lycéens entre 15 et 17 ans connaissent les moyens de contraception les plus prescrits et les plus efficaces avec en tête le préservatif masculin (82%) et la pilule contraceptive (80%) ; 78% pour le préservatif féminin, 72% pour le dispositif intra-utérin, avec une différence statistiquement significative entre les âges, le sexe ou le type de lycée. 57% déclarent connaître l'anneau vaginal et le patch, 55% pour l'implant. Seuls 20% connaissent le diaphragme et la cape cervicale, 29% pour les spermicides et 53% pour les méthodes naturelles. Néanmoins, ils connaissent peu sur la façon d'agir en situation d'urgence, comment se procurer ces différents moyens de contraception et leur maniabilité. Par ailleurs, le médecin généraliste n'est pas leur premier intervenant ni leur première source éducative par conséquent, le médecin généraliste ne présente qu'un impact mineur sur la diminution du taux d'interruptions volontaires de grossesse. CONCLUSION : La connaissance des moyens de contraception n'est pas suffisante pour assurer une contraception efficace et efficiente chez les adolescents. Il leur est nécessaire d'acquérir une conduite à tenir. Le rôle du médecin généraliste en prévention primaire est limité. Par conséquent, la capacité du médecin généraliste d'agir en amont sur la diminution du taux d'interruptions volontaires de grossesse chez les mineures est réduite.

Book excerpt: Introduction : Malgré la multiplicité des moyens contraceptifs, le taux d’interruption volontaire de grossesse reste constant en France. Notre objectif principal était de décrire les connaissances des adolescents de la Somme concernant les méthodes contraceptives, leurs moyens d’obtention et leur utilisation pratique. L’objectif secondaire était d’aborder leurs représentations en matière de sexualité et de contraception. Méthode : Nous avons mené une étude épidémiologique descriptive pluri-centrique, basée sur des données recueillies à partir d’un questionnaire anonyme distribué dans des collèges, des lycées et des Centres de Planification et d’Education Familiale (CPEF) de la Somme. Le questionnaire abordait les thématiques de la sexualité et de la contraception chez les adolescents : leurs connaissances théoriques, leurs pratiques sexuelles et contraceptives ou encore leurs représentations. Les données ont été recueillies auprès de 560 adolescents de 13 à 18 ans. Résultats : Notre échantillon comprenait 57,3% de filles et 42,7% de garçons, dont l’âge moyen était 15,4 ± 1,37 ans. Ils étaient issus du collège pour 43,4% d’entre eux, du lycée pour 56,2% et des CPEF pour 3,8% de l’échantillon. Seuls 3,6% des adolescents avaient eu recours au médecin traitant pour obtenir des informations sur la contraception. Les connaissances théoriques du cycle étaient maîtrisées par 47,3 % des adolescents. Le préservatif était utilisé par 82% des adolescents lors du premier rapport sexuel qui avait lieu entre 13 et 15 ans pour 59,2% d’entre eux. Le premier rapport n’était pas protégé chez 12,6% des adolescents, associé à une conduite addictive dans 23,1% des cas. L’utilisation du préservatif n’était pas systématique au décours d’une relation stable pour 36,9% des adolescents. Parmi les adolescentes sous pilule, 45,8% déclaraient avoir eu au moins un oubli au cours des 3 derniers mois. La conduite à tenir en cas d’oubli de pilule était connue par 43,9% des adolescents. 10,5% des adolescents ne connaissaient pas les moyens d’obtention de la contraception d’urgence. 82,8% des adolescents interrogés considéraient le fait de se sentir prêt psychologiquement comme un préalable nécessaire au projet d’une grossesse, devant l’autonomie financière (55,1%) ou la fin du cursus scolaire (37%). Conclusion : Accepter la sexualité des adolescents, et intégrer les facteurs sociocognitifs influençant leurs comportements contraceptifs sont les préalables indispensables à la prescription d’une méthode contraceptive personnalisée. Le médecin traitant, du fait de sa place privilégiée au sein de la cellule familiale, a un rôle fondamental pour évaluer l’adéquation de la méthode contraceptive prescrite et détecter les situations à risque d’échec contraceptif.
