Obtenir une normoglycémie de la période périconceptionnelle à la fin de grossesse est actuellement le seul facteur pronostique identifié afin de réduire la morbidité materno-foetale même s'il existe vraisemblablement d'autres facteurs impliqués dans la genèse de ces complications. L'objectif de l'étude était de comparer le pronostic materno-foetal et le contrôle métabolique chez des patientes diabétiques de type 1, traitées par insuline en pompe ambulatoire comparativement aux patientes traitées par multi-injections. Méthode : Il s'agit d'une étude rétrospective unicentrique (hôpital Jeanne de Flandre, Lille). Les patientes diabétiques de type 1 traitées par insuline en multi-injections ou pompe, ayant accouché entre 2005 et 2013 ont été incluses. Nous avons analysé et comparé les complications maternofoetales, les issues de grossesse et l'évolution du contrôle métabolique (hémoglobine glyquée, fructosamine, doses totale, basale et bolus d'insuline par kilogramme) chez les patientes traitées par pompe ambulatoire versus les patientes traitées par multi injections. Résultats : 103 patientes sous multi-injections et 152 patientes sous pompe ont été incluses. Les groupes étaient comparables au niveau de l'âge, de l'indicé de masse corporelle, des antécédents d'hypertension artérielle et de microalbuminurie. Les patientes sous pompe avaient un diabète plus ancien, plus d'antécédents de rétinopathie et une moyenne d'hémoglobine glyquée préconceptionne'lle plus basse (p=0.02). L'équilibre du diabète était similaire dans les deux groupes pendant la grossesse. Les patientes sous pompe recevaient en moyenne moins d'insuline en fin de grossesse (p
