L'antibioprophylaxie (ABP) chirurgicale est une mesure fondamentale de la prévention des ISO. Elle correspond à l'administration d'antibiotiques avant la contamination potentielle du fait d'une situation à risque au cours d'un geste chirurgical. C'est une mesure complémentaire de l'asepsie. Elle est utilisée en pré-opératoire afin d'empêcher la prolifération bactérienne au niveau du site opératoire, de façon à réduire au maximum le risque d'infection post-opératoire. L'ABP doit être la plus efficace possible afin de limiter le risque infectieux mais également les résistances. Afin de préserver la qualité de l'ABP chirurgicale, il est recommandé d'évaluer régulièrement les pratiques au sein de ces établissements de santé en s'appuyant sur les recommandations régulièrement actualisées par la Société Française d'Anesthésie-Réanimation (SFAR). C'est ainsi que nous avons souhaité évaluer les pratiques à travers une une étude rétrospective réalisée au CHU de Poitiers sur des interventions menées en 2018 et en 2019. Les interventions suivantes ont été choisies : prothèse de hanche (PTH), prothèse de genou (PTG), hernie inguinale avec pose d'une plaque prothétique et sleeve gastrique.. Concernant l'ensemble des critères évalués, nous pouvons noter que les pratiques sont globalement tout à fait conformes avec le référentiel institutionnel s'appuyant sur les recommandations de la SFAR. Dans tous les cas l'ABP était limitée à la période opératoire. Que ce soit en ce qui concerne la molécule utilisée, la posologie, le délai entre l'ABP et l'induction ou l'incision, les taux de conformité sont compris entre 85 et 100%. En comparant nos résultats à ceux d'une précédente étude menée sur des PTH et PTG pratiquées en 2013 et 2014, il ressort une amélioration globale des pratiques. La vigilance et l'application des recommandations dans le choix et l'administration de l'ABP chirurgicale restent de mise pour continuer à prodiguer des soins de qualité aux patients opérés.

Patients et Méthodes : Il s'agit d'une étude observationnelle multicentrique sur cinq établissements de santé de la région stéphanoise. Le plan de l'étude s'est calqué sur le référentiel des pratiques professionnelles sur l'antibioprophylaxie publié en 2005 par le CFAR et l'HAS. Une première phase d'analyse structurelle a été réalisée (qualité et diffusion des protocoles locaux d'antibioprophylaxie) puis une période d'observation de deux semaines (juin2006) a été tirée au sort pour la deuxième phase qui a consisté en une analyse de dossiers visant à évaluer la qualité de l'antibioprophylaxie en terme de prescription, de réalisation et de conformité aux référentiels. Résultats : Au total, 1 100 dossiers ont été analysés correspondant à 585 prescriptions d'antibioprophylaxie. La conformité globale avec les recommandations nationales de l'indication d'antibioprophylaxie était de 88%. Les anomalies étaient principalement liées à l'absence de prescription en consultation d'anesthésie (52,1% de conformité) et au délai, non précisé ou non respecté, entre l'injection de l'antibiotique et l'incision (39,7% de conformité). Conclusion : notre étude a permis d'objectiver les anomalies de prescription. Les médecins, qui pour la plupart y ont participé activement, ont pu en prendre conscience. Elle a initié la mise en place d'actions d'amélioration, notamment la rédaction de nouveaux protocoles par chaque service. Un deuxième audit doit être réalisé afin d'évaluer leur application. Ce travail s'intégrera également dans le processus de certification des établissements.

Book excerpt: Infections that occur in the wound created by an invasive surgical procedure are generally referred to as surgical site infections (SSIs). SSIs are one of the most important causes of healthcare-associated infections (HCAIs). A prevalence survey undertaken in 2006 suggested that approximately 8% of patients in hospital in the UK have an HCAI. SSIs accounted for 14% of these infections and nearly 5% of patients who had undergone a surgical procedure were found to have developed an SSI. However, prevalence studies tend to underestimate SSI because many of these infections occur after the patient has been discharged from hospital. SSIs are associated with considerable morbidity and it has been reported that over one-third of postoperative deaths are related, at least in part, to SSI. However, it is important to recognise that SSIs can range from a relatively trivial wound discharge with no other complications to a life-threatening condition. Other clinical outcomes of SSIs include poor scars that are cosmetically unacceptable, such as those that are spreading, hypertrophic or keloid, persistent pain and itching, restriction of movement, particularly when over joints, and a significant impact on emotional wellbeing. SSI can double the length of time a patient stays in hospital and thereby increase the costs of health care. Additional costs attributable to SSI of between £814 and £6626 have been reported depending on the type of surgery and the severity of the infection. The main additional costs are related to re-operation, extra nursing care and interventions, and drug treatment costs. The indirect costs, due to loss of productivity, patient dissatisfaction and litigation, and reduced quality of life, have been studied less extensively.

One is a clearer and more suitable model for conceptualising the mechanism of humoral immune response which enables identification of antigens and antibodies and a therapeutic strategy against rejection.
