Aufgrund des hohen medizinischen Wissensstands können immer mehr Infektionskrankheiten adäquat behandelt werden, verbesserte Hygiene, Vorsorgeimpfungen und erfolgreiche Behandlungsmethoden führen zu einem Rückgang tödlich verlaufender Krankheiten. Die Säuglingssterblichkeit liegt deutlich niedriger als noch vor einigen Jahrzehnten und zahlreiche (Früh-) Förderungsmöglichkeiten stehen zur Verfügung. Die Prävalenzrate chronischer Krankheiten stieg in den letzten Jahren um das Doppelte, ohne daß die Inzidenz sich signifikant verändert hätte. Was dazu führt, daß sich neben medizinischen und helfenden Berufsgruppen auch Pädagogen und nicht zuletzt Familien immer häufiger mit dem Problem der Bewältigung einer chronischen Erkrankung oder Behinderung konfrontiert sehen. Eine Implikation der chronischen körperlichen Erkrankung scheint eine erhöhte Prävalenz psychiatrischer Störungen zu sein. In der oft zitierten Isle oft Wight-Studie lag die Prävalenzrate für psychische Störungen in der Gruppe der Kinder von 5-14 Jahren bei 6,6 % im Vergleich zu einer Prävalenzrate von 11,6 % in der Gruppe der ohne ZNS-Beteiligung körperlich erkrankten Kinder. Neueren Untersuchungen aus den letzten 15 Jahren zufolge liegt die internationale Prävalenzrate für psychische Störungen im Kindes- und Jugendalter zwischen 28% nach den International Classification of Diseases 9 und 29% nach dem Diagnostic and Statistical Manual of Mental Disorders III oder nach DSM-III-R Kriterien. Menschen mit geistiger Behinderung sind besonders anfällig für verschiedenste Arten von Erkrankungen und Behinderungen wie Epilepsien oder Zerebralparese, und unter anderem auch für Psychopathologien in Form von schweren Verhaltensauffälligkeiten ... Im Gegensatz zu psychiatrischen Diagnosen wie Schizophrenie oder Depression steigt das Vorkommen dieser Verhaltensprobleme (zB. Fremdaggression) mit steigendem Grad der geistigen Behinderung an. Dosen geht davon aus, daß unabhängig vom Alter bei ca.30% der geistig behinderten Menschen eine zusätzliche psychische Störung im Sinne der Kriterien des ICD vorliegt. Die bisherigen Befunde sind wenig einheitlich und verschiedene Erkrankungen scheinen ein unterschiedlich hohes Risiko für psychische Probleme zu implizieren. Ebenso deuten einige Befunde darauf hin, daß die Art der Therapie einen Einfluß auf die psychische Befindlichkeit hat.

Both contemporary and in-depth, this multidisciplinary, multidimensional volume covers all available therapeutic methods, including psychopharmacotherapy, psychotherapy, behavior therapies, cognitive therapy, and the systems approach for all the main diagnostic disorders in people with mental retardation. Parts I and II present an overview of epidemiology and clinical presentation, including research trends, and therapeutic methods, including psychotherapy, psychopharmacology, behavioral therapies, cognitive and social learning treatments, and working with families and caregivers. Parts III and IV focus on psychotherapeutic interventions, such as rational emotive group treatment with dually diagnosed adults, pre-therapy for persons with mental retardation who are also psychotic, and systemic therapy, and how to apply these methods to the treatment of specific mental illnesses, such as schizophrenia, epilepsy, and mood and anxiety disorders. Parts V and VI discuss how to treat behavior disorders such as aggression/self-aggression (pharmacotherapy and strategic behavioral interventions) and self-injurious behaviors (multimodal contextual approach), including group therapy for sex offenders and a pedagogical approach to behavior problems, and which treatment methods, such as psychodynamically oriented psychotherapy and pharmacotherapy, are most effective with children, including developmental-dynamic relationship therapy with more severely mentally retarded children.
