Book excerpt: Ce mémoire a pour objet d'étudier les effets théoriques et constatés empiriquement du choix de la tarification à l'activité (T2A) comme mode de financement des activités de médecine, chirurgie et obstétrique (MCO) sur la qualité de l'offre de soins à l'Hôpital public ainsi que les principaux outils disponibles pour limiter les risques de baisse de cette qualité. Dix ans après la mise en oeuvre de la réforme, instaurée par le plan "hôpital 2007", cette étude commence par la définition du champ et des modalités d'application de la T2A ainsi que du cadre d'analyse de la notion, par nature multidimensionnel, de qualité des soins. Par la suite, ce mémoire s'intéresse aux bienfaits et risques anticipés par la littérature économique, aussi bien française qu'internationale, de ce mode de financement sur la qualité du service hospitalier mais aussi sur les constats empiriques pour lesquels il est difficile de tirer des conclusions claires et univoques. Ce constat conduit, dans une dernière section, à étudier les opportunités internes et extérieures à la T2A, pour prévenir toute baisse de la qualité des soins et en permettre l'amélioration. Enfin la conclusion ouvre le débat sur les autres enjeux, notamment éthiques, qui accompagnent les récentes réformes de l'hôpital public et de son financement.

Chaque DRG regroupe des patients présentant des similitudes de diagnostic et de prise en charge. Ce nouveau mode de financement est suspecté d'avoir des effets sur la qualité et l'accès aux soins. Une revue de la littérature internationale avait démontré que quelques effets tels que le raccourcissement de la durée de séjour, le phénomène du quicker and sicker et l'augmentation des transferts vers les services de soins infirmiers avaient été notés par plusieurs études empiriques. En France, il n'y a pas eu d'études empiriques démontrant l'existence de tels effets mais le personnel travaillant au sein de l'hôpital tel que les médecins et les infirmiers éprouvent des contraintes et une surcharge de travail depuis la mise en place du nouveau mode de financement. L'expérience récente de la nouvelle tarification en France et l'absence d'études empiriques ne nous permettent pas de faire des conclusions identiques à celles retrouvées au niveau international.

Par ailleurs, elle inciterait peu à adopter une vision de long cours, et serait ainsi inadaptée aux enjeux épidémiologiques actuels. Face à ce constat, les autorités ont engagé un ambitieux processus de réforme du financement de l'hospitalisation MCO, basé sur une réduction de la part de T2A dans les recettes au profit d'une diversification des modalités. L'objectif de cette étude était d'analyser la concordance entre les attentes des praticiens de terrain concernant les modalités de financement d'une part, et le projet de réforme élaboré par les autorités d'autre part. Pour ce faire, nous avons réalisé une dizaine d'entretiens semi-dirigés auprès de praticiens hospitaliers bas-normands, qu'ils soient cliniciens, pharmaciens ou médecins DIM. Ces entretiens révèlent un accord sur l'intérêt d'un système plurimodal pour tempérer l'effet inflationniste de la T2A, ainsi que pour mieux adapter le financement aux spécificités de chaque type d'activité. Les répondants alertent toutefois sur la lourdeur logistique qu'implique un tel système, et insistent sur l'importance d'une montée en charge prudente. Le déploiement effectif des différentes propositions, retardé par la crise sanitaire, devra quoi qu'il advienne faire l'objet d'une évaluation extensive afin d'en déterminer la pertinence pour le système de santé.
