Book excerpt: Although, elevated risk for lung cancer has been associated with certain industries and occupations in previous studies, the lack of cigarette smoking information in many of these investigations resulted in estimates that could not be adjusted for the effects of smoking. To determine lung cancer risk due to occupation and smoking, for New Mexico's Anglos and Hispanics, a population-based case-control study was conducted. Incident cases diagnosed 1980-1982, and controls from the general population, were interviewed for lifetime occupational and smoking histories. Specific high risk industries and occupations were identified in advance and linked with industrial and occupational codes for hypotheses-testings. Significantly elevated risks were found for welders (RR = 3.5) and underground miners (RR = 2.0) with adjustment for smoking. Because shipbuilding was the industry of employment for only five of the 18 cases who were welders, exposures other than asbestos could be causal agents. Among the underground for only five of the 18 cases who were welders, exposures other than asbestos could be causal agents. Among the underground miners, uranium, copper, lead and zinc, coal, and potash mining industries were represented. Low prevalence of employment in some of the industries and occupations of interest resulted in inconclusive results.

Book excerpt: Describes the design and findings of a pooled re-analysis of epidemiological studies investigating carcinogenicity in workers exposed to wood dust. The analysis included a large number of studies whose results have not been fully explored due to limitations, mainly linked to small size, inherent in each study. By pooling these studies the IARC analysis allowed a more powerful interpretation of previously published case-control studies of sino-nasal cancer and cohort studies of workers in wood-related industries. The result is the largest data set ever assembled to examine the relationship between exposure to wood dust and human cancer. Although exposure to wood dust has long been associated with a very high excess of sino-nasal cancer, the IARC analysis aimed to answer several remaining questions concerning the possible excess risk of cancer at other sites, the level of wood dust exposure needed to cause cancer, the differences between exposure to dust from soft- and hardwood, and the reasons for the wide range of relative risks for sino-nasal cancer and the results observed for other cancers. A uniform exposure assessment strategy was applied in order to classify study subjects according to levels of exposure. The opening chapters explain the methodology of the pooled re-analysis and provide a detailed description for each of the twelve case-control studies of sino-nasal cancer and five cohorts of workers in wood-related industries. In addition to the predicted excess of sino-nasal cancer, the analysis found an excess of nasopharyngeal cancer and lymphatic and haematopoietic cancer, particularly multiple myeloma. No evidence was found for an excess risk of deaths from lung cancer.

Book excerpt: The present project aimed to regroup major studies in Europe that are able to answer more precisely the questions of long-term health effects in relation to chronic internal exposure. Advantage has been made of already existing European collaborations that worked successfully during previous EU contracts in order to strengthen expertise and increase statistical power for these low risk studies. Various complementary fields of expertise were involved: organ specific dosimetry, statistical modelling, and accounting for uncertainty in risk assessment. New studies were launched in the field of occupational exposure, by focusing on those workers for whom precise individual information on internal exposure has been registered during their occupational life. Collection and validation of these data made it possible to plan future large cohorts able to answer the question of a possible link between a disease and a specific exposure (example: uranium under different chemical forms). A case-control study realized during this program focused mainly on lung cancer and leukaemia risk. The results of these studies were expressed as excess risk coefficients per unit of exposure or per organ dose and modelled in relation to time-dependent variables. The possibility of taking into account co-factors like occupational chemicals, tobacco, gender, age at exposure, and attained age was a major component of these joint analyses based on a large amount of data collected under the commonly shared study protocol. It was possible to compare the calculated specific risk per organ dose with the more common risk per unit of exposure characterised by environmental measurements. The final objective of this large collaboration was the discussion of more or less sophisticated risk models as a tool for prediction of lifelong risks and for application to populations that differ from those directly involved in the present cohort or case-control studies. The comparison of these risk factors with those from populations exposed solely to external exposure was enhanced through this ?organ dose approach?.

Book excerpt: Nickel, chromium VI and cadmium were designated as IARC class 1 lung carcinogens in the early 1990s. However, the study results informing this designation have not been consistently replicated in the past two decades, and there remain some important questions about these metals effects at low levels of exposure. Further empiric research is therefore required to confidently reaffirm the carcinogenicity of these agents and to understand the circumstances in which they may be harmful. The objective of this study was to investigate the relationship between exposure to either nickel, chromium VI or cadmium and subsequent risk of lung cancer among workers exposed to these substances at a variety of levels and in a wide range of occupations. Two large population-based case-control studies conducted in Montreal provided the data to investigate the carcinogenicity of these substances. Study I was conducted from 1979 to 1986, and included males aged 35 to 70 diagnosed with cancer at any of 19 selected cancer sites. Study II was conducted from 1996 to 2001 and included men and women aged 35 to 75 diagnosed with lung malignancies. In both studies, cases were ascertained in all hospitals on the island of Montreal, while age- and sexstratified population controls were selected from electoral lists. Detailed job histories, as well as lifestyle and socioeconomic measures, were elicited by interviewers with each subject. Job descriptions were then evaluated by an expert team of chemists and hygienists in order to determine whether the subject was exposed to each agent, and if so, to create measures of concentration, duration, and cumulative exposure over the course of each participant lifetime. In order to determine whether lifetime exposure to any of the metals of interest was associated with increased incidence of lung cancer, exposure data were analysed by logistic regression, adjusting for relevant confounders including detailed smoking history.
