Medical countermeasures--such as drugs, vaccines, and diagnostic devices--can prevent or treat the health effects of exposure, but few are currently available for many of these CBRN agents. GAO was asked to testify on the Department of Health and Human Services' (HHS) CBRN medical countermeasure development and acquisition activities. This statement focuses on (1) how HHS determines needed CBRN medical countermeasures and priorities for development and acquisition and (2) selected challenges to medical countermeasure development and acquisition. This statement of preliminary findings is based on ongoing work. To do this work, GAO examined relevant laws and presidential directives, analyzed federal agency documents and reports from advisory boards and expert groups, and interviewed officials from HHS and the Department of Homeland Security (DHS) about the processes for developing and acquiring CBRN medical countermeasures and the challenges related to those efforts. GAO shared the information in this statement with HHS. HHS provided technical comments, which GAO incorporated as appropriate.

Government-funded medical research is not always focused on countermeasures for the most serious potential threats, and it is difficult to engage pharmaceutical and biotechnology companies to develop and manufacture medical countermeasures that have a limited commercial market. At the request of the Secretary of the U.S. Department of Health and Human Services and the Assistant Secretary for Preparedness and Response, the IOM held a workshop February 22-24, 2010, to address challenges facing the PHEMCE. Workshop participants discussed federal policies and procedures affecting the research, development, and approval of medical countermeasures and explored opportunities to improve the process and protect Americans' safety and health.

Book excerpt: During public health emergencies (PHEs) involving chemical, biological, radiological, or nuclear threats or emerging infectious diseases, medical countermeasures (MCMs) (e.g., drugs, vaccines, devices) may need to be dispensed or administered to affected populations to help mitigate the human health impact of the threat. The optimal MCMs determined for use during an emergency might be U.S. Food and Drug Administration (FDA) approved but used in unapproved ways (e.g., in a new age group or against a new agent); FDA approved using animal models because human efficacy testing is not ethical or feasible; or not yet FDA approved for any indication. As part of the United States' scientific and research preparedness enterprise, there is an imperative to go "beyond the last mile" of MCM dispensing and administration to build and maintain a national capability to monitor and assess the use of MCMs (e.g., safety, compliance, clinical benefit) after they have been dispensed during PHEs. To further the discussion on this need, the Board on Health Sciences Policy of the National Academies of Sciences, Engineering, and Medicine hosted a 2-day public workshop, Building a National Capability to Monitor and Assess Medical Countermeasure Use in Response to Public Health Emergencies. The workshop, sponsored by FDA, was held on June 6â€"7, 2017, in Washington, DC. Workshop participants discussed the roles and efforts of the federal government and of relevant stakeholders with an interest in building and maintaining a national PHE MCM active monitoring and assessment capability. This publication summarizes the presentations and discussions from the workshop.

Book excerpt: A failed car bomb attempt in New York City in spring 2010 underscored the nation's vulnerability to intentional terrorist threats from explosive devices, such as conventional explosives, radiological "dirty bombs," and nuclear weapons. The blast and subsequent fires from such weapons could inflict serious thermal burns; in the case of a nuclear detonation, these injuries could affect hundreds to thousands of people. In such an attack, stabilizing individuals with burns and other injuries would be an immediate priority. Medical care for thermal burns in a mass casualty incident would require the ready availability of large quantities of medical countermeasures, such as pain medications, wound dressings, and intravenous fluids, both on-site and in emergency treatment facilities. The Department of Health and Human Services (HHS) is the federal agency primarily responsible for identifying and supporting the development and acquisition of the medical countermeasures needed to prevent or mitigate potential health effects from exposure to chemical, biological, radiological, and nuclear (CBRN) agents and other terrorist threats. In addition to identifying these countermeasures, including those for thermal burns, HHS also has responsibility for engaging with industry to research and develop them and, ultimately, for acquiring them for the U.S. Strategic National Stockpile (SNS), if appropriate. The Project BioShield Act of 2004 authorized the establishment of a procurement fund for these countermeasures. Specifically, the act authorized the appropriation of about $5.6 billion over the 10-year period from fiscal year 2004 through fiscal year 2013 for the Project BioShield Special Reserve Fund to acquire certain medical countermeasures for the SNS. Multiple organizations have raised concerns about HHS's ability to work with industry to successfully develop and acquire medical countermeasures to respond to CBRN incidents and other terrorist threats, and since 2004 congressional committees have held several hearings to assess HHS's medical countermeasure development and acquisition efforts. You requested that we examine whether HHS has developed and acquired medical countermeasures that address thermal burn injuries that would result from conventional explosives or radiological or nuclear devices. Our review addresses (1) the medical countermeasures in the SNS that would address thermal burns, (2) the steps HHS has taken to obtain information about and inform industry of its interest in additional countermeasures for thermal burns, and (3) the medical countermeasures in the pipeline for development and acquisition into the SNS that could serve to address thermal burns.
