Patient Tales follows the development of psychiatric case histories from their origins at Edinburgh Medical School and the Royal Edinburgh Infirmary in the mid-eighteenth century to the medical records of contemporary American mental health clinics. Spanning two centuries and several disciplines, Berkenkotter's investigation illustrates how discursive changes in this genre mirrored evolving assumptions and epistemological commitments among those who cared for the mentally ill. During the asylum era, case histories were a means by which practitioners organized and disseminated local knowledge through professional societies, affiliations, and journals. The way in which these histories were recorded was subsequently codified, giving rise to a genre. In her thorough reading of Sigmund Freud's Fragment of an Analysis of a Case of Hysteria, Berkenkotter shows how this account of Freud's famous patient "Dora" led to technical innovation in the genre through the incorporation of literary devices. In the volume's final section, Berkenkotter carries the discussion forward to the present in her examination of the turn from psychoanalysis to a research-based and medically oriented classification system now utilized by the American Psychiatric Association. Throughout her work Berkenkotter stresses the value of reading case histories as an interdisciplinary bridge between the humanities and sciences.

In this context, by using Lakoff and Johnson’s conceptual metaphor theory (CMT), this study uncovers the broad spectrum of the mentally ills’ experiences, a relatively undertheorised area in medical humanities. The aim is to demonstrate that mentally ill people are often represented as either grotesquely exaggerated or overly romanticised across diverse media and biomedical discourses. Further, they have been disparaged as emotionally drained and unreasonable individuals, incapable of active social engagements and against the healthy/sane society. The study also aims to unsettle the sanity/insanity binary and its related patterns of fixed categories of normal/abnormal, which depersonalise the mentally ill by critically analysing seven graphic narratives on mental illness.

In his collection of short stories and vignettes, A Five Hundred Pound Amoeba and Other Psychiatric Tales, Dr. Sobel describes how those suffering the frequently incapacitating consequences of psychiatric disorders experience their illnesses, how their loved ones are affected and how the clinician responds. By combining insight into the patients' perspectives with his psychiatric knowledge, he offers a uniquely informative and intriguing narrative. The stories provide an accurate portrayal of a diverse sample of psychiatric disorders with an emphasis on the more severe end of the spectrum such as psychotic disorders or treatment-refractory illnesses. Other stories describe disorders which are less familiar to laypeople and less commonly described in such narratives. Each story includes a “follow-up note” -- providing a glimpse into the viewpoint of the treating psychiatrist of each fictional case. Long-term outcome is revealed in the follow up note. Diagnostic and treatment considerations and other impressions are addressed. The stories are fictionalized accounts, but are consistent with Dr. Sobel's experience of actual cases.

Book excerpt: Metaphors help us understand abstract concepts, emotions, and social relations through the concrete experience of our own bodies. Conceptual Metaphor Theory (CMT), which dominates the field of contemporary metaphor studies, is centered on this claim. According to this theory, correlations in the way the world is perceived in early childhood (e.g., happy/good is up, understanding is seeing) persist in our conceptual system, influencing our thoughts throughout life at a mostly unconscious level. What happens, though, when ordinary embodied experience is disrupted by illness? In this book, Elisabeth El Refaie explores how metaphors change according to our body's alteration due to disease. She analyzes visual metaphor in thirty-five graphic illness narratives (book-length stories about disease in the comics medium), re-examining embodiment in traditional CMT and proposing the notion of "dynamic embodiment." Building on recent strands of research within CMT and engaging relevant concepts from phenomenology, psychology, semiotics, and media studies, El Refaie demonstrates how the experience of our own bodies is constantly adjusting to changes in our individual states of health, socio-cultural practices, and the modes and media by which we communicate. This fundamentally interdisciplinary work also proposes a novel classification system of visual metaphor, based on a three-way distinction between pictorial, spatial, and stylistic metaphors. This approach will enable readers to advance knowledge and understanding of phenomena involved in shaping our everyday thoughts, interactions, and behavior.
