The evaluation plan has reasoning as well as tools to be used in the process of piloting the program allowing for improvement before dissemination to other facilities for a larger scale impact.

Yearly, 20-30 percent of hospitalized patients suffer an injury that could have been prevented (Ganz, Saliba, Huang and Shier, 2013). Based on documented studies, implementing purposeful rounding or intentional rounding decreases call lights, alarms, falls, increases patient and employee satisfaction, supports a more productive shift for nursing staff, and most importantly increases patient safety (Olrich, Kalman, and Nigolian, 2012). Several documented studies have reported a decrease in fall rates post implementation of intentional rounding, one hospital reported a decrease in falls by 23 percent post implementation (Olrich, Kalman, and Nigolian, 2012). Falls in the acute hospital setting are a serious complex problem, addressing and implementing purposeful or intentional hourly rounding while better serving patients will decrease the fall rate in the acute hospital setting. Based on several journal readings successful implementation of intentional rounding or purposeful rounding that included the nursing staff in the implementation process yielded the most rewarding results. Identifying the problem or issue, developing an implementation plan and evaluation plan, along with finding the best avenues to disseminate the evidence are key to successful implementation of any project.

The patient population consists of cardiovascular and thoracic post-operative surgical patients of all ages primarily adults. The implementation of the evidence based practice of hourly rounding has been proven to increase overall patient satisfaction by improving staff responsiveness and increasing patient safety within many hospitals. The measurement of effectiveness and success will be evidenced by seeing an increase in patient care scores and a reduction of falls within the unit; when these metrics are met, there is data that proves that this evidence based practice will drive the results we are looking for. In this microsystem, there are many indicators that a more efficient workflow is necessary to be established. There is the constant sound of call lights ringing about an estimated 12-15 in an eight hour shift for non-urgent needs. Upon interviewing many staff, I found there was an inconsistent way and lack of education on how hourly rounds should be performed. Basic patient needs were not addressed within each visit from a staff member. By using a process map, timeline, PDSA, Lewin's Change Theory, SWOT analysis and graphs, I can show you my process of improving staff responsiveness within the nursing unit. In conclusion, as a Clinical Nurse Leader and patient advocate, I have utilized the information gained from the microsystem data and implementation of hourly rounding, thereby proving how to improve quality, safety and patient care experiences within the busy, medical surgical nursing unit with the evidence based practice of hourly rounding.

Book excerpt: The purpose of this practice improvement project was to determine if a relationship exists between purposeful hourly rounding and incidence of patient falls and patient satisfaction with information received from nurses for male and female patients over 18 years in one 60-bed acute rehabilitation setting. The first question that guided this project was: Are patient falls in an acute rehabilitation setting reduced when purposeful hourly rounding occurs? The second question was: Do patient satisfaction scores improve when purposeful hourly rounding was implemented in acute rehabilitation setting? This project implemented purposeful hourly rounding by nursing staff over a three-week intervention period in one rehabilitation hospital with an average daily census of 40 male and female patients. Employee education occurred in February, 2017. Fall rates were gathered from the hospital's internal incident reporting system. Patient satisfaction scores were obtained from Press Ganey surveys. The patient satisfaction results and the fall rates of 39 patients who did not experience purposeful hourly rounding were compared to the patient satisfaction and fall rates of 68 patients who experienced the purposeful hourly rounding. This project used a quasi-experimental correlational design. An independent t-test analysis did not demonstrate a difference between the patient satisfaction scores on 10 surveys received from the first group of patients and the 5 surveys received from the patients who experienced rounding. A chi-square analysis of the fall rates did not demonstrate a statistical difference between the groups. Although the results were not statistically significant, the project provided a baseline for future projects such as identifying barriers for staff to perform purposeful hourly rounding and improving the preparation for staff to perform hourly rounding.

Patient safety and fall prevention are a top priority for nursing. Hourly or purposeful rounding has been suggested as an intervention to combat patient falls. Research shows that hourly rounding reduces patient falls, decreases call light use and helps to improve patient satisfaction scores. A systematic review of literature was conducted to determine the effectiveness of hourly rounding in preventing patient falls as oppose to no rounding or current clinical practices. Fifteen peer-reviewed research articles on the subject were examined, including 13 quantitative or qualitative studies and two literature review studies. Overall, based on the research, hourly rounding helped to reduce patient falls. Therefore, hourly rounding should be incorporated into nursing work flow as an evidence-based best practice.

Research has indicated that by regularly rounding on patients in a purposeful manner, such as addressing pain, toileting, positioning needs and placing important and personal items within reach of the patient decreases the incidences of patient falls and increases patient satisfaction (Meade, Bursell, and Ketelsen, 2006). An hourly rounding protocol already is in place at Hoag Hospital, but patient falls and hospital-acquired pressure ulcers still continue to happen. Hoag Hospital can greatly benefit by re-educating nursing staff and nursing assistants on proper hourly rounding protocol, including rationale for hourly rounding and important factors to address with patients. A chime will also be introduced to ring when hourly rounding is performed, every one hour for day shift and every two hours for night shift, to gently remind staff to round on their patients. The staff can also benefit by reinforcing the importance of hourly rounding through the yearly house side mandatory review session accessed through the hospital's intranet website. This proposal will include an implementation plan that shows the positive benefits implementing hourly rounding re-education given to nurses and nursing assistants has improved patient outcomes, specifically improved patient safety as seen by a decrease in falls, and improved patient satisfaction. In addition, evaluation of the findings as well as disseminating them to all unit department directors and the nursing community will be discussed.

Reducing falls and falls with injury is a goal for this facility. The falls metric is an important nurse-sensitive quality indicator that impacts mortality, the patient experience, and hospital length-of-stay. A pilot study conducted over two months included an interdisciplinary approach to purposeful hourly rounding and the incorporation of a purposeful pause to consider fall-prevention measures before leaving a patient's room. The intervention demonstrated an improvement in the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) overall patient experience of care score as well as an improvement in questions in the responsiveness domain. Falls on the unit slightly increased during the timeframe of the pilot, likely due to other contributing factors, as the hospital was struggling with a high registered nurse vacancy rate of 23% compared to the national average of 9.9% (NSI.com). The Coronavirus-19 (COVID-19) pandemic surge negatively impacted purposeful hourly rounding on inpatient units.
