Mais elle reste controversée en cas de chimiothérapie néoadjuvante pour les cancers inaccessibles à un traitement conservateur ou les tumeurs inflammatoires parce que faire une chirurgie complexe pourrait retarder le traitement adjuvant par chimiothérapie ou radiothérapie. Nous avons étudié rétrospectivement 271 patients traités au Centre Antoine Lacassagne par chimiothérapie néoadjuvante de 1985 à 2005 qui comptaient 148 mammectomies dont 65 RMI et 123 traitements conservateurs. Nous en avons conclu que l'attitude chirurgicale doit être guidée par la réponse à la chimiothérapie dont va dépendre le pronostic des patientes et qu'il ne faut pas hésiter à proposer aux patientes qui ont peu ou pas répondu une reconstruction mammaire immédiate. Ces patientes, en effet, n'auront peut-être pas le temps d'envisager une reconstruction différée avant que n'apparaissent des métastases.

La série retrouve des résultats comparables à ceux de la littérature, avec un taux de conservation mammaire de 57.6% et un taux de marges chirurgicales atteintes de 9% ayant conduit à 5% de mammectomie complémentaire. 22% des patientes présentaient une réponse histologique complète au niveau tumoral et 8% au niveau ganglionnaire, alors même que 29.7% d'entre elles avaient une réponse clinique complète. Les taux de survie sans récidive loco-régionale, sans maladie et global sont respectivement de 90%, 75.8% et 87.6%.

L’objectif secondaire est l’étude du rôle et des indications de la radiothérapie après chimiothérapie néo-adjuvante et traitement chirurgical conservateur. Matériels et méthodes : Analyse rétrospective de 290 patientes prises en charge, de 1990 à 2004, à l’Institut Bergonié à Bordeaux, pour un cancer du sein non métastatique traité par une chimiothérapie première suivie d’un traitement chirurgical conservateur et d’une radiothérapie. Etude des taux de rechute locorégionale en fonction de plusieurs facteurs pronostiques potentiels. Résultats : L’analyse univariée a permis d’identifier cinq facteurs pronostiques de rechute locorégionale, statistiquement significatifs : l’âge au diagnostique inférieur à 40 ans (p = 5.69x10-5), la négativité des récepteurs hormonaux (p = 0.0001), l’envahissement ganglionnaire clinique initial (p = 0.006), l’envahissement ganglionnaire au curage axillaire, après chimiothérapie (p 0.0008), la présence de carcinome canalaire in situ dans la pièce opératoire (p = 0.0006). En analyse multivariée, trois de ces facteurs sont indépendants : la négativité des récepteurs hormonaux (RR = 4.015 IC 95% [2.057-7.837], l’âge initial inférieur à 40 ans (RR = 2.801 IC 95% [1.592-4.926]), et l’envahissement ganglionnaire au curage axillaire (RR = 2.302 IC 95% [1.248-4.246]). Conclusion. Dans notre série, la négativité des récepteurs hormonaux, l’âge inférieur à 40 ans et l’envahissement ganglionnaire au curage axillaire sont des facteurs pronostiques de rechute locorégionale après chimiothérapie première et chirurgie conservatrice. Cela soulève la question de la prise en charge thérapeutique optimale pour ces patientes à haut risque : faut-il proposer une mastectomie, ou peut-on intensifier le traitement locorégional et notamment la radiothérapie afin de limiter ce risque de rechute ?

Le taux de rechute locale à 4 ans est alors de 4,5%. Dans 20,8% des cas une réponse histologique complète à la fois tumorale et ganglionnaire a été obtenue. En cas de réponse partielle le mode de régression tumorale était centripète dans 37,9% des cas et éclaté pour les 62,1% restants. Une amélioration de l'évaluation du résidu tumoral par l'imagerie avec le temps a été notée, notamment avec l'introduction de l'IRM systématique.

Book excerpt: Contexte : Bien que le développement de la chimiothérapie néo-adjuvante (CNA) et des techniques d’oncoplastie ait permis l’augmentation du taux de chirurgie conservatrice, le recours à une mastectomie reste le standard pour la prise en charge de certains cancers du sein (CS) multicentrique ou extensif. Cette chirurgie radicale est mal supportée par de nombreuses patientes. La volonté d’améliorer la qualité de vie des patientes en préservant la sécurité oncologique a mené au développement des séquences thérapeutiques inversées. Après un premier temps de chimio et de radiothérapie néo-adjuvante les patientes bénéficient d’une mastectomie avec conservation de l’étui cutané (MCEC) et reconstruction mammaire immédiate (RMI). Bien qu’utilisée par quelques équipes, la sécurité oncologique de cette séquence est toujours en cours d’évaluation. Notre étude a analysée le taux de réponse histologique complète (RHC), facteur pronostic de survie sans récidive. Méthodes : Entre 2010 et 2016, 103 patientes ayant bénéficiée d’une inversion de séquence thérapeutique ont été incluses dans l’étude. La chimiothérapie et la radiothérapie néo-adjuvante étaient administrées selon nos référentiels standard. Une chirurgie de clôture avec mastectomie et geste axillaire éventuel était réalisée 6 à 8 semaines après la fin de la RT. La RMI associée était réalisée par une technique de lambeau de grand dorsal afin de minimiser le risque de complication. La RHC était définie par l’absence de résidu infiltrant que ce soit sur la mastectomie ou dans les ganglions prélevés. La RHC était analysée en sous types immunohistochimiques. Résultats : Le taux de RHC était de 53,4%. Ce taux était supérieur pour les tumeurs RH- (HER2 et triple négative) par rapport aux tumeurs luminales (69,7% vs 45,7%, p=0,023). Le taux de complication était de 9,7% avec un taux de ré intervention de 7,8%. Discussion : La RHC était élevée en comparaison aux taux habituellement décrits après CNA seule (12,5%-27,1%). Ce taux s’explique par l’utilisation de l’association d’anthracyclines et de taxanes, par le recours au trastuzumab pour toute lésion HER 2 positive, et par l’ajout de la radiothérapie. Le taux de complication bas correspond aux données de la littérature. Conclusion : L’inversion de séquence thérapeutique pour les CS nécessitant une association de CT et de RT permet la RMI sans augmenter le risque de complication tout en conservant un taux élevé de RHC, et pourrait donc être considérée comme une option thérapeutique acceptable.

The neurologic symptoms are often more disabling than the primary cancer. Symptoms including confusion, seizures, pain and paralysis may be a result of either metastases to the nervous system or one of several nonmetastatic complications of cancer. The physician who promptly recognizes neurologic symptoms occurring in a patient with cancer and makes an early diagnosis may prevent the symptoms from becoming permanently disabling or sometimes lethal. This monograph, an update of the first edition published in 1995, is divided into 3 sections. The first classifies the wide variety of disorders that can cause neurologic symptoms the patient with cancer, discusses the pathophysiology of nervous system metastases, the pathophysiology and treatment of brain edema and the approach to supportive care of common neurologic symptoms such as seizures, pain, and side effects of commonly used supportive care agents. The second section is devoted to nervous system metastases, addressing in turn, brain, spinal cord, meningeal and cranial and peripheral nerve metastases, describing clinical symptoms, approach to diagnosis and current treatment. The third section addresses several nonmetastatic complications of cancer and includes sections on vascular disease, infections, metabolic and nutritional disorders, side chemotherapy, radiation and other diagnostic and therapeutic procedures. The final chapter addresses paraneoplastic syndromes.The book is intended for practicing oncologists, neurologists, neurosurgeons and radiation oncologists as well as internists who treated patients with cancer. Our attempt was to write a book that would assist oncologists in understanding neurologic problems and neurologists in understanding oncologic problems. The book is also intended for physicians training to specialize in any of the above areas. It includes a practical approach to the diagnosis and management of patients with neurologic disease who are with known to have cancer or in whom cancer is suspected.
