Book excerpt: Excerpt from Pulmonary Phthisis: Its Etiology, Pathology and Treatment My Dear Dr Duncan, I have a genuine pleasure in dedicating this book to you, because I feel that thus I may in some measure express my appreciation of the teaching which, as one of your pupils, I had the good fortune to enjoy. This pleasure is, however, somewhat alloyed by the fear that the performance may be found to fall far short of what you might justly have expected. I can only beg of you to believe that I as cordially ascribe to your good influence any merit which it may be deemed to possess, as I unfeignedly attribute to my own deficient power and knowledge its many shortcomings. For this addition to the numerous works on the subject of Consumption, the apology which I offer is a very old one, viz., the trust that I may have been able to add something to the sum of our knowledge of this disease. Whether or not I have succeeded in doing this, remains to be seen; I have at any rate tried to bear in mind the maxim, that in order to do what has never been done, methods must be used which have not yet been employed. Recognising this, I have endeavoured to apply more closely than to my knowledge has been done hitherto, the ascertained facts of physiology to the elucidation of the phenomena of this disease. Jaccoud, the eminent Professor of the Ecole de Medecine, Paris, is generally recognized on the continent as one of the best authorities on pulmonary phthisis, and it has been thought that an English edition of his work would be acceptable to those interested in the subject. His views are stated with so much clearness and the reasons for their existence so fully set forth in the work itself, that it is scarcely necessary to say anything by way of introduction. I may, however, perhaps add a few words; firstly, with reference to the pathology of the disease; secondly, from the fact that the bacillus, whose presence is regarded by many as the characteristic, and even the cause, of tuberculosis, was only discovered by Dr. Robert Koch in 1882, namely, at a date subsequent to the publication of this book. With respect to the pathology of pulmonary phthisis, the author's ideas, though analogous to those which have been and are still held by many of the best authorities, are expressed in terms which necessitate a knowledge of the different opinions expressed in recent years upon this subject. Book excerpt: Excerpt from A Practical Treatise on the Pathology and Treatment of Pulmonary Consumption: Also, Remarks Upon the Most Effectual Treatment of Other Obstinate Chronic Diseases Pulmonary consumption is, undoubtedly, the consequence of a perverted condition of the general system, of a peculiar/r aha/raster; but the first process of development, that is usu ally discovered, as clearly indicating the nature of the dis ease, is the deposit or formation of a characteristic substance in the lungs. This is technically denominated tubercle, hence the name Tubercular Consumption, signifying the same thing as Phthisis Pulmonalis, and Pulmonary Consumption, but seldom used, except when physicians talk with each other, or wish to make a flourish in the presence of their patients, with out being understood. These tubercles vary, not a little, in size, form, color, and consistence. They are found no larger than millet-seeds and small peas, up to the size of a hen's egg; roundish or irregular in form; yellowish, gr'ayish, or dull dark brown; isolated or clustered in bunches; of a hard consist ence, or so soft and friable that they easily break down be tween the thumb and finger.
